Perioperative central retinal artery occlusion after prone posterior cervical spine surgery treated with delayed hyperbaric oxygen therapy: a case report and targeted literature review.

BACKGROUND: Perioperative central retinal artery occlusion (CRAO) is a rare but potentially devastating cause of visual loss after non-ocular surgery. Prone spinal and posterior cervical procedures may increase ocular risk through local orbital compression, impaired venous drainage and reduced ocular perfusion. Management is particularly difficult after surgery, when diagnosis may be delayed, conventional reperfusion strategies may be impractical or relatively contraindicated, and the role of delayed hyperbaric oxygen therapy (HBOT) remains uncertain. CASE PRESENTATION: A 53-year-old man with

Retinal Vaso-Occlusive Complications Associated with Systemic Lupus Erythematosus: A Systematic Review of Clinical Presentation and Visual Outcomes.

BACKGROUND: Ophthalmic involvement in systemic lupus erythematosus (SLE), particularly retinal vascular diseases, remains under-recognised despite its potential to cause severe visual impairment. This systematic review aims to evaluate the clinical presentation, pathophysiology, and treatment outcomes of retinal vascular diseases associated with SLE. METHODS: A comprehensive literature search was conducted across PubMed/MEDLINE, ScienceDirect, Cochrane and Google Scholar using a combination of relevant MeSH terms. Data were extracted independently and quality assessment was conducted using th

Case Report: Optic nerve infiltration associated with sequential CRVO and CRAO in isolated CNS relapse of AML.

Optic nerve infiltration as the initial manifestation of isolated central nervous system (CNS) relapse in acute myeloid leukemia (AML) has rarely been reported. We present the case of a 70-years-old man with AML in clinical remission who presented with the sudden onset of a visual field defect and vision loss in the left eye. Initially the patient was diagnosed with non-arteritic anterior ischemic optic neuropathy (NAION) at a local hospital based on findings of decreased vision, optic disc edema and an inferior sectoral visual field defect connected to the physiological blind spot in the left

Central Retinal Artery Occlusion Diagnosed via Ocular Point-of-care Ultrasound: Case Report.

INTRODUCTION: Central retinal artery occlusion (CRAO) is a neurological and ophthalmologic emergency that presents as sudden, painless, monocular vision loss. Central retinal artery occlusion can be classified as arteritic or non-arteritic. Most cases of non-arteritic CRAO are due to embolism, commonly from atherosclerosis of the ipsilateral carotid artery. More proximal sources of embolism are uncommon but can occur. Prompt recognition of CRAO is critical for vision preservation therapy and initiation of ischemic stroke diagnosis protocols. CASE REPORT: We present the case of a 66-year-old f

CRP-TyG index and risk of new‑onset central retinal artery occlusion and subsequent major adverse cardiovascular and cerebrovascular events: a propensity score‑matched cohort study.

BACKGROUND: The C‑reactive protein-triglyceride glucose index (CTI), a composite marker of systemic inflammation and metabolic status, has been associated with cardiovascular and cerebrovascular risk. Its role in central retinal artery occlusion (CRAO) and subsequent adverse outcomes is unclear. METHODS: In this cohort study, 122 CRAO patients and 488 matched controls who underwent coronary angiography without coronary artery disease were analyzed. CTI was calculated as 0.412 × ln [CRP (mg/L) + 0.5 × ln [TG × FPG(mg/dL)]. Logistic regression adjusted for demographic, clinical, laboratory, and

The Value of Point-of-care Ocular Ultrasound in Physician-in-triage Model: A Case Series.

INTRODUCTION: Physician-in-triage (PIT) models have become increasingly common in emergency medicine. The goal is to facilitate rapid patient evaluation and improve key operational emergency department (ED) metrics. However, there is limited time for the PIT encounter, which often involves an abbreviated patient evaluation. Point-of-care ultrasound (POCUS) has been shown to improve patient care and speed diagnosis in a variety of scenarios. Although physicians working within a PIT model must remain mindful of time constraints, POCUS can help identify time-sensitive diagnoses and guide appropri

Healing of ischemic injury in the retina.

