BACKGROUND: Amyloid-β-related angiitis (ABRA) is a rare steroid-responsive inflammatory vasculopathy that can mimic or complicate acute cerebrovascular disease. Its interaction with thrombolytic therapy is poorly understood. We report a biopsy-confirmed case of fulminant ABRA temporally associated with intravenous tenecteplase.
CASE DESCRIPTION: An 86-year-old man presented with acute painless monocular vision loss and received tenecteplase for presumed central retinal artery occlusion. Initial imaging showed no hemorrhage. Within 24 hours, he developed progressive encephalopathy with a large

Background/Objectives: Retinal artery occlusion (RAO) represents an ischemic event of the anterior circulation, driven by impaired blood flow through the ophthalmic branches of the internal carotid artery. Atrial fibrillation (AF) is an increasingly recognized cardioembolic source in patients with RAO, yet its prevalence remains poorly characterized. We conducted a systematic review and meta-analysis to estimate the pooled prevalence of AF in RAO patients and to summarize evidence on its incidence and association with this condition. Methods: Four databases were searched through April 2025, up

BACKGROUND: Retinal vascular occlusions (RVaO) are signs of systemic vascular disease and share risk factors with stroke and myocardial infarction. The cardiovascular risk of RVaO subtypes and their relationship to transient ischemic attack (TIA) remain unclear.
METHODS: Using the TriNetX database, we compared the 1-year incidence of major adverse cardiovascular events (MACE), including ischemic and hemorrhagic stroke, acute myocardial infarction, and death, between patients with RVaO and those with TIA. Cox regression was used to analyze the association between risk factors and MACE risk.

We report the case of a 77-year-old woman who presented with progressive, painless bilateral visual loss over three weeks. Ophthalmologic evaluation demonstrated bilateral anterior ischemic optic neuropathy and right central retinal artery occlusion. Notably, she lacked classical systemic features of giant cell arteritis (GCA), including headache, scalp tenderness, or jaw claudication. Laboratory evaluation revealed an elevated erythrocyte sedimentation rate (ESR 64 mm/h). Temporal artery ultrasound demonstrated bilateral halo signs, supporting the diagnosis of GCA. High-dose corticosteroid th

OBJECTIVES: To present three cases where the retrobulbar spot sign (RBSS) was observed, but central retinal artery occlusion (CRAO) was not confirmed.
METHODS: Transorbital sonography was employed to detect the presence of RBSS in all patients.
RESULTS: RBSS was detected in all three patients. However, only one patient had CRAO, with RBSS-positive findings in both eyes, although one eye showed normal function. The other two patients exhibited RBSS without evidence of CRAO. RBSS remained unchanged in all four eyes during the follow-up period.
CONCLUSION: RBSS may not be specific to CRAO, as

Central retinal artery occlusion (CRAO) is a vision-threatening ophthalmic emergency requiring rapid recognition and evaluation. Although embolic etiologies predominate, low-flow ischemia related to hemodynamic instability represents an underrecognized mechanism. We report the case of a 66-year-old woman with end-stage renal disease (ESRD) on hemodialysis who experienced recurrent transient right-eye visual symptoms near the end of dialysis sessions. She had chronic intradialytic hypotension, with systolic blood pressures ranging from 90 to 100 mmHg and post-dialysis pressures averaging 90/50

INTRODUCTION: The diagnosis of acute central retinal artery occlusion (CRAO) and branch retinal artery occlusion (BRAO) is often delayed or missed in emergency departments (EDs) because of limited ocular funduscopic skills and lack of immediate ophthalmology access. We evaluated the clinical impact of our Eye-Stroke protocol using non-mydriatic ocular imaging (non-mydriatic fundus photography and optical coherence tomography [NMFP-OCT]) in our general ED with remote interpretation by ophthalmology on the time to diagnosis and emergent management of acute CRAO/BRAO.
METHODS: Prospective consec

BACKGROUND: Central retinal artery occlusion (CRAO) is an ophthalmic emergency with poor visual prognosis. Although intravenous thrombolysis (IVT) within 4.5 h may be beneficial, the effectiveness and safety of low-dose alteplase remain uncertain.
METHODS: We conducted a retrospective study at three centres in Japan, enrolling patients who presented within 24 h of onset with CRAO or macula-involving branch retinal artery occlusion (BRAO) between June 2021 and September 2024. Patients were analysed if they had baseline best-corrected visual acuity (BCVA) < 20/400, clearly defined symptom onset

