Systemic lupus erythematosus (SLE) or Lupus is often called the "great mimicker" due to its ability to mimic other systemic disorders. SLE is a multisystem autoimmune disorder with varied manifestations, including ocular involvement. Retinal vascular occlusions are an uncommon but potentially vision-threatening presentation of lupus retinopathy. When coexisting systemic features mimic infectious etiologies such as tuberculosis (TB), diagnosis becomes challenging, delaying appropriate immunosuppressive therapy. We report a diagnostically complex case of a 22-year-old female who presented with s

Central retinal artery occlusion (CRAO) is a vision-threatening ophthalmic emergency requiring rapid recognition and evaluation. Although embolic etiologies predominate, low-flow ischemia related to hemodynamic instability represents an underrecognized mechanism. We report the case of a 66-year-old woman with end-stage renal disease (ESRD) on hemodialysis who experienced recurrent transient right-eye visual symptoms near the end of dialysis sessions. She had chronic intradialytic hypotension, with systolic blood pressures ranging from 90 to 100 mmHg and post-dialysis pressures averaging 90/50

INTRODUCTION: The diagnosis of acute central retinal artery occlusion (CRAO) and branch retinal artery occlusion (BRAO) is often delayed or missed in emergency departments (EDs) because of limited ocular funduscopic skills and lack of immediate ophthalmology access. We evaluated the clinical impact of our Eye-Stroke protocol using non-mydriatic ocular imaging (non-mydriatic fundus photography and optical coherence tomography [NMFP-OCT]) in our general ED with remote interpretation by ophthalmology on the time to diagnosis and emergent management of acute CRAO/BRAO.
METHODS: Prospective consec

BACKGROUND: Central retinal artery occlusion (CRAO) is an ophthalmic emergency with poor visual prognosis. Although intravenous thrombolysis (IVT) within 4.5 h may be beneficial, the effectiveness and safety of low-dose alteplase remain uncertain.
METHODS: We conducted a retrospective study at three centres in Japan, enrolling patients who presented within 24 h of onset with CRAO or macula-involving branch retinal artery occlusion (BRAO) between June 2021 and September 2024. Patients were analysed if they had baseline best-corrected visual acuity (BCVA) < 20/400, clearly defined symptom onset

PURPOSE: Neovascular glaucoma (NVG) is a refractory secondary glaucoma, yet longitudinal outcome data remain limited. This study reports its aetiologies, treatments and outcomes.
METHODS: Patients with NVG were identified through electronic medical record review. Inclusion required documented rubeosis of the iris and/or anterior chamber angle, an intraocular pressure (IOP) ≥25 mmHg, diagnosis during 2008-2024, and follow-up at Helsinki University Hospital. Baseline data and all follow-up visits were included.
RESULTS: Of 919 patients identified, 626 met inclusion criteria, with a median foll

We report a fatal case of a 40-year-old man who developed rhino-orbito-cerebral mucormycosis complicated by central retinal artery occlusion, cavernous sinus thrombosis, and internal carotid artery occlusion. Mechanical thrombectomy was performed for the internal carotid artery occlusion, with subsequent Sanger sequencing of the fungal components within the retrieved thrombus to identify Rhizopus arrhizus. To the best of our knowledge, this is the first report establishing the species-level identification of R. arrhizus in mucormycosis-associated large-vessel occlusion. These findings provide

PURPOSE: The term white dot syndromes (WDS) has historically grouped multiple non-infectious posterior uveitis (NIPU) entities based on a similar funduscopic appearance of "white dots." Despite decades of use, the clinical relevance of this umbrella terminology has been questioned. This perspective critically examines whether WDS remains a valid conceptual and diagnostic construct in the era of advanced retinal and choroidal imaging.
DESIGN: Perspective review.
METHODS: Critical interpretation of the available literature on imaging and current pathophysiological evidence, combined with obser

PURPOSE OF REVIEW: Optical coherence tomography angiography (OCTA) has rapidly evolved from an experimental imaging technique into a routine component of many retina practices. By providing noninvasive, depth-resolved visualization of retinal and choroidal vasculature without dye injection, OCTA promises safer, faster, and more detailed assessment of retinal vascular disease than fluorescein angiography. However, there is still debate about its clinical applicability and its role in everyday ophthalmology practice, which we review in this article.
RECENT FINDINGS: Over the past decade, OCTA h