Neuro- and retinal degenerative diseases, including Alzheimer's, stroke, age-related macular degeneration, and central retinal artery occlusion, rob millions of their independence. Studying these diseases in human retinas has been hindered by the rapid loss of neuronal activity after death. While some CNS activity has been restored postmortem, synchronized neuronal transmission beyond 30 min has remained elusive. We overcome this barrier by reviving and sustaining light signal transmission in human retinas recovered up to 4 hours after death and stored for up to 48 hours. We also introduce inf

Ocular blood flow and retinal oxygen saturation measurements in central retinal artery occlusion.

OBJECTIVES: To provide numerical quantitative measurements of blood flow velocity in the optic nerve head (ONH) and choroid and retinal vascular oxygen saturation in patients with central retinal artery occlusion (CRAO). METHODS: Twelve consecutive patients (12 eyes) underwent laser speckle flowgraphy (LSFG) and retinal oximetry measurements at presentation. The normal fellow eyes were evaluated and used as controls. RESULTS: In eyes with CRAO, the ONH mean blur rate (MBR)-vessel (25 ± 15 AU), representing blood flow velocity in the large vessels originating from the central retinal artery,

Exploring hyperbaric oxygen therapy for central retinal artery occlusion beyond 24 h: case report.

Central retinal artery occlusion (CRAO) is a rare emergency, often affecting individuals over 60, with risk factors such as hypertension, diabetes, and smoking. Hyperbaric oxygen therapy (HBOT), classified as level IIb by the American Heart Association for CRAO, helps maintain retinal oxygenation during ischemic events by diffusing oxygen through choroidal capillaries. While HBOT appears promising for addressing various vision-threatening conditions, including retinal occlusions and diabetic macular edema, it has not been officially approved for these indications. A 70-year-old male presented

Safe use of fluorinated gases in vitreoretinal surgery: learning from patient safety incidents with expert panel recommendations from the British and Eire Association of Vitreoretinal Surgeons (BEAVRS).

BACKGROUND: Errors in intravitreal fluorinated gas selection or dilution can result in unintended expansile concentrations, causing irreversible visual loss. We aimed to quantify the problem using national incident reporting, published literature and a UK surgeon survey, developing expert consensus recommendations on safe storage, handling and intraocular use. METHODS: NHS England patient safety incident repositories (National Reporting and Learning System and Learn from Patient Safety Events) were searched for events related to intravitreal gas use (2010-2025), followed by thematic analysis.

Reinforced double-layer vein patch angioplasty in carotid endarterectomy to mitigate patch aneurysm.

Carotid endarterectomy with patch angioplasty is standard for symptomatic carotid stenosis. However, prosthetic patches carry a risk of infection, particularly in patients with recent endocarditis. We report the case of a 67-year-old man who presented with central retinal artery occlusion and active mitral valve vegetation and subsequently underwent left carotid endarterectomy using a dual-layer facial vein patch. The harvested facial vein was everted to create a reinforced, double-layered conduit that was used for patch angioplasty, thereby avoiding prosthetic material. The patient awoke neur

Comparative effectiveness of varying hyperbaric oxygen protocols in the treatment of acute central retinal artery occlusion.

BACKGROUND: The comparative effectiveness of different hyperbaric oxygen (HBO) therapy doses in the acute treatment of central retinal artery occlusion (CRAO) has not been evaluated. METHODS: This retrospective cohort study aimed to compare the efficacy of an initial 2.8ATA HBO session (HBO18) to the more prevalent 2.0ATA protocol (HBO10). Excluded were patients with suspected inflammatory arteritis or branch retinal artery occlusion, and when HBO was medically contraindicated. Following the initial session, all patients completed two additional HBO10 sessions within 24 h. The primary outcome

Triptan Initiation and Cerebrovascular Events in Patients With Migraine: A Nationwide Cohort Study.

BACKGROUND: Triptan medications are proposed as an association of cerebral ischemic stroke in patients with migraine, but population-based data on this are lacking. METHODS: A retrospective, observational, cohort study was performed using computerized claims data from a proprietary, insurance-based registry-Marketscan (by Merative). Patients with at least 1 claim related to a migraine diagnosis were included. The primary exposure was initiation of any triptan medication defined by at least 1 prescription fill. The primary end point was time to cerebral ischemic stroke. Secondary end points in
