Management of Retinopathy of Prematurity in the United Arab Emirates: A Consensus Statement From the Emirates Society of Ophthalmology.

Retinopathy of prematurity (ROP) is a leading cause of preventable childhood blindness worldwide and affects a substantial proportion of very preterm or very-low-birth-weight infants. In the United Arab Emirates (UAE), hospital-based cohorts report a notable incidence of ROP among preterm infants, underscoring the need for standardized protocols. This Nominal Group Technique (NGT)-based national expert consensus aims to provide unified guidance for early detection, effective management, and improved outcomes for infants at risk of ROP in the UAE. A nationally representative panel of UAE-based

A Multidisciplinary Approach in the Management of Aggressive Retinopathy of Prematurity With Bleb-Like Posterior Combined Retinal Detachment.

We report two cases of bleb-like combined retinal detachment (BLRD) in infants with aggressive retinopathy of prematurity (A-ROP). Both cases involved premature infants born at 28 weeks of gestation with complex postnatal courses, who were referred to our clinic for ROP management. On presentation, both babies were diagnosed with stage 4B ROP in zone 1 with bilateral BLRD. Indirect ophthalmoscope-guided laser photocoagulation under topical anesthesia was performed on the same day. Pediatric clearance for surgery was obtained. Both infants underwent blood transfusions for anemia. Subsequently,

Risk factors and prevalence of retinopathy of prematurity in very low birth weight preterm infants: a systematic review and meta-analysis.

BACKGROUND: Retinopathy of prematurity (ROP) is a potentially blinding disorder affecting preterm infants, especially those with very low birth weight (<1,500 g). While advances in neonatal care have improved survival, the incidence of ROP remains high, and risk factors vary across studies. This meta-analysis aims to determine the prevalence of ROP in very low birth weight preterm infants and identify key risk factors to inform clinical screening and preventive strategies. METHODS: PubMed, Embase, Cochrane Library, and Web of Science databases were searched from database creation to January 7

Efavirenz treatment improves retinal vaso-obliteration and pathological neovascularization in a mouse model of retinopathy of prematurity.

OBJECTIVES: Previous studies have shown the metabolic and regulatory significance of CYP46A1 in the adult retina; however, its role in the developing retina is unknown. Here, we evaluate CYP46A1 expression and the impact of its activation in the developing mouse retina under normal and pathological conditions. METHODS: Seven-day-old (P7) C57BL/6 J mice maintained in room air (controls) or subjected to oxygen-induced retinopathy (OIR) were treated with/without 20 mg/kg efavirenz (EFV), a CYP46A1 activator administered intraperitoneally from P7 to P17. RESULTS: Retinal cross sections and flat

Association of red blood cell transfusions with periventricular leukomalacia in very preterm infants.

BACKGROUND AND OBJECTIVES: Red blood cell (RBC) transfusions have been associated with retinopathy of prematurity (ROP) and adverse long-term neurodevelopmental outcomes in very preterm infants. Although lower transfusion thresholds reduce the incidence of ROP, they have not improved neurological outcome. Periventricular leukomalacia (PVL) is a major cause of long-term neurodevelopmental impairment in very preterm infants. The objective of this study was to determine whether exposure to RBC transfusions is an independent, dose-dependent risk factor for PVL. MATERIALS AND METHODS: A retrospect

Association between risk-adjusted mortality and severe morbidity in very low birth weight infants: a multicenter study from the NEOCOSUR network.

BACKGROUND: Very low birth weight infants (VLBWI) are at high risk of mortality and severe morbidities. While many neonatal networks monitor risk-adjusted outcomes separately, the relationship between mortality and major morbidities across centers remains unclear. OBJECTIVE: To evaluate the association between risk-adjusted in-hospital mortality and six major morbidities in VLBWI across 20 South American NICUs participating in the NEOCOSUR Network between 2011 and 2023. METHODS: Retrospective cohort study including 14,100 VLBWI (500-1500 g) from 20 NICUs in Argentina, Chile, Paraguay, Peru,

Polyunsaturated Fatty Acid Balance Modulates Microglial State in a Murine Model of Oxygen-Induced Neovascularization.

