Vitreous surgery techniques are useful in the management of selected retinal detachments with an otherwise poor prognosis. Vitrectomy methods can be used to remove intraocular opacities; cut vitreous sheets causing vitreo-retinal traction and separate epiretinal membranes covering or distorting the retina; provide a fluid space to aid in creation of a scleral buckle or use of an intravitreal gas bubble; perform other intraocular procedures, including transvitreal treatment of retinal breaks, transvitreal removal of subretinal fluid, and intraocular fluid-gas exchange. These surgical capabiliti

A technique has been developed which produces an experimental posterior penetrating eye injury that reproducibly results in traction retinal detachment in rabbit eyes. The standard injury is an incision through the pars plana with vitreous prolapse and incarceration; the wound is then carefully closed with microsurgical techniques. It appears that blood in the vitreous is an essential factor in the development of vitreous traction and of traction retinal detachment. The model, which has been successfully transferred to Rhesus monkey eyes, is considered useful for further histological, electrom

We developed an experimental model for a posterior penetrating eye injury that resulted in traction retinal detachment in 21 rhesus monkey eyes. The standard injury was an incision through the pars plana with vitreous prolapse and incarceration; the wound was then carefully closed with microsurgical techniques. At one to two weeks after injury, detachment of the posterior vitreous occurred and detachment of the retina occurred between seven and 11 weeks. The development of retinal detachment was related to traction on the peripheral retina over the vitreous base; and to the contraction of epir

A delay in absorption of subretinal fluid after surgical repair for rhegmatogenous retinal detachment beyond 6 weeks was recognized in 39 of 575 consecutive cases undergoing scleral buckling procedures. The most common preoperative condition that was identified in these eyes was large clumps of cells on the undersurface of the detached retina. These cream-colored aggregates appeared similar to mutton-fat keratic precipitates and are referred to as subretinal precipitates. Most likely caused by aggregates of macrophages, they were present in 12 percent of the overall series of cases; subretinal

Pars plana vitrectomy was used to treat 134 consecutive eyes with complications resulting from proliferative diabetic retinopathy, including nonabsorbing vitreous hemorrhage, recent traction and rhegmatogenous retinal detachment, progressive fibrovascular proliferation, and early rubeosis iridis with recent vitreous hemorrhage. Vision improved after surgery in 65% of the eyes, was unchanged in 16%, and decreased after surgery in 19%. In 24% of the successful cases, vision of 20/50 or better was achieved. When all posterior cortical vitreous was removed, no preretinal fibrovascular growth occur

Forty-two eyes in 30 patients with disc neovascularization were treated by the method of extensive photocoagulation of the retinal periphery (ablation). Total obliteration of disc neovascularization was accomplished in 76% of eyes treated. Partial regression was noted in 21% of eyes treated. There were only 2 major complications--traction retinal detachment and one major hemorrhage--in this series. Feeder-frond treatment was necessary to obliterate recurrent disc neovascularization in 2 eyes. It is concluded that therapy is efficacious in the treatment of papillary neovascularization.

Proliferative neovascular retinopathies may be caused by the release of a hypothetical vasoproliferative factor, but mechanical factors seem to self-perpetuate the disease as well. Contraction of proliferative tissue causes vitreous detachment and traction on the retina. Congestion of vascular fronds by traction further stimulates vascularization. Traction on vessels results in hemorrhages. Surgical removal of all intravitreal scaffolds by vitrectomy seems to eliminate the development of this vicious cycle. Results of vitreous surgery seem to indicate that the hypothesis of scaffold removal is

Holes of the peripheral retina, defined as full-thickness breaks of trophic origin with no associated flap or free operculum, were found in 136 (2.4%) eyes from 2,800 autopsied subjects. Primary retinal holes (those with no indication of a proximal causative lesion and with no lattice degeneration in either eye) occurred in only eight of the 5,600 eyes studied; all were unilateral, single, less than 0.25 disk diameter in size, within the basal zone, and in eyes from elderly subjects. Secondary holes were found in 128 (2.3%) of eyes and of these, lattice degeneration was the most common cause (

Closed vitrectomy techniques provide new capabilities to treat severe penetrating ocular injuries involving posterior segment structures. With these methods damaged iris tissue, lens, and vitreous can be removed and intraocular opacities can be cleared to identify and treat any retinal damage. Removal of damaged vitreous also prevents later cyclitic membrane formation and traction on the retina. The vitrectomy technique can be combined with additional methods to treat other conditions including nonmagnetic intraocular foreign bodies. We present the rationale and technique of vitrectomy togethe