PURPOSE: To develop and validate a deep learning model for simultaneous detection of nine fundus conditions from a single color fundus photograph.
METHODS: A development dataset of 236,242 color fundus images was assembled from 17 heterogeneous sources and partitioned at the patient level into training (70%), tuning (10%), and internal validation (20%) sets; 5-fold cross-validation was applied on the training set. Nine target conditions were labeled using standardized or photographic criteria: diabetic retinopathy (DR), age-related macular degeneration (AMD), myopic macular degeneration (MMD)

Systemic sclerosis (SSc), also known as scleroderma, is a rare autoimmune disease characterized by fibroproliferative changes, vascular dysfunction, and small-vessel proliferation. It can affect multiple organ systems, including the gastrointestinal tract, skin, heart, eyes, kidneys, and lungs. We report a case of a 67-year-old woman with systemic sclerosis who tested negative for specific antibodies and simultaneously developed two rare complications: autoimmune atrophic gastritis resulting in megaloblastic anemia and acute retinal artery occlusion. Notably, she presented with megaloblastic a

PURPOSE: It is not clear whether vasopressin versus norepinephrine changed mortality in clinical practice in the Vasopressin and Septic Shock Trial (VASST) coordinating center hospital after VASST was published. We tested the hypothesis that vasopressin changed mortality compared to norepinephrine using propensity matching of vasopressin to norepinephrine-treated patients in the VASST coordinating center hospital before (SPH1) and after (SPH2) VASST was published.
METHODS: Vasopressin-treated patients were propensity score matched to norepinephrine-treated patients based on age, APACHE II, re

PURPOSE: To report the unusual case of a herpes zoster ophthalmicus (HZO) complicated by central retinal artery occlusion and viral encephalitis.
CASE REPORT: A 65-year-old male with HZO who underwent intravenous acyclovir treatment complained of sudden onset blurry vision of his left eye. Central retinal artery occlusion (CRAO) was revealed. We administered methylprednisolone pulse therapy, yet resulted in no visual acuity improvement.
CONCLUSION: There were only a few cases reported to have CRAO after diagnosed HZO. This cause of viral-related CRAO led to a complicated treatment decision.

BACKGROUND: Postoperative complications and delayed physical recovery are significant problems following emergency abdominal surgery. Physiotherapy aims to aid recovery and prevent complications in the acute phase after surgery and is commonplace in most first-world hospitals. Despite ubiquitous service provision, no well-designed, adequately powered, parallel-group, randomised controlled trial has investigated the effect of physiotherapy on the incidence of respiratory complications, paralytic ileus, rate of physical recovery, ongoing need for formal sub-acute rehabilitation, hospital length

Central retinal artery occlusion (CRAO) is a rare but vision-threatening complication following retinal detachment surgery. We report a man in his late 60s who underwent combined phacoemulsification with intraocular lens implantation and segmental scleral buckling for macula-sparing rhegmatogenous retinal detachment caused by inferotemporal ora dialysis. Shortly after surgery, he developed recurrent ocular pain with marked intraocular pressure (IOP) spikes, anterior chamber shallowing, iris bombe, and secondary angle closure. Approximately 3 h post-operatively, visual acuity deteriorated to no

Macular disease is the leading cause of vision loss in the United Kingdom, affecting nearly 1.5 million people. Vascular endothelial growth factor (VEGF) has a major role in the onset and progression of vision-threatening macular conditions such as neovascular age-related macular degeneration, diabetic macular oedema and retinal vein occlusion. Anti-VEGF treatments, given by intravitreal injection, can prevent vision loss and may improve vision but must be started quickly before sight loss is irreversible. Informed by an observational study using Hospital Episode Statistics and the Emergency C

Central retinal artery occlusion is rare in pediatrics and typically linked to embolic or hematologic disorders. The authors describe a 13-year-old boy with acute central retinal artery occlusion and elevated Bartonella henselae titers following kitten exposure. Extensive evaluation was unrevealing, suggesting a rare inflammatory association between cat scratch disease and pediatric retinal arterial ischemia. The main take-away from this case is to also look for infectious causes, which are rare and not the first to be investigated for during central retinal artery occlusion.

BACKGROUND: Attention deficit hyperactivity disorder (ADHD) is a common and growing problem and a leading cause of child referrals to Child and Adult Mental Health Services (CAMHS). It is a drain on resources across nationally funded support agencies and associated with negative outcomes such as early criminality, school disruption and antisocial behaviour. Mainstream interventions (pharmacological and behavioural) demonstrate effectiveness whilst implemented, but are costly, often have unwanted side effects and do not appear to be affecting long-term outcomes.Development of a robust evidence

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm
