A 42-year-old woman with sickle cell anemia and proliferative retinopathy underwent neodymium-YAG laser therapy for a taut posterior hyaloid membrane causing peripapillary and peripheral traction detachment of the retina. Six cases of recurrent vitreous hemorrhage are described. The hemorrhage was due to a retinal vessel rupture or injury caused by vitreous traction. Twenty-five cases of retinal detachment were treated pre-operatively by the so-called active immobilization method described by Rosengren and Algvère. This procedure can be employed for all types of detachment, and gives significant reapplication in 80 p. cent of cases, simplifying subsequent surgery. Eighteen patients (19 eyes) with sickling hemoglobinopathies underwent vitrectomy or scleral buckling operations. A single intravitreal injection of fluorouracil was effective in the treatment of an experimental model of massive periretinal proliferation. Six consecutive cases of retinal detachment with macular hole were treated successfully by vitrectomy, partial gas-fluid exchange, and prone positioning of the patient for 12 hours. Macular coagulation and macular buckling were not necessary. In five cases the retina remained flat during follow-up periods ranging from one to 16 months.