We report a fatal case of a 40-year-old man who developed rhino-orbito-cerebral mucormycosis complicated by central retinal artery occlusion, cavernous sinus thrombosis, and internal carotid artery occlusion. Mechanical thrombectomy was performed for the internal carotid artery occlusion, with subsequent Sanger sequencing of the fungal components within the retrieved thrombus to identify Rhizopus arrhizus. To the best of our knowledge, this is the first report establishing the species-level identification of R. arrhizus in mucormycosis-associated large-vessel occlusion. These findings provide

PURPOSE: To report the unusual case of a herpes zoster ophthalmicus (HZO) complicated by central retinal artery occlusion and viral encephalitis.
CASE REPORT: A 65-year-old male with HZO who underwent intravenous acyclovir treatment complained of sudden onset blurry vision of his left eye. Central retinal artery occlusion (CRAO) was revealed. We administered methylprednisolone pulse therapy, yet resulted in no visual acuity improvement.
CONCLUSION: There were only a few cases reported to have CRAO after diagnosed HZO. This cause of viral-related CRAO led to a complicated treatment decision.

Central retinal artery occlusion (CRAO) is a rare but vision-threatening complication following retinal detachment surgery. We report a man in his late 60s who underwent combined phacoemulsification with intraocular lens implantation and segmental scleral buckling for macula-sparing rhegmatogenous retinal detachment caused by inferotemporal ora dialysis. Shortly after surgery, he developed recurrent ocular pain with marked intraocular pressure (IOP) spikes, anterior chamber shallowing, iris bombe, and secondary angle closure. Approximately 3 h post-operatively, visual acuity deteriorated to no

Central retinal artery occlusion is rare in pediatrics and typically linked to embolic or hematologic disorders. The authors describe a 13-year-old boy with acute central retinal artery occlusion and elevated Bartonella henselae titers following kitten exposure. Extensive evaluation was unrevealing, suggesting a rare inflammatory association between cat scratch disease and pediatric retinal arterial ischemia. The main take-away from this case is to also look for infectious causes, which are rare and not the first to be investigated for during central retinal artery occlusion.

Polyarteritis nodosa (PAN) is a systemic necrotizing vasculitis of medium-sized arteries, and histologic confirmation is often challenging. Temporal artery biopsy (TAB) is established for suspected giant cell arteritis (GCA), but is rarely performed in PAN. We describe a 27-year-old Japanese man who presented with melena and acute visual loss in the right eye. Contrast-enhanced computed tomography revealed multiple visceral aneurysms involving the hepatic, renal, and mesenteric arteries, with active extravasation from a jejunal artery aneurysm. Ophthalmologic evaluation confirmed central retin

BACKGROUND: Idiopathic pulmonary fibrosis (IPF) is a rare interstitial lung disease with a poor prognosis that currently lacks effective treatment methods. Preventing the acute exacerbation of IPF, identifying the molecular subtypes of patients, providing personalized treatment, and developing individualized drugs are guidelines for predictive, preventive, and personalized medicine (PPPM / 3PM) to promote the development of IPF. Oxidative stress (OS) is an important pathological process of IPF. However, the relationship between the expression levels of oxidative stress-related genes (OSRGs) an

PURPOSE: To evaluate retinal circulation in eyes with central retinal artery occlusion.
METHODS: The best-corrected visual acuity (BCVA), fluorescein angiography, and laser speckle flowgraphy values were measured at the first visit in 42 patients with central retinal artery occlusion (42 eyes; mean age, 66.0 ± 14.0 years). Laser speckle flowgraphy was performed at baseline, 1 week, and 1, 3, and 6 months; the difference between the mean blur rate vessel area and mean blur rate tissue area (MV-MT), reflecting the retinal vessel blood flow, was measured. Fluorescein angiography measured the art

UNLABELLED: Neuro- and retinal degenerative diseases, including Alzheimer's, age-related macular degeneration, stroke, and central retinal artery occlusion, rob millions of their independence. Studying these diseases in human retinas has been hindered by the immediate loss of neuronal activity postmortem. While recent studies restored limited activity in postmortem CNS tissues, synchronized neuronal transmission >30 minutes postmortem remained elusive. Our study overcomes this barrier by reviving and sustaining light signal transmission in human retinas recovered up to four hours and stored 48
