Suprachoroidal hemorrhage associated with cataract surgery: A case series analysis.

This study aimed to identify risk factors, evaluate management strategies, and assess visual outcomes of suprachoroidal hemorrhage during cataract surgery through a retrospective analysis of five clinical cases and a literature review. We analyzed five cases of suprachoroidal hemorrhage occurring during phacoemulsification cataract surgery with intraocular lens implantation or intrascleral fixation of intraocular lens haptics at our institution (January 2013-September 2024). Among 48,725 cataract surgeries, five cases of suprachoroidal hemorrhage were identified (incidence: 0.01%). Key risk fa

Postoperative rupture of an artery dissected from a cerebral aneurysm dome following clipping: A rare and fatal complication.

The adhesion of arteries to aneurysm domes can pose significant technical challenges during surgical clipping. Dissection of these vessels carries a risk of iatrogenic wall damage and subsequent complications. We present the case of a 67-year-old woman with three unruptured intracranial aneurysms. Following successful coil embolization of a right posterior communicating artery aneurysm, surgical clipping was planned for the remaining left middle cerebral artery and anterior choroidal artery aneurysms. Intraoperatively, the M2 inferior division was found to be densely adherent to an aneurysm lo

When small aneurysms bleed: Functional outcomes and the limitations of size-based risk stratification.

BACKGROUND: Small intracranial aneurysms (≤5 mm) are often considered low risk and less clinically significant. While rupture risk has been extensively studied, the severity of clinical outcomes following rupture remains uncertain. This study assessed whether aneurysm size independently predicts functional outcome after aneurysmal subarachnoid haemorrhage (aSAH), with a focus on post-rupture morbidity. METHODS: This retrospective cohort study included 421 patients with ruptured saccular intracranial aneurysms treated at a tertiary neurovascular centre between July 2015 and June 2024. Aneurysm

Oculomotor nerve palsy caused by arterial displacement due to subarachnoid hemorrhage from a ruptured duplicated middle cerebral artery aneurysm: illustrative case.

BACKGROUND: A duplicated middle cerebral artery (DMCA) is a rare variant of the middle cerebral artery (MCA), arising between the anterior choroidal artery and distal end of the internal carotid artery. Despite its rarity, DMCA is clinically significant due to its association with aneurysm formation and high risk of rupture. No case of oculomotor nerve palsy related to DMCA aneurysm has been reported. OBSERVATIONS: A 49-year-old woman presented with sudden severe headache (Hunt and Kosnik grade III) and left pupil dilation. Imaging showed a diffuse, left predominant subarachnoid hemorrhage an

Endovascular electrocoagulation for treating a blister-like microaneurysm with an extremely narrow neck: A case report.

BACKGROUND: Intracranial blister-like microaneurysms are an extremely rare disease. Rupture of intracranial aneurysms can lead to subarachnoid hemorrhage (SAH). Patients with SAH may experience severe neurological symptoms, including severe headache, nausea, vomiting, transient or persistent loss of consciousness, limb weakness, and blurred vision. Ruptured aneurysms should be surgically treated as soon as possible. Intravascular electrocoagulation is becoming a promising treatment method for intracranial blister-like microaneurysms. The short-term follow-up results demonstrated that this meth

Successful Clazosentan Therapy for Subarachnoid Hemorrhage after Coil Embolization of Ruptured Posterior Cerebral Artery Aneurysm in a Patient with Moyamoya Disease: A Case Report.

Moyamoya disease is a progressive steno-occlusive cerebrovascular disorder that may be complicated by intracranial aneurysms, particularly, in the posterior circulation, which are at higher risk of rupture than those in the general population. Clazosentan, a selective endothelin A receptor antagonist, has been approved in Japan for the prevention of cerebral vasospasm after aneurysmal subarachnoid hemorrhage. However, cerebral hemodynamics after subarachnoid hemorrhage and safety of clazosentan in patients with moyamoya disease remain unknown. We report the case of a 46-year-old man with bilat

Choroidal Rupture Secondary to Treble Fishhook Ocular Injury.

INTRODUCTION: Choroidal rupture is a vision-threatening complication of blunt ocular trauma but is rarely reported in association with fishing-related injuries. We describe a case of choroidal rupture following ocular trauma from a fishing lure. CASE PRESENTATION: A 9-year-old boy presented with a penetrating fishhook right eye injury from a treble hook lure. Ophthalmic examination and imaging confirmed choroidal rupture. Initial visual acuity was 20/200 in the right eye and improved to 20/60 at 2 months postinjury. At 6 months, visual acuity declined to hand motions, and optical coherence to

A Ruptured Tri-Lobulated ICA-PCom Aneurysm Presenting with Preserved Neurological Function: Case Report and Clinical-Anatomical Analysis.

