BACKGROUND: Vitreomacular traction is characterized by incomplete posterior vitreous detachment, wherein persistent adherence of the vitreous to the macula results in tractional distortion and associated visual symptoms. Although spontaneous resolution of vitreomacular traction may occur, surgical intervention is typically pursued when significant visual impairment occurs. The mechanism by which spontaneous release of vitreomacular traction is unclear, as there are numerous intrinsic and extrinsic factors that affect the eye. This case highlights the potential effects of sudden changes in baro

BACKGROUND: Endotamponade selection is best understood as a physics-informed, phenotype-based decision rather than one driven mainly by tamponade duration or material class. This narrative review integrates density, interfacial tension, contact angle, viscosity, and drug-tamponade interactions with phenotype-specific clinical outcomes. The result is a practical framework linking tamponade behavior to retinal geography, disease complexity, and postoperative feasibility.
MAIN BODY: Lighter-than-water agents (air, expansile gases, and light silicone oils [LSOs]) preferentially support the superi

PURPOSE: To determine risk factors for retinal breaks (RB) during the surgical induction of posterior vitreous detachment (IPVD) in macula surgery.
METHODS: We retrospectively reviewed electronic records for all adult patients undergoing 25-G or 27-G vitrectomy for macula pathology between November 2019 and January 2024 requiring intraoperative IPVD. We excluded eyes with pre-existing PVD, panretinal photocoagulation, trauma, age < 18 years or missing information. The outcome measure was non-sclerotomy-related iatrogenic RB. We assessed the effect of age, gauge size (25-G vs 27-G), surgical i

BACKGROUND: Diabetic tractional retinal detachment (TRD) secondary to proliferative diabetic retinopathy is a vision-threatening complication that may require pars plana vitrectomy in selected cases, particularly when the macula is threatened or detached. Silicone oil tamponade is commonly used in complex cases, although tamponade choice varies according to surgical findings and surgeon preference.
OBJECTIVES: To evaluate surgical outcomes associated with adjunctive dexamethasone implant during pars plana vitrectomy with silicone oil or heavy silicone oil tamponade for diabetic tractional ret

Bioadhesive materials have been successfully applied across several ophthalmic fields, yet their translation into vitreoretinal surgery for the repair of retinal breaks and defects remains limited. Conventional strategies, including gas and silicone oil tamponade, continue to dominate retinal stabilization; however, they primarily provide temporary cavity-based support rather than direct interfacial closure of retinal defects and may require postoperative positioning, secondary interventions, or carry tamponade-related complications. Despite advances in biomaterial engineering, broader transla

BACKGROUND: Proliferative diabetic retinopathy (PDR) is a leading cause of irreversible blindness worldwide, characterized by pathological neovascularization and progressive retinal fibrosis. Although anti-VEGF therapy effectively suppresses neovascularization, it does not address fibrovascular membranes (FVMs) formation and may paradoxically accelerate membrane contraction, increasing the risk of tractional retinal detachment. The lack of effective anti-fibrotic strategies highlights the importance to elucidate the key cellular mechanisms underlying retinal fibrosis in PDR.