Background/Objectives: The retina is enriched in polyunsaturated fatty acids (PUFAs) which are indispensable for normal vision, and recent clinical studies have shown that dietary supplementation of ω-6-and ω-3-polyunsaturated fatty acids (PUFAs) can provide a protective role against retinopathy of prematurity (ROP). Our study aims to understand the mechanisms by which altering ω-6-and ω-3-polyunsaturated fatty acids (PUFAs) in the eye can protect against pathologic retinal neovascularization (NV). Methods: We interrogated the effects of endogenous ω-3-PUFA enrichment using transgenic fat-1 mi

Understanding the Molecular Basis of Pathological Retinal Angiogenesis: Roles of Lipids, Non-Coding RNAs, and Cellular Crosstalk.

Pathological retinal neovascularization is a prevalent cause of blindness, impacting millions of individuals across all age groups, from infants to older adults. It is a key clinical feature of several retinal diseases, such as diabetic retinopathy (DR), retinopathy of prematurity (ROP), age-related macular degeneration (AMD), and Coats disease. Researchers have extensively investigated the involvement of vascular endothelial growth factor (VEGF) signaling, hypoxia inducible factor (HIF) signaling, and inflammation in the regulation of pathological retinal neovascularization. Recent breakthrou

From Science and Technology to Eye-Health Policy: A Review on Global and Asia-Pacific Strategies on the Prevention of Blindness from Retinal Diseases.

The World Health Organization (WHO) is making a concerted effort to enhance health, including eye health and overall well-being. This global effort has resulted in a reduced age-standardized prevalence of avoidable blindness and no increase in avoidable moderate and severe vision impairment. However, the absolute number of cases has increased for both avoidable blindness and moderate-to-severe vision impairment. Globally, the leading causes of visual impairment include uncorrected refractive errors and cataract, though many retinal conditions equally contribute. All causes of blindness and mod

Development and validation of the Prognostic Respiratory Intensity Scoring Metric (PRISM).

AIMS: Invasive respiratory interventions save infant lives, yet have negative consequences for their health and neurodevelopment. Here we present the Prognostic Respiratory Intensity Scoring Metric (PRISM) as an accurate, summative tool of respiratory support needs in hospitalized neonates well suited for predicting an infant’s future neurodevelopmental outcomes. METHODS: 218 (112 male) infants were included in this study. We trained a classification and regression tree model using PRISM scores to classify respiratory diagnoses, and compared performance to the Neonatal Sequential Organ Failure

Risk factor ranking of retinopathy of prematurity using classification and regression tree analysis.

PURPOSE: Numerous risk factors for ROP have been identified, with gestational age and birth weight being the most established. Ongoing improvements in neonatal care may change the critical thresholds for these factors associated with ROP development. We applied a classification and regression tree (CART) analysis to predict the likelihood of developing ROP using a contemporary database. METHODS: This cross-sectional study included 1296 premature infants who completed ROP screening by pediatric ophthalmologists from a children's hospital. Relevant infant and maternal parameters were obtained f

Urinary biomarkers and serum VEGF in preterm infants with retinopathy of prematurity treated with ranibizumab.

BACKGROUND AND OBJECTIVE: Intravitreal injection of anti-vascular endothelial growth factor (VEGF) antibodies is widely used to treat retinopathy of prematurity (ROP), yet, its systemic effects remain unclear. In adults, intravitreal anti-VEGF therapy has been associated with proteinuria and acute kidney injury. This study aimed to evaluate the renal effects of anti-VEGF antibody (Ranibizumab) by longitudinally measuring urinary biomarkers and serum VEGF levels in preterm infants with ROP. METHODS: Preterm infants were categorized into two groups based on ROP treatment status: those receiving
