Vitreous Hemorrhage Induced by a Persistent Hyaloid Artery.

BACKGROUND: The hyaloid artery is a branch of the ophthalmic artery and part of the fetal hyaloid vascular system; failure of these fetal vessels to regress can lead to a persistent hyaloid artery (PHA). PHA can induce complications, including amblyopia, cataracts, glaucoma, vitreous hemorrhage, and retinal detachment. We present a case of vitreous hemorrhage secondary to traction of the PHA in an infant treated for threshold retinopathy of prematurity (ROP). CASE PRESENTATION: A 660-g female infant was born at 23 weeks of gestation and was found to have bilateral Stage 3 Zone 2 ROP with plus

A Historical Review of Encircling Laser Retinopexy as a Prophylaxis for Rhegmatogenous Retinal Detachment; and a Commentary on Recent Progress in Stickler Syndrome.

Rhegmatogenous retinal detachment (RD) is typically caused by retinal tears anterior to the equator, usually occurring in normal appearing retina. We here review the chronological history of encircling (360-degree) laser retinopexy to the peripheral retina as a stand-alone prophylaxis for retinal tears in asymptomatic eyes determined to be at high risk. We describe recent strong proof that such encircling prophylaxis prevents RD in Stickler syndrome, the highest risk predisposing condition known. Finally, we note that peripheral vitreous traction tears are the common pathogenesis of RD in both

Update and review of the current medical and surgical management of sickle cell retinopathy.

BACKGROUND: Sickle cell retinopathy (SCR) is a well-documented and potentially vision-threatening presentation of sickle cell disease (SCD). In this article, we provide a comprehensive review of the management options for non-proliferative sickle cell retinopathy (NPSR) and proliferative sickle cell retinopathy (PSR) based on the existing ophthalmic literature. MAIN TEXT: The mainstay of NPSR treatment focuses on preventing progression to PSR by identifying and altering modifiable risk factors. Once NPSR progresses to PSR, suppression of vascular endothelial growth factor (VEGF) expression wi

Pars plana vitrectomy for tractional and combined tractional rhegmatogenous retinal detachment in type 1 diabetes mellitus: outcomes, prognostic factors, and a proposed staging system.

BACKGROUND: Pars plana vitrectomy (PPV) in young type 1 diabetes mellitus (T1DM) patients presents greater surgical challenges than in type 2 (T2DM) cases. Despite advances in vitreoretinal surgery, there is no universally accepted classification system for tractional retinal detachment (TRD), and limited data exist on the comparative effectiveness of endotamponade agents and the prognostic significance of outer retinal biomarkers in T1DM cohorts. This study evaluates PPV outcomes for TRD and combined tractional rhegmatogenous retinal detachment (TRRD) in T1DM, introduces a proposed staging sy

Neurodegeneration and neuroprotection in retinal detachment.

Retinal detachment (RD) occurs when the neurosensory retina separates from the retinal pigment epithelium (RPE). The most frequent type, rhegmatogenous retinal detachment (RRD), is caused by full-thickness retinal breaks that typically arise from vitreoretinal traction during posterior vitreous detachment. These breaks permit fluid to enter the subretinal space, leading to acute and often severe visual loss. Key risk factors include aging, myopia, pseudophakia, and the occurrence of posterior vitreous detachment. RRD constitutes a surgical emergency. Modern vitreoretinal procedures achieve hig
