Giant Retinal Pigment Epithelium Tear Secondary to Hypotony After Trabeculectomy for Open-Angle Glaucoma With Preoperative Untreated Choroidal Neovascularization: A Case Report.

Retinal pigment epithelium (RPE) tears are frequently observed following anti-vascular endothelial growth factor therapy for age-related macular degeneration. While rare, giant RPE tears have also been reported secondary to choroidal detachment (CD) induced by postoperative hypotony after trabeculectomy (TLE). Although mechanical stress on the RPE is considered a common underlying factor in both scenarios, the exact mechanisms remain unclear. This report describes a case of a giant RPE tear originating from the site of a previously untreated choroidal neovascularization (CNV) following TLE. A

Longitudinal Progression of Horseshoe Retinal Tear-Associated Rhegmatogenous Retinal Detachment Without Clinically Evident Complete Posterior Vitreous Detachment: A Case Report.

Posterior vitreous detachment (PVD) is considered a key event in the pathogenesis of rhegmatogenous retinal detachment (RRD), and horseshoe retinal tears are generally associated with vitreoretinal traction during PVD evolution. We report the longitudinal progression of horseshoe retinal tear-associated RRD without clinically evident complete PVD. A 64-year-old nonmyopic man presented to a general ophthalmology clinic with floaters and preserved visual acuity. Retrospective review of baseline ultrawidefield (UWF) imaging demonstrated a horseshoe retinal tear with localized macula-on temporal R

Role of Autotaxin in the Pathogenesis of Retina Ischemia and Its Therapeutic Implications.

Retinal vein occlusion (RVO) is a common vascular disease that leads to vision loss due to macular edema (ME). This study investigated the role of autotaxin (ATX), a lysophospholipase D, in the pathogenesis of RVO. In mice, RVO was induced by intravenous administration of rose bengal followed by laser irradiation of retinal veins. ATX expression in the retina was evaluated using immunohistochemistry. Intravitreal ATX was administered, and retinal changes were assessed using fluorescence angiography and optical coherence tomography (OCT). In human retinal microvascular endothelial cells (HRMECs

Bilateral Serous Retinal Detachment Associated With Nonhypertensive Preeclampsia: A Case Report.

Preeclampsia is a pregnancy-specific multisystem disorder characterized by new-onset hypertension and proteinuria, which may present with a variety of other maternal organ dysfunctions after 20 weeks of gestation. Although ocular complications are uncommon, serous (exudative) retinal detachment (SRD) can occur, especially in association with severe preeclampsia or HELLP (hemolysis, elevated liver enzymes, low platelets) syndrome. While SRD may cause some discomfort for the patient due to decreased visual acuity resulting from its central location, full recovery is the rule, within a maximum of

The choroid as a vascular sensor in pulmonary hypertension: an oculomic mini review.

BACKGROUND: Pulmonary hypertension (PH) is increasingly recognized as a systemic vascular disorder with extrapulmonary end-organ involvement. Within oculomics, ocular imaging biomarkers are explored as systemic disease surrogates reflecting microvascular pathology. Owing to its high flow, fenestrated vasculature, and venous drainage pattern, the choroid is particularly susceptible to venous congestion and impaired ocular perfusion pressure. Optical coherence tomography (OCT) and OCT angiography (OCT-A) enable non-invasive quantitative assessment of choroidal structure, vascularity, and perfusi

Multifocal Posterior Pigment Epitheliopathy Complicated by Circumferential Choroidal Detachment Treated With Fluorescein Angiography-Guided Focal Laser Photocoagulation: A Case Report.

Multifocal posterior pigment epitheliopathy (MPPE) is a pachychoroid-related disorder characterized by choroidal hyperpermeability and serous retinal detachment (SRD), typically without choroidal detachment (CD). We report a rare case of MPPE complicated by circumferential CD in a man in his late 50s with chronic myeloid leukemia in molecular remission. Fundus examination revealed multiple yellowish-white exudative lesions with SRD at the posterior pole and peripheral CD. Fluorescein angiography (FA) demonstrated multiple focal leakage points with progressive dye leakage, while indocyanine gre

Anti-vascular endothelial growth factor therapy in uveitis: Bridging inflammation and vascular pathology.

Anti-vascular endothelial growth factor (VEGF) therapy has emerged as an important adjunct in the management of uveitis, particularly for complications driven by vascular permeability and ischemia, such as uveitic macular edema, inflammatory choroidal neovascularization, and retinal neovascularization (RNV). By directly neutralizing VEGF, these agents offer rapid anatomical improvement and visual stabilization, especially in patients who are steroid-intolerant or present with refractory disease. Although current evidence is largely derived from small series and observational data, the accumula

Mechanical Stress Induces VEGF Expression and RPE Disruption in Mouse Eyes.

Mechanical stress has been implicated in retinal pigment epithelium (RPE) dysfunction and angiogenic signaling in retinal disorders; however, its direct in vivo effects on the RPE-choroid complex remain incompletely understood. Here, we established a mouse model of localized mechanical stress by subconjunctival implantation of glass beads (0.8-1.2 mm in diameter) in eight-week-old C57BL/6J mice to induce transscleral stretching of the RPE. Ocular tissues were analyzed two days after implantation using histological, immunohistochemical, and molecular approaches, and inflammatory mediators were
