Central serous chorioretinopathy and indocyanine green angiography.

BACKGROUND: Because of limitations in imaging through the retinal pigment epithelium (RPE), fluorescein angiography has not been able to characterize the choroidal abnormalities that are thought to be causative factors in central serous chorioretinopathy (CSC). METHODS: Digital indocyanine green (ICG) videoangiography and fluorescein angiography were performed in 34 consecutive patients with various forms of CSC to investigate choroidal abnormalities. RESULTS: The ICG videoangiographic studies revealed choroidal staining in association with active, spontaneously resolved, and previously phot

[Ocular findings in former premature infants at the age of 11 to 14 years].

BACKGROUND: Functional disturbances as well as morphologic changes can be various in regressed retinopathy of prematurity (ROP). PATIENTS AND METHODS: To investigate the incidence and the degree of such changes, we examined prematurely born infants in the age of 11 to 14 years. Of the 142 prematures (birthweight < or = 2000 g), who were cared for in their first weeks of life in the University Children's Hospital at the Klinikum Mannheim in 1977 to 1979, 48 could be reexamined. RESULTS: Of 48 infants (mean birth weight 1808 +/- 211 g, gestational age 34.6 +/- 3.1 weeks) six (12.5%; birth weig

Clinical features of a previously undescribed codon 216 (proline to serine) mutation in the peripherin/retinal degeneration slow gene in autosomal dominant retinitis pigmentosa.

BACKGROUND: Mutations in the human peripherin/retinal degeneration slow (rds) gene have been found in patients with macular dystrophies as well as in those with autosomal dominant retinitis pigmentosa. The authors studied the clinical features in members of two families with autosomal dominant retinitis pigmentosa and a previously unreported mutation in the peripherin/rds gene. METHODS: Affected family members underwent a clinical ophthalmic examination and electrophysiologic and psychophysical testing. Available family members were evaluated for a mutation in the peripherin/rds gene. RESULT

Amelioration of diabetes-like retinal changes in galactose-fed dogs.

Diabetic retinopathy, the leading cause of blindness among young adults in the developed world, is characterized by vascular changes of the retinal capillary bed. Beagles fed a diet containing 30% galactose develop retinal vascular lesions that are similar to those observed in diabetics. These progress from initial retinal changes which include aldose-reductase-linked formation of pericyte ghosts and the subsequent development of acellular capillaries, microaneurysms, and intraretinal hemorrhages to the appearance of occluded vessels, areas of nonperfusion, and intraretinal microvascular abnor

Concurrent sickle cell hemoglobin C disease and diabetes mellitus: no added risk of proliferative retinopathy?

Of 148 adults with sickle cell hemoglobin C (SC) disease seen at Cook County Hospital and Clinics, Chicago, Illinois, eight patients had coexistent noninsulin-dependent diabetes mellitus. The clinical findings were notable for the paucity of retinal vascular changes. No patient showed proliferative diabetic retinopathy; one patient showed background diabetic retinopathy consisting of a few microaneurysms. Six of the eight patients showed no lesions of proliferative sickle retinopathy. Coexistence of noninsulin-dependent diabetes mellitus and SC disease does not appear to have an additive adver

[Proliferative retinopathy in chronic myeloid leukemia].

BACKGROUND: In 50% of patients with acute and chronic myelocytic leukemia retinal alterations occur which are similar to those of nonproliferative diabetic retinopathy or hypertensive retinopathy. Retinal neovascularizations, however, are rarely seen and only found in patients with the chronic disease. PATIENTS AND METHODS: We report on the clinical course of three patients with advanced stage of proliferative retinopathy in CML. In two patients, the diagnosis of CML was established after ocular involvement. Therapy was planned according to the principles, which have proven effective in other

Early changes in diabetic retinopathy: capillary loss and blood-retina barrier permeability in relation to metabolic control.

In diabetic retinopathy capillary loss and blood-retina barrier leakage are prominent factors. We present a study with quantitative measurements of both capillary loss and leakage and their relation to cumulative metabolic control. Seventeen insulin-dependent diabetic patients with no retinopathy or only mild background retinopathy and 10 years' duration of the disease were included in the study. Status of metabolic regulation had been followed for at least 6 years. Seven healthy subjects were included as controls. In diabetic patients the perifoveal intervascular areas were found to increase

Ocular blood flow velocity reduction after buckling surgery.