The records of 74 patients--young and almost exclusively males--with retinal detachment due to ocular penetration were reviewed to study the characteristics and surgical results of this type of traumatic retinal detachment. There was a high incidence of industrial and domestic accidents. The incidence and degree of myopia were significantly lower than in nontraumatic cases of retinal detachment. The time interval between the injury and the detection of the retinal detachment was variable. Many of the eyes with longer latent intervals had classic signs of traumatic retinal detachment. The most

The retinal periphery of 60 premature children now between 5 and 8 years of age and with a weight at birth equal or less than 1,500 g has been investigated. In 20% of these children, pigmentary modifications were noted which were often associated with chorioretinal atrophy. Polymorphic vascular abnormalities were seen in 14% of the cases, i.e. sinuous vessels of irregular caliber or neovascularization. Abnormalities of the vitreous body were also noted in 12% of the patients with glial bags on the periphery and sometimes potentially dangerous tractions folds (4%). All these abnormalities could

Cases of retinal detachment due to retinopathy of prematurity are presented. 1. There are cases of total retinal detachment without a complete retrolental fibroplasia. 2. Very typical are cases with a dragged disc and mascular ectopy due to preretinal strands in the temporal periphery, the symptoms of an earlier retinopathy of prematurity. By traction these temporal alterations can cause a retinal detachment, the distortion of the retinal vessels can heavily increase after surgical reattachment of the retina. 3. There are cases of juvenile myopes without further signs of a retinopathy of prema

40 eyes with retinal detachment were immobilized by traction sutures under the inferior and medial rectus muscles in order to study the rate of "spontaneous" reattachment following such fixation. There was an almost complete reattachment in 45% of cases, and partial reattachment (at least half of the detached area) in 40%. Little or no reattachment was observed in 15% of cases. Reattachment of the retina prior to the operation is considered advantageous for the success of surgery.

In order to open the scleral incision during the whole length of microsurgical drainage of the subretinal fluid, a simple retractor was designed which allows the edges of the sclerotomy wound to spread out without the additional help of the surgeon's or assistant's hands. By elevating the instrument, traction release of subretinal fluid is also possible, even in areas far behind the equator.

We have presenteed a late follow-up study of 8 eyes previously presented which were treated with scleral buckling procedures under hyperbaric oxygen conditions for repair of rhegmatogenous retinal detachments complicating sickle cell disease. In addition, we have presented a patient treated under similar conditions with a scleral shortening procedure and drainage of subretinal fluid for repair of a non-rhegmatogenous traction detachment secondary to diabetic retinopathy and sickle disease. We feel that our successful results in these cases and our lack of the usually reported complications mer

Neovascularization of the retina occurs in several types of sickle cell hemoglobinopathies and is highly characteristic in appearance. It arises in the equatorial plane of the fundus following preliminary arteriolar occlusions and arteriolar-venular anastomoses. Neovascular sea fans arise at the interface of perfused and nonperfused portions of the retina, starting during the first decade of life and continuing for many years thereafter. Although some sea fans undergo spontaneous autoinfarction, most show progressive growth and constant intravitreal transudation of plasma components. Traction

A series of 69 eyes in which silicone oil injection was used to treat retinal detachment with massive preretinal retraction was reviewed with respect to retinal re-attachment and visual function. The selection of patients for treatment with silicone oil injection was influenced by the poor prognosis with conventional methods, the high incidence of postoperative complications after silicone oil injection, and the prognosis of the fellow eye. Although silicone oil injection is not an ideal method of re-attaching the retina, it remains the only successful method in many cases of massive preretina

The vitreous cavity may be invaded by cells penetrating through dehiscences in the inner limiting lamina of the retina, and exaggerated vitreo-retinal adhesions may also develop at such sites. The vitreous often separates from the inner limiting lamina except at these anomalous adhesions, and retinal traction thus results from excessive movement of the otherwise detached gel; traction on the retina may also follow proliferation and contraction of invading cells on the retinal surface, along the posterior hyaloid interface, or traversing the gel. Traction complications are liable to occur at si

Seven retinal breaks occuring after occlusion of a major retinal arterial or venous branch were found in the eyes of six patients. Five holes were in the midretina, in a nonperfused avascular area corresponding to the sector affected by the vascular accident. One was a macular hole, and one a horseshoe tear outside the area involved in the vascular occlusion caused by traction of vitreous on a tuft of neovascular tissue. Five of the holes may have been caused by a kind of sequestration of the retina secondary to vascular insufficiency.

We observed 341 patients who had received cryotherapy for lattice degeneration in order to identify possible late complications. Sequelae such as retinal tears posterior to an operculum or flap tears within treated areas showed that treatment did not necessarily prevent subsequent vitreous traction. Moreover, the newly created flap tears may extend beyond the treated area and can cause retinal detachment. Even scleral buckling did not necesserily prevent further traction. Therefore, we concluded that when cryotherapy is used to treat lattice degeneration, an adequate margin of surrounding reti
