Clinical Features, Multimodal Imaging, and Neurological Differential Diagnoses in Recurrent Susac Syndrome.

BACKGROUND: Susac's syndrome (SuS) is a rare microangiopathy, often consisting of a classic, pathognomonic, clinical triad of sensorineural hearing impairment, encephalopathy, and retinal arterial occlusions. The variable presentation of SuS leads to underdiagnosis, despite the syndrome standing as an important differential in a variety of ophthalmic, neurological, and psychiatric presentations. Low case numbers and a fast progressive clinical course add to the difficulty of early recognition and treatment. METHODS: We report on a case of Susac's syndrome in a 40-year-old woman with the class

Regulation of YAP1 during oocyte maturation.

CONTEXT: Yes-associated protein 1 (YAP1) plays essential roles in somatic cell proliferation and differentiation and is regulated by cell cycle proteins and mechanical cues transduced by the actin cytoskeleton. YAP1 has been localized to the mitotic spindle. Whether YAP1 may participate in oocyte maturation is not known. AIMS: Determine effect of YAP inhibitor on oocyte maturation and the potential involvement of cell cycle regulators and actin polymerization modulators on YAP localization and phosphorylation status during oocyte maturation. METHODS: Mouse oocytes were cultured in vitro alon

Secondary stroke evaluation in patients admitted for CRAO in a large national inpatient sample.

BACKGROUND: Central retinal artery occlusion (CRAO) is an ophthalmic condition associated with cerebrovascular ischaemia. In patients with acute CRAO, the 2021 American Heart Association (AHA) scientific statement recommends a diagnostic evaluation for secondary stroke prevention, including cerebrovascular imaging, cardiac rhythm monitoring, echocardiogram and labs. This study aims to understand the national stroke evaluation rates for patients with CRAO before 2021. METHODS: Retrospective, cross-sectional analysis of inpatient admissions from the National Inpatient Sample between 2016 and 20

Correlation between quantitative OCT metrics and contrast sensitivity function in retinal artery occlusion.

AIMS: To investigate the association between optical coherence tomography (OCT) biomarkers and contrast sensitivity (CS) measured by the quantitative contrast sensitivity function (qCSF) in retinal artery occlusion (RAO). METHODS: This cross-sectional study included RAO patients and age-matched controls undergoing visual acuity (VA), qCSF testing and spectral-domain OCT. Only eyes with VA≥20/200 were included for reliable CS evaluation. OCT quantified retinal layer thickness, ischaemic lesion size and distance from ischaemia to the fovea. Linear regression analyses were performed. RESULTS: 2

BACKGROUND AND OBJECTIVES: Calcitonin gene-related peptide (CGRP) inhibitors have entered widespread use in the United States for treatment of migraine, but data on their cardiovascular risk profile are lacking. The objective of this study was to determine whether exposure to a CGRP inhibitor is associated with cardiovascular events in patients with migraine. METHODS: A retrospective, observational, cohort study was conducted using computerized claims data from a proprietary, insurance-based registry, MarketScan (by Merative). Beneficiaries with at least one claim related to a migraine diagno

CRP-TyG index and risk of new‑onset central retinal artery occlusion and subsequent major adverse cardiovascular and cerebrovascular events: a propensity score‑matched cohort study.

BACKGROUND: The C‑reactive protein-triglyceride glucose index (CTI), a composite marker of systemic inflammation and metabolic status, has been associated with cardiovascular and cerebrovascular risk. Its role in central retinal artery occlusion (CRAO) and subsequent adverse outcomes is unclear. METHODS: In this cohort study, 122 CRAO patients and 488 matched controls who underwent coronary angiography without coronary artery disease were analyzed. CTI was calculated as 0.412 × ln [CRP (mg/L) + 0.5 × ln [TG × FPG(mg/dL)]. Logistic regression adjusted for demographic, clinical, laboratory, and

Healing of ischemic injury in the retina.

Neuro- and retinal degenerative diseases, including Alzheimer's, stroke, age-related macular degeneration, and central retinal artery occlusion, rob millions of their independence. Studying these diseases in human retinas has been hindered by the rapid loss of neuronal activity after death. While some CNS activity has been restored postmortem, synchronized neuronal transmission beyond 30 min has remained elusive. We overcome this barrier by reviving and sustaining light signal transmission in human retinas recovered up to 4 hours after death and stored for up to 48 hours. We also introduce inf

Exploring hyperbaric oxygen therapy for central retinal artery occlusion beyond 24 h: case report.

