Collaborative care for retinal artery occlusion by ophthalmologists and vascular neurologists: management of systemic complications.

PURPOSE: Retinal artery occlusion (RAO) is frequently associated with cerebrovascular and cardiovascular events, and guidelines highlight the need for secondary prevention. Since 2019, our hospital has implemented collaborative care between ophthalmologists and vascular neurologists for acute RAO, and here, we report the clinical outcomes before and after this approach. STUDY DESIGN: Retrospective observational study. METHODS: Eighty-seven patients (mean age: 72.2 ± 13.4 years; 40 male) diagnosed with central retinal artery occlusion (CRAO), branch retinal artery occlusion (BRAO), or ciliore

Management of Visual Loss Following Hyaluronic Acid Filler Injections: An Algorithm-based Approach.

BACKGROUND: Hyaluronic acid (HA) filler injections have been associated with the rare but vision-threatening complication of retinal artery embolism. Clinical management of this condition remains challenging due to the lack of high-level evidence. Randomized controlled trials are neither ethically justifiable nor practically feasible, and current treatment recommendations are derived almost exclusively from individual case reports and small case series. METHODS: The aim of this article is to offer evidence-informed recommendations for the management of central retinal artery occlusion followi

Retrospective treatment comparisons in patients with Susac syndrome.

INTRODUCTION: Susac syndrome is a rare disease affecting arterioles in the brain, retina and inner ear. There is uncertainty about a clinical benefit of adding intravenous immunoglobulins or immunosuppressants to corticosteroid maintenance treatment. METHODS: This restrospective study identified adult patients from case records who met crite-ria for probable or definite Susac syndrome. Patients with a sole manifestation of arterial wall hyperintensity (AWH) on retinal fluorescein angiography (RFA) were also included. All clinically symptomatic disease activity and clinically asymptomatic AWH

Simultaneous Bilateral Central Retinal Artery Occlusions: Partial Recovery With Multimodal Therapy.

Central retinal artery occlusion (CRAO) is a rare cause of sudden, profound vision loss, and bilateral involvement is exceptionally uncommon, with no clearly established optimal management. We describe a patient who developed near-simultaneous, painless vision loss in both eyes and underwent urgent multidisciplinary assessment followed by systemic and ocular therapies intended to restore perfusion and limit ischemic injury. Although partial visual recovery was achieved, the course was complicated by retinal neovascularization requiring treatment and resulting in severe long-term visual impairm

Systemic Inflammation and Structural Retinal Damage on OCT in Acute Non-Arteritic Central Retinal Artery Occlusion-A Retrospective Study.

Central retinal artery occlusion (CRAO) is an acute ophthalmic emergency causing irreversible vision loss due to retinal ischemia. Inflammation is increasingly recognized as a contributor to vascular occlusions, but the relationship between systemic inflammatory biomarkers and structural retinal damage in CRAO remains unclear. This retrospective study included 37 patients with non-arteritic CRAO presenting within 168 h of symptom onset. Optical coherence tomography (OCT) classified ischemic severity as mild, moderate, or severe based on inner retinal morphology. Systemic inflammatory indices,

Incidence or Coincidence: Subaortic Membrane, Bicuspid Aortic Valve, and Aortic Root Dilatation in a Patient Presenting With Stroke.

Bicuspid aortic valve (BAV) is the most common congenital cardiac malformation and is frequently associated with progressive aortopathy. Subaortic membrane, a discrete form of left ventricular outflow tract (LVOT) obstruction, is uncommon in adults and rarely coexists with BAV outside of complex congenital syndromes. The simultaneous presence of BAV, aortic root dilation, and a subaortic membrane represents an unusual anatomical constellation with important diagnostic and clinical implications. We present the case of a 49-year-old man with no known medical history who developed sudden, painles

CD5L promotes efferocytosis and resolution of retinal ischemic injury.

Ischemia-induced retinopathy is a defining feature of prevalent ocular conditions, including diabetic retinopathy and central retinal artery or vein occlusion. Therapeutic interventions for ischemic retinopathies show limited efficacy and adverse effects, highlighting the need to thoroughly investigate the underlying mechanisms. Histone deacetylase 3 (HDAC3), a member of the histone deacetylase family, plays a central role in regulating gene expression in myeloid cells (microglia and macrophages). We recently showed that myeloid HDAC3 deletion promotes tissue repair and functional recovery aft

An Unusual Case of Giant Cell Arteritis (GCA) Presenting With Bilateral Visual Loss.

We report the case of a 77-year-old woman who presented with progressive, painless bilateral visual loss over three weeks. Ophthalmologic evaluation demonstrated bilateral anterior ischemic optic neuropathy and right central retinal artery occlusion. Notably, she lacked classical systemic features of giant cell arteritis (GCA), including headache, scalp tenderness, or jaw claudication. Laboratory evaluation revealed an elevated erythrocyte sedimentation rate (ESR 64 mm/h). Temporal artery ultrasound demonstrated bilateral halo signs, supporting the diagnosis of GCA. High-dose corticosteroid th

Anesthetic Management of Patients With Advanced Carotid Artery Disease Undergoing Ophthalmic Surgery: Implications for Cerebral Perfusion and Visual Outcomes.

Carotid artery disease represents a significant clinical challenge in patients undergoing ophthalmic surgery due to its direct impact on cerebral and ocular perfusion. Its prevalence increases with age and is strongly associated with cardiovascular risk factors such as hypertension, diabetes mellitus, dyslipidemia, and smoking. These patients often present with impaired cerebral autoregulation, making cerebral blood flow highly dependent on systemic blood pressure. Consequently, even minor hemodynamic fluctuations during the perioperative period may precipitate ischemic complications. The path

Retrobulbar spot sign in non-ischemic conditions: a case series challenging its diagnostic role.

OBJECTIVES: To present three cases where the retrobulbar spot sign (RBSS) was observed, but central retinal artery occlusion (CRAO) was not confirmed. METHODS: Transorbital sonography was employed to detect the presence of RBSS in all patients. RESULTS: RBSS was detected in all three patients. However, only one patient had CRAO, with RBSS-positive findings in both eyes, although one eye showed normal function. The other two patients exhibited RBSS without evidence of CRAO. RBSS remained unchanged in all four eyes during the follow-up period. CONCLUSION: RBSS may not be specific to CRAO, as

Managing Nasal Ischemia and Necrosis Post-Triamcinolone Injection: A New Case Series.

Nasal ischemia following cosmetic injections has been reported in several cases. Postsurgery triamcinolone administration in the nasal region is a common practice to reduce inflammation and address pollybeak deformity. While cases of retinal artery occlusion following triamcinolone injection have been documented, this case series presents the first reported instance of nasal necrosis resulting from triamcinolone injection. This article explores the management strategies and therapeutic approaches for ischemia following triamcinolone administration. Four cases of nasal ischemia following triamc
