[The eye as a mirror of the heart-retinal findings in heart failure and cardiovascular diseases].

BACKGROUND: Due to its unique anatomical and functional characteristics, the retina represents a promising organ for the evaluation of systemic vascular diseases. Modern imaging techniques, particularly optical coherence tomography angiography (OCT-A), enable noninvasive and quantitative assessment of retinal microcirculation. The aim of this review is to present current findings on the significance of retinal biomarkers in cardiovascular diseases, with a focus on heart failure. RESULTS: Numerous studies show associations between retinal vascular changes and arterial hypertension, coronary he

Scutellarin attenuates high glucose-induced edema and inflammation in retinal Müller cells via AQP4/Kir4.1 restoration and TLR4/MyD88/NF-κB inhibition.

BACKGROUND: Diabetic retinopathy (DR) is a metabolic eye disease closely linked to inflammation and oxidative stress, potentially causing vision decline or blindness. Scutellarin (SCU), a major flavonoid extracted from Erigeron breviscapus, possesses anti-inflammatory and antioxidant properties; however, its protective mechanisms against retinal Müller cell (RMC) injury in DR remain unclear. OBJECTIVE: To investigate the protective effects and underlying mechanisms of SCU against high-glucose(HG)-induced edema and inflammation in retinal Müller cells (RMCs). MATERIALS AND METHODS: Primary RM

Comparison of retinal thickness and choroidal vascularity index after vitrectomy in idiopathic versus diabetic epiretinal membrane.

BACKGROUND: Epiretinal membrane (ERM) formation, frequently encountered in retinal outpatient practice, is commonly caused by idiopathic and diabetic factors. In this surgically treated condition, choroidal changes occur alongside macular changes following ERM peeling. METHODS: A total of 36 patients who underwent ERM peeling were included in the study: 18 in the idiopathic group and 18 in the diabetic group. Preoperative and postoperative one-month macular and choroidal measurements were obtained using the Heidelberg Engineering Spectralis spectral-domain optical coherence tomography device.

Retinal neovascularization detection from fundus images using deformable attention ResNet and swarm-hiking optimization.

BACKGROUND: In the detection of retinal neovascularization (RNV) from fundus images, defining anatomically consistent perivascular regions is challenging due to asymmetric retinal tissue deformation caused by abnormal vessel proliferation in RNV, leading to inaccurate extraction of pathological features. Moreover, early pathological changes in perivascular zones are minute and blend with normal tissue variability, while existing models focused on global retinal features tend to miss these localized abnormalities, resulting in reduced detection accuracy. METHODS: A novel deep learning (DL) fra

The oxidative and carbonyl stress axis in anti-vascular endothelial growth factor-refractory neovascular age-related macular degeneration: Implications for therapeutic repurposing.

BACKGROUND AND PURPOSE: Neovascular age-related macular degeneration is a leading cause of irreversible blindness, and approximately 25% of patients respond inadequately to intravitreal anti-VEGF therapy, representing a major unmet clinical need. We aimed to identify repurposable therapeutic alternatives by anchoring a phenotypic screen in the molecular signature of inadequate responders. EXPERIMENTAL APPROACH: The molecular phenotype of the disease was characterised by data-independent acquisition proteomic profiling of peripheral blood mononuclear cells from patients with an inadequate anti

Ophthalmic artery angioplasty in a cohort of patients with geographic atrophy secondary to non-exudative age-related macular degeneration.

BACKGROUND: There is increasing evidence that vascular disease and chronic perfusion deficits play a role in the progression of age-related macular degeneration (AMD). This controlled clinical study evaluated the safety and feasibility of treating patients with late-stage AMD and associated geographic atrophy (GA). In addition, exploratory endpoints evaluated the potential to disrupt the disease process and initiate functional changes. METHODS: A total of 17 subjects with confirmed late-stage AMD with GA, ophthalmic artery stenosis, and systemic stability were enrolled in this prospective mul

The correlation between nailfold capillaroscopic findings and adaptive optics imaging of retinal microvasculature in patients with systemic sclerosis.

BACKGROUND: Vascular injury is a central and early feature of systemic sclerosis (SSc) pathogenesis. Although nailfold capillaroscopy (NC) effectively visualizes characteristic peripheral arteriolar and capillary changes, the retinal microcirculation provides a noninvasive, high-resolution view into subtler vascular dysfunction. Consequently, retinal vascular imaging may offer an ideal modality for monitoring microvascular injury and detecting early manifestations of SSc. OBJECTIVES: To compare retinal microvascular parameters between SSc patients and healthy controls using adaptive optics (A

Modulating One-Carbon Metabolism with B-Vitamins to Protect the Retinal Barrier and Prevent Retinal Degeneration.

BACKGROUND/OBJECTIVES: Vitamin B12 deficiency is increasingly recognized as a contributor in both vascular and neurodegenerative aging-related disorders. Its deficiency disrupts one-carbon metabolism, leading to impaired homocysteine (Hcy) cycling. Elevated Hcy is a well-established risk factor for vascular dysfunction. Previously, we established that elevated Hcy contributes to aging retinal diseases and plays a central role in blood retinal barrier (BRB) dysfunction. Building on this foundation, the present study examines how B-vitamin deficiency disrupts one-carbon metabolism and whether re

Changes in inflammatory blood parameters in patients with radiation-related complications due to radiotherapy for uveal melanoma.

CLINICAL RELEVANCE: Radiation retinopathy is a common cause of visual impairment following ocular radiotherapy, and systemic inflammatory activity may contribute to retinal vascular damage. Understanding inflammatory mechanisms may support earlier identification of patients at risk and improve clinical monitoring strategies. BACKGROUND: Radiation retinopathy is a vision-threatening complication of radiotherapy for uveal melanoma and has traditionally been considered a localised microvascular disorder. However, the potential contribution of systemic inflammation to its development remains uncl

Optical Coherence Tomography and Angiography in Hydroxychloroquine Retinopathy: A Narrative Review.

Background/Objectives: Hydroxychloroquine (HCQ) is widely used in the treatment of autoimmune rheumatologic diseases due to its immunomodulatory and anti-inflammatory properties. However, long-term HCQ therapy carries a risk of irreversible retinal toxicity caused by drug accumulation in the retinal pigment epithelium. The early identification of preclinical retinal changes is essential to prevent permanent visual impairment. Optical coherence tomography (OCT) and OCT-angiography (OCT-A) have emerged as key imaging modalities for the detection of structural and microvascular biomarkers of HCQ
