Meta-analysis: long/short-term efficacy of anti-VEGF vs. panretinal photocoagulation in preventing severe complications in proliferative diabetic retinopathy.

BACKGROUND: Studies diverge on the relevance of long-term protection of anti-VEGF against severe proliferative diabetic retinopathy (PDR) complications compared to pan-retinal photocoagulation (PRP). We aim to assess this dispute through a meta-analysis. METHODS: We searched PubMed, Embase, and Cochrane databases until August 2024 for studies comparing anti-VEGF with PRP in PDR. Primary outcomes were long-term and short-term incidences of VH, TRD, and PPV-setting short-term follow-up up to 2 years and long-term follow-up over 5 years. Due to a lack of consistent data, TRD events were not stra

Vitreoretinal interface abnormalities in age-related macular degeneration: Prevalence, pathophysiology, and reciprocal influence.

Age-related macular degeneration (AMD) represents a leading cause of vision loss worldwide, while vitreoretinal interface (VRI) abnormalities constitute a dynamic boundary where posterior vitreous interacts with the retinal surface. We explore the intricate relationship between VRI abnormalities and AMD, examining prevalence, underlying pathophysiological mechanisms, and their reciprocal influence on disease development, progression, and treatment outcomes. Evidence suggests a higher prevalence of vitreomacular adhesion in exudative versus nonexudative AMD, while complete posterior vitreous de

Vitrectomy as a Prevention for Retinal Detachment in High-Risk Eyes.

Rhegmatogenous retinal detachment (retinal detachment, RD) is the most common cause of spontaneous vision loss in aging eyes that were seeing normally until the moment of its occurrence. Causative retinal breaks usually occur in the form of peripheral tears at the vitreous base, resulting from traction exerted by the aging vitreous as it detaches posteriorly. Peripheral encircling laser retinopexy increases retinal resistance to vitreous traction in high-risk eyes. To achieve maximal lifetime prophylaxis, we show that in selected high-risk cases the vitreous can thereafter be removed by advanc

Pathogenic mechanisms and treatment advances in proliferative vitreoretinopathy: A review.

Proliferative vitreoretinopathy (PVR) is the most common cause of repair failure in rhegmatogenous retinal detachment and a severe complication of posterior segment penetrating injuries. The pathogenesis of PVR is complex, and there is currently no ideal therapeutic drug in clinical practice. Surgical intervention involves the removal of proliferative membranes to relieve traction on the retina, but it carries a certain postoperative recurrence rate. This review provides an overview of the pathogenesis and treatment strategies for PVR.

Retinal morphological changes detected by wide-field optical coherence tomography in eyes after vitrectomy for proliferative diabetic retinopathy.

PURPOSE: Proliferative diabetic retinopathy (PDR) is a severe vitreoretinal disease that can lead to significant visual impairment. Vitrectomy is commonly performed in advanced cases; however, postoperative complications, such as proliferative membrane formation and retinal detachment may still occur. Early detection of these complications is essential for timely intervention and prevention of further vision loss. Retinal morphological changes, including tractional retinal detachment due to re-proliferation, may develop after vitreoretinal surgery. This study aimed to evaluate postoperative re
