BACKGROUND: Prompt diagnosis of acute central retinal artery occlusion (CRAO) is crucial for therapeutic management and stroke prevention. However, most stroke centers lack onsite ophthalmic expertise before considering fibrinolytic treatment. This study aimed to develop, train, and test a deep learning system to detect hyperacute CRAO on retinal fundus photographs within the critical 4.5-hour treatment window and up to 24 hours after visual loss to aid in secondary stroke prevention.
METHODS: Our retrospective, cross-sectional study included 1322 color fundus photographs from 771 patients wi

BACKGROUND: Chronic internal carotid artery occlusion (ICAO) is an uncommon etiology of central retinal artery occlusion (CRAO). The role of intra-arterial thrombolysis (IAT) remains contentious in such cases. This report details the technical feasibility of IAT administered through angiographically identified maxillary-ophthalmic collaterals for acute CRAO occurring in a patient with chronic ICAO.
CASE PRESENTATION: A 52-year-old male presented with sudden, painless vision loss (finger counting) in the left eye due to CRAO secondary to chronic ICAO. After failed conservative treatment, IAT w

Microscopic polyangiitis (MPA) is a rare autoimmune vasculitis of small vessels. It is often linked with ANCA. Central retinal artery occlusion (CRAO) is characterized by sudden, painless vision loss and is typically connected with embolic or inflammatory causes. However, limited literature exists on its association with MPA. This case report uniquely highlights a 50 year old patient diagnosed with MPA exhibiting CRAO and brainstem infarct. The report stresses the importance of early recognition of uncommon vascular manifestations, multidisciplinary diagnostic approaches, and the integration o

BACKGROUND: Visual field defect and visual impairment are important but poorly characterized complications after flow-diverter (FD) stent placement for internal carotid artery (ICA) aneurysms.
METHODS: We retrospectively analyzed 31 consecutive patients with 32 ICA aneurysms treated by FD stent implantation between October 2015 and April 2021. Aneurysms were classified into two groups those with visual symptoms before and after FD stenting (symptomatic group) and those without (asymptomatic group). We analyzed patients' background characteristics, treatment details, and imaging findings.

Central retinal artery occlusion is typically a thromboembolic phenomenon associated with atherosclerotic disease. This case presentation describes a rare case of CRAO in a young male secondary to hyperviscosity syndrome as a first symptom ofMixed Phenotype Acute Leukaemia. It is imperative for emergency physicians to recognise an atypical presentation of painless vision loss and evaluate such patients thoroughly, ensuring timely and vision sparing interventions.

BACKGROUND: Cardiac calcifications are a rare consequence of end-stage renal failure and hyperparathyroidism.
CASE SUMMARY: A 35-year-old man with no prior medical history was admitted to our institution with sudden blindness in his right eye and severe renal failure. Ophthalmologic evaluation revealed central retinal artery occlusion with arterial calcifications. End-stage chronic renal failure was diagnosed. A positron emission tomography-computed tomography (CT) and a Tc-99m multiplex ion beam imaging scintigraphy confirmed hyperparathyroidism. Transthoracic echocardiography, magnetic reso

This case involves a patient with coronary artery disease and oropharyngeal cancer, previously treated with surgery, chemotherapy, and radiation therapy, who underwent a cervical lymph node excision. Postoperatively, the patient developed acute, painless, and monocular vision loss. Postoperative visual loss following nonocular surgery is most commonly caused by ischemic optic neuropathy or central retinal artery occlusion. In this case, visual loss resulted from emboli originating in the common carotid artery leading to retinal artery occlusion. The underlying cause was radiation arteritis, a

BACKGROUND: Retinal artery occlusion (RAO) is a vision-threatening condition with limited therapeutic options. Hyperbaric oxygen therapy (HBOT) has emerged as a potential treatment to enhance retinal oxygenation and salvage ischemic tissue, though its efficacy and safety remain debated.
METHODS: We followed the Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines and the Cochrane Handbook for Systematic Reviews of Interventions. Databases were searched through November 2024 for studies comparing HBOT with control in RAO patients. Risk of bias was assessed usi

Retinal artery occlusions, encompassing central retinal artery occlusion and branch retinal artery occlusion, are acute vascular events that can result in significant and often irreversible vision loss. These ocular emergencies are closely associated with systemic vascular risk factors, including hypertension, diabetes mellitus, advanced age, and cardiovascular comorbidities. Atrial fibrillation (AFib), the most common sustained cardiac arrhythmia, has emerged as a notable contributor to the risk of retinal artery occlusions. The prevalence of AFib is rising, particularly among older adults, p

BACKGROUND: Vision loss following cosmetic filler injections is a rare but devastating complication resulting from inadvertent intravascular embolization, most often affecting the ophthalmic artery. Retrobulbar hyaluronidase injection has been proposed as an emergency intervention, yet anatomical guidelines for its administration remain poorly defined.
OBJECTIVE: To propose an anatomically informed protocol for safe and effective retrobulbar hyaluronidase injection in cases of filler-induced central retinal artery occlusion.
METHODS: Cadaveric dissections and micro-CT imaging were used to ma

BACKGROUND AND PURPOSE: Central retinal artery occlusion (CRAO) is a rare form of acute ischemic stroke that causes severe visual loss, which is a relatively rare emergency but devastating eye condition. There is currently no guideline-endorsed treatment for CRAO. Data on hyperbaric oxygen therapy (HBOT) for CRAO is minimal. We aimed to investigate the benefit of HBOT in patients with non-arteritic CRAO.
METHODS: We conducted a retrospective study from two medical centers that recruited patients with diagnosed non-arteritic CRAO from January 2019 to December 2024. HBOT was offered to CRAO pat

Central retinal artery occlusion (CRAO) is a relatively rare emergent condition of the eye resulting in sudden painless vision loss. This vision loss is usually dramatic and permanent, and the prognosis for visual recovery is poor. Patients particularly at risk include those with giant cell arteritis, atherosclerosis, and thromboembolic disease. A wide variety of treatment modalities have been tried over the last one hundred years with little to no success, with the exception of hyperbaric oxygen therapy (HBO₂).

BACKGROUND: Central retinal vein occlusion (CRVO) and central retinal artery occlusion (CRAO) are serious eye blood vessel problems usually linked to heart health issues. In this case study, a patient with a diagnosis of obstructive sleep apnea syndrome (OSAS) but no traditional thrombotic or atherosclerotic risk factors experienced a rare co-occurrence of CRVO and CRAO.
CASE PRESENTATION: A formerly healthy 42-year-old man came with dark shadows in his right eye and acute-onset blurred vision. A thorough ophthalmic exam verified simultaneous CRVO and CRAO. Later, polysomnography showed moder