Background and Clinical Significance: Although rupture of aneurysms at the internal carotid-posterior communicating artery (ICA-PCom) junction accounts for a small percentage of all ruptured intracranial aneurysms, they are clinically relevant due to their proximity to perforator-rich cisterns, the optic-carotid-oculomotor pathways and flow-diverting zones, as well as their high likelihood for causing early neurological instability. Additionally, ruptured ICA-PCom aneurysms that have multiple lobulations are associated with increased variability in wall shear stress, local inflammatory remodel

Acute Angle Closure Glaucoma Secondary to Suprachoroidal Hemorrhage Triggered by Valsalva Maneuver: A Rare Case.

Spontaneous suprachoroidal hemorrhage is a rare ophthalmic emergency, typically associated with ocular surgery or trauma. However, it can occur spontaneously in anticoagulated elderly patients after a sudden rise in venous pressure, such as during a Valsalva maneuver. We report a rare case of acute angle closure glaucoma secondary to suprachoroidal hemorrhage triggered by severe vomiting in an anticoagulated patient. An 84-year-old male with atrial fibrillation on apixaban was hospitalized for biliary colic. Following repeated vomiting episodes, he experienced sudden, painful vision loss in th

Delayed Sensory Exotropia With Retained Metallic Intraorbital Foreign Body.

This report describes the longest case of a retained metallic intraorbital foreign body with no complications and development of delayed sensory exotropia following traumatic sclopetaria in childhood. A 9-year-old girl suffered a BB gun injury to the left eye, leading to chorioretinitis sclopetaria and loss of vision. The visual acuity was 20/800 with a relative afferent pupillary defect and choroidal rupture with subretinal hemorrhage that evolved to sclopetaria over time. Computed tomography of the orbit demonstrated the BB foreign body. A sensory exotropia of 30 prism diopters developed ove

Effect of subretinal injection of tissue-type plasminogen activator for the treatment of submacular hemorrhage complicating polypoidal choroidal vasculopathy.

OBJECTIVES: Polypoidal choroidal vasculopathy (PCV) is a major cause of vision loss among older adults worldwide and is particularly common in Asians. When the aneurysmal dilatation of blood vessels or the rupture of widened small veins/arteries occurs, it leads to submacular hemorrhage (SMH), which can result in irreversible damage to vision over time. This study aims to evaluate the surgical outcomes and complications of pars plana vitrectomy (PPV) combined with intraoperative subretinal injection of tissue-type plasminogen activator (t-PA) and postoperative intravitreal injection of anti-va

Coexistence of anterior choroidal artery and posterior cerebral artery retia mirabilia presenting with subarachnoid hemorrhage: illustrative case.

BACKGROUND: A rete mirabile is a rare vascular anomaly, with posterior cerebral artery (PCA) involvement being especially rare. Its pathogenesis has been speculated as a remnant of "distal annexation" between the primitive anterior choroidal artery (AchA) and the PCA at this site, but the exact mechanisms remain unclear. OBSERVATIONS: A 29-year-old man presented with subarachnoid hemorrhage. Arteriovenous malformation in the medial temporal lobe was initially suspected, but an arteriovenous shunt was not detected. First, conservative treatment was administered; however, rebleeding occurred 1

Nomograms for assessing the rupture risk of anterior choroid artery aneurysms based on clinical, morphological, and hemodynamic features.

BACKGROUND AND PURPOSE: A notable prevalence of subarachnoid hemorrhage is evident among patients with anterior choroidal artery aneurysms in clinical practice. To evaluate the risk of rupture in unruptured anterior choroidal artery aneurysms, we conducted a comprehensive analysis of risk factors and subsequently developed two nomograms. METHODS: A total of 120 cases of anterior choroidal artery aneurysms (66 unruptured and 54 ruptured) from 4 medical institutions were assessed utilizing computational fluid dynamics (CFD) and digital subtraction angiography (DSA). The training set, consisting

Rapidly growing distal choroidal artery aneurysm re-rupture following revascularization for hemorrhagic Moyamoya disease: A case report.

Intracranial hemorrhage is the leading cause of neurological deficits and poor prognosis in adult patients with Moyamoya disease (MMD). Intracranial hemorrhage is occasionally accompanied by MMD-associated aneurysm and requires additional treatment. To date, direct or indirect bypass surgery or endovascular treatment, such as coil embolization, has been adopted and has achieved successful outcomes. The rapid growth of MMD-associated aneurysms and rebleeding after direct bypass surgery via superficial temporal artery-middle cerebral artery (STA-MCA) anastomosis has rarely been reported. We repo

Arachnoid cyst in the pediatric patient: What the radiologist needs to know.

Arachnoid cysts are the most common incidentally discovered intracranial lesions on imaging and the most common cystic intracranial lesions. They may be developmental or secondary. A relative lack of recent literature and any comprehensive radiological review on arachnoid cysts has led to a general lack of awareness among radiologists of symptomatic or complicated arachnoid cysts. This is particularly concerning in pediatric patients. While arachnoid cysts are asymptomatic in most cases, they can cause clinical symptoms in a minority of cases, especially when they occur in unusual sites. These