Central retinal artery occlusion (CRAO) is a rare emergency, often affecting individuals over 60, with risk factors such as hypertension, diabetes, and smoking. Hyperbaric oxygen therapy (HBOT), classified as level IIb by the American Heart Association for CRAO, helps maintain retinal oxygenation during ischemic events by diffusing oxygen through choroidal capillaries. While HBOT appears promising for addressing various vision-threatening conditions, including retinal occlusions and diabetic macular edema, it has not been officially approved for these indications. A 70-year-old male presented

Monocyte-derived macrophages promote retinal damage after ischemic stroke via IL-1β-dependent mechanism.

Vision impairment following ischemic stroke is a prevalent complication that significantly compromises patients' quality of life. Inflammatory responses critically contribute to retinal dysfunction in this condition. Retinal myeloid cells contributed to the retinal inflammatory response, which presented heterogeneity after retinal injury. In this study, we employed the classical middle cerebral artery occlusion (MCAO) mouse model to simulate ischemic stroke. We demonstrated that stroke-induced retinal damage manifests as diminished photoreceptor responses and increased retinal cell apoptosis b

Agarose Gel as an Injectable Filler Significantly Reduced the Risk of Rabbit Retinal Artery Embolism Compared to Hyaluronic Acid.

BACKGROUND: Vascular embolism is a common complication associated with the use of injectable fillers. Agarose gel, a novel filler, may have a lower risk of embolism. However, there are few studies investigated the embolism rate of agarose gel and other fillers. This study aims to compare the risk of rabbit retinal artery embolism caused by hyaluronic acid or agarose gel in vivo, as well as their dissolution characteristics in vitro. METHODS: The study used a rabbit model of retinal artery embolism. Agarose gel or hyaluronic acid was injected into the carotid artery at doses of 0.1, 0.2, and 0

Comparative effectiveness of varying hyperbaric oxygen protocols in the treatment of acute central retinal artery occlusion.

BACKGROUND: The comparative effectiveness of different hyperbaric oxygen (HBO) therapy doses in the acute treatment of central retinal artery occlusion (CRAO) has not been evaluated. METHODS: This retrospective cohort study aimed to compare the efficacy of an initial 2.8ATA HBO session (HBO18) to the more prevalent 2.0ATA protocol (HBO10). Excluded were patients with suspected inflammatory arteritis or branch retinal artery occlusion, and when HBO was medically contraindicated. Following the initial session, all patients completed two additional HBO10 sessions within 24 h. The primary outcome

Branch retinal artery occlusion time to presentation and diagnosis: a retrospective review.

BACKGROUND: Branch retinal artery occlusion (BRAO) is an acute, vision-threatening condition that often signals underlying systemic vascular disease and the need for urgent vascular risk assessment and mitigation. Although prompt evaluation is critical for accurate diagnosis and early identification of modifiable vascular risk factors, patterns of clinical presentation remain poorly characterized. METHODS: This retrospective study included BRAO patients who presented to Kaiser Permanente Northern California (KPNC) within 30 days of symptom onset from 2014 to 2023. Targeting CDH13 as a therapeutic strategy to mitigate pathological ocular angiogenesis.

BACKGROUND: Anti-angiogenic therapies, which suppress pathological vessel growth in neovascular diseases such as diabetic retinopathy and neovascular age-related macular degeneration (nAMD), have transformed the clinical landscape. However, a critical unmet challenge is to shift the therapeutic paradigm from broad inhibition towards achieving vascular normalization, as non-selective angiogenic blockade can result in detrimental off-target effects. Consequently, the identification of novel molecular targets is imperative for developing more precise and effective treatment strategies. METHODS: W

Clinical Guidelines for the Diagnosis and Management of Retinal Artery Occlusions by the Spanish Society of Retina and Vitreous.

Retinal artery occlusions (RAOs) constitute a rare but serious ophthalmological emergency with a poor prognosis for visual recovery, for which there is currently no adequate therapeutic strategy. However, there is substantial evidence highlighting the importance of systemic (non-ophthalmological) management of patients with RAO. Since RAOs are the ocular analogous of a cerebral stroke, they require a similar urgent, multidisciplinary approach involving laboratory tests and imaging studies that go beyond the typical scope of ophthalmologists. This calls for cooperation and understanding from al
