Post-Marketing Ocular Adverse Events Associated with Tamoxifen and Raloxifene: A Population-Based Pharmacovigilance Study.

PURPOSE: Tamoxifen and raloxifene are selective estrogen receptor modulators used for breast cancer treatment, and osteoporosis prevention. While systemic adverse events (AEs) are well recognized, ocular AEs have been less comprehensively characterized. The purpose of this study was to evaluate postmarketing ocular AEs associated with tamoxifen and raloxifene using a pharmacovigilance database. METHODS: The U.S. Food and Drug Administration Adverse Event Reporting System (FAERS) data from Q4 2003 to Q2 2025 were analyzed using OpenVigil 2.1. Disproportionality analyses were conducted to calcu

Lupus Retinopathy Mimicking Ocular Tuberculosis: A Diagnostic Dilemma.

Systemic lupus erythematosus (SLE) or Lupus is often called the "great mimicker" due to its ability to mimic other systemic disorders. SLE is a multisystem autoimmune disorder with varied manifestations, including ocular involvement. Retinal vascular occlusions are an uncommon but potentially vision-threatening presentation of lupus retinopathy. When coexisting systemic features mimic infectious etiologies such as tuberculosis (TB), diagnosis becomes challenging, delaying appropriate immunosuppressive therapy. We report a diagnostically complex case of a 22-year-old female who presented with s

Central Retinal Artery Occlusion Following Intradialytic Hypotension in End-Stage Renal Disease.

Central retinal artery occlusion (CRAO) is a vision-threatening ophthalmic emergency requiring rapid recognition and evaluation. Although embolic etiologies predominate, low-flow ischemia related to hemodynamic instability represents an underrecognized mechanism. We report the case of a 66-year-old woman with end-stage renal disease (ESRD) on hemodialysis who experienced recurrent transient right-eye visual symptoms near the end of dialysis sessions. She had chronic intradialytic hypotension, with systolic blood pressures ranging from 90 to 100 mmHg and post-dialysis pressures averaging 90/50

Impact of an eye-stroke protocol with non-mydriatic ocular imaging in an emergency department.

INTRODUCTION: The diagnosis of acute central retinal artery occlusion (CRAO) and branch retinal artery occlusion (BRAO) is often delayed or missed in emergency departments (EDs) because of limited ocular funduscopic skills and lack of immediate ophthalmology access. We evaluated the clinical impact of our Eye-Stroke protocol using non-mydriatic ocular imaging (non-mydriatic fundus photography and optical coherence tomography [NMFP-OCT]) in our general ED with remote interpretation by ophthalmology on the time to diagnosis and emergent management of acute CRAO/BRAO. METHODS: Prospective consec

Low-dose intravenous alteplase for acute retinal artery occlusion: A multicentre retrospective study.

BACKGROUND: Central retinal artery occlusion (CRAO) is an ophthalmic emergency with poor visual prognosis. Although intravenous thrombolysis (IVT) within 4.5 h may be beneficial, the effectiveness and safety of low-dose alteplase remain uncertain. METHODS: We conducted a retrospective study at three centres in Japan, enrolling patients who presented within 24 h of onset with CRAO or macula-involving branch retinal artery occlusion (BRAO) between June 2021 and September 2024. Patients were analysed if they had baseline best-corrected visual acuity (BCVA) < 20/400, clearly defined symptom onset

Systemic Sclerosis Presenting With Chronic Atrophic Gastritis and Retinal Artery Occlusion: A Case Report.

Systemic sclerosis (SSc), also known as scleroderma, is a rare autoimmune disease characterized by fibroproliferative changes, vascular dysfunction, and small-vessel proliferation. It can affect multiple organ systems, including the gastrointestinal tract, skin, heart, eyes, kidneys, and lungs. We report a case of a 67-year-old woman with systemic sclerosis who tested negative for specific antibodies and simultaneously developed two rare complications: autoimmune atrophic gastritis resulting in megaloblastic anemia and acute retinal artery occlusion. Notably, she presented with megaloblastic a

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Optic nerve sheath enhancement on orbital MRI in giant cell arteritis.

BACKGROUND: Differentiating arteritic anterior ischaemic optic neuropathy (A-AION) due to giant cell arteritis (GCA) from non-arteritic anterior ischaemic optic neuropathy (NA-AION) may pose a diagnostic challenge. Our study aimed to assess the use of standard orbital MRI in distinguishing ocular manifestations of GCA from NA-AION. METHODS: This study included 25 consecutive patients (11 GCA, 14 NA-AION) who underwent contrast-enhanced orbital MRIs within 3 months of symptom onset. Two radiologists blinded to clinical data independently evaluated MRIs for the enhancement of the optic nerve sh

Atrial Fibrillation and Retinal Stroke.

IMPORTANCE: Atrial fibrillation (AF) is the most common, chronic, cardiac arrythmia in older US adults. It is not known whether AF is independently associated with increased risk of retinal stroke (central retinal artery occlusion), a subtype of ischemic stroke that causes severely disabling visual loss in most cases and is a harbinger of further vascular events. OBJECTIVE: To determine whether there is an association between AF and retinal stroke. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study was performed between July 2023 and May 2024 using computerized inpatient, out

Safety of intravenous thrombolysis in central retinal artery occlusion patients with concomitant acute cerebral infarctions.

BACKGROUND AND OBJECTIVES: Central retinal artery occlusion (CRAO) results in severe visual impairment. Intravenous thrombolysis (IVT) is a potential treatment for CRAO, but ambiguous data regarding efficacy and the possibility that the presence of cerebral infarctions may increase the risk of symptomatic intracranial hemorrhage (sICH) raise concerns. The aim of the current study was to evaluate the safety of IVT in patients with CRAO. METHODS: This multi-center, international registry study included 66 patients with CRAO that presented within 4.5 h from symptom onset. Patients that received

Simultaneous central retinal vein and cilioretinal artery occlusion in a young female patient with progestin-only oral contraceptive use: a case report.

BACKGROUND: Central retinal vein occlusion and cilioretinal artery occlusion are rare but serious ocular conditions that can lead to significant visual impairment. While few cases of central retinal vein occlusion and cilioretinal artery occlusion have been individually reported, concurrent occlusion of both vessels is extremely rare, particularly in younger patients without traditional vascular risk factors. We present the first reported case of simultaneous central retinal vein occlusion and cilioretinal artery occlusion in a young female patient associated with short-term use of progestin-o

From Heart to Eye: Central Retinal Artery Occlusion Secondary to Endocarditis: Septic Embolism.

Infective endocarditis (IE) remains a medical challenge. Potential complications such as heart failure and septic embolization may arise, prompting early recognition and treatment. We present the case of a 78-year-old woman with sudden, painless monocular vision loss in the left eye, without other accompanying symptoms. Ophthalmologic evaluation revealed central retinal artery occlusion. A holosystolic murmur was heard at the apex. Further echocardiography confirmed IE. Despite efforts, prognosis of IE is poor, with an in-hospital mortality of 25% in our country, with high risk of embolism (20

Unilateral Central Retinal Artery Occlusion Revealing Giant Cell Arteritis: A Case Report.

Giant cell arteritis (GCA), also known as temporal arteritis, is a granulomatous vasculitis that affects large and medium-sized blood vessels. While the exact etiology remains unclear, the inflammation predominantly involves the aorta and its branches, particularly the orbital arteries. Among these, the posterior ciliary arteries (PCAs), which supply the choroid, optic nerve head, and cilioretinal artery, are especially prone to involvement. Ocular manifestations of GCA occur frequently and may result in significant visual loss. Ophthalmic conditions associated with GCA include arteritic anter