BACKGROUND: It has been reported that scleral buckling reduces the blood flow velocity in retinal vessels. Blood flow changes may also appear in other ocular and extraocular vessels. This study describes the blood flow velocity changes in the ophthalmic artery (OA) after performing this procedure. METHODS: The study was carried out in 12 patients (12 eyes) with rhegmatogenous retinal detachment. Color Doppler imaging was used to measure the peak and average blood flow velocity in the OA. Measurements were taken 1 day before and 2 days after scleral buckling surgery was performed. Intraocular

Progressive proliferative diabetic retinopathy after transplantation of the pancreas. A case and a review of the topic.

The unusual case history of the development of a progressive diabetic retinopathy despite normal blood glucose after transplantation of the pancreas and despite panretinal laser coagulations is described. A 26-year-old male with diabetic background retinopathy underwent a segmental transplantation of the pancreas in 1988. One year after surgery, a progressive proliferative retinopathy despite normoglycemia was found, which required an intensive panretinal photocoagulation during the following months. However, despite massive retinal coagulations, vitreous hemorrhages and retinal neovasculariza

Histopathology of diabetic retinopathy in man.

The retinal changes associated with diabetes mellitus are a consequence of the systemic microangiopathy with modifications related to the intraocular environment. The vascular disorders underlying background retinopathy are arteriolar hyalinosis (which together with abnormalities in the circulating blood can give rise to focal capillary closure), venular dilatation, and capillaropathy in the form of pericyte degeneration, basement membrane thickening and microaneurysm formation. Retinal complications consist of plasma exudation and punctate haemorrhages. Maculopathy is due to cystoid oedema. I

Clinical features of autosomal dominant retinitis pigmentosa with rhodopsin gene codon 17 mutation and retinal neovascularization in a Japanese patient.

A 49-year-old Japanese man had autosomal dominant retinitis pigmentosa with a point mutation in codon 17 of the rhodopsin gene, resulting in a threonine-to-methionine change, and retinal neovascularization in both eyes. Pigmentary degeneration mainly in the inferior area of the fundus, and severe loss in the upper portion of the visual field were observed. Moderately preserved rod and cone functions were demonstrated by electroretinograms. These findings differed from those of Japanese and white patients with autosomal dominant retinitis pigmentosa with a codon 347 mutation and were almost the

Ophthalmic complications after heart transplantation.

To ascertain the ophthalmic complications after heart transplantation, the authors reviewed 59 patients who had been referred to the ophthalmology department during a 4-year period and were subsequently followed for at least 1 year. Twenty-five patients (43%) were found to have lens changes in one or both eyes, which is typical of prolonged oral corticosteroid therapy. In three patients (5.2%) posterior subcapsular cataract formation progressed to a level where cataract surgery was required. In two patients (3.4%) cytomegalovirus retinitis developed within 6 months of the transplantation, and

Retinal vascular changes in congenital hypertrophy of the retinal pigment epithelium.

BACKGROUND: Congenital hypertrophy of the retinal pigment epithelium (CHRPE) is a well-defined clinical entity with usually well-delineated, pigmented oval or round lesion with smooth or scalloped margins. Occasional retinal vascular changes have been reported previously. PURPOSE AND METHOD: To assess the prevalence of these changes, the authors performed a retrospective analysis of 12 patients with CHRPE, for whom fluorescein angiography allowed visualization of the entire lesion and of the retinal vascular capillary bed. RESULTS: Retinal vascular changes were found in 11 (91%) of these 12

Successful scleral buckling procedures decrease central retinal artery blood flow velocity.

BACKGROUND: Limited data suggest that scleral buckling procedures may decrease posterior segment blood flow. How this may affect functional outcome remains unknown. METHODS: In a prospective, controlled study, color Doppler imaging was used to measure blood flow velocities in the central retinal and ophthalmic arteries of both eyes in patients with unilateral rhegmatogenous retinal detachments before and after primary scleral buckle repair. Encircling elements were used in all patients. Fellow eyes served as controls. RESULTS: Data from six consecutive patients did not show significant preop

Hemodynamic alterations in the acute retinal necrosis syndrome.

BACKGROUND: Clinical and histopathologic observations suggest a role for ischemia in the pathogenesis of the acute retinal necrosis (ARN) syndrome. Disruption of blood flow appears to occur at some level in the retina or choroid and may account for some of the major features of the syndrome. METHODS: To investigate these potential circulatory changes, color Doppler imaging (CDI) was used to quantitate blood flow velocities and vascular resistance in the central retinal, ophthalmic, and short posterior ciliary arteries in ten consecutive patients with unilateral ARN syndrome. Data were analyze

[Lipoprotein (a) and sclerotic changes in retinal arterioles].

Lipoprotein (a) (Lp(a)) has been considered an independent risk factor for arteriosclerotic disease and its role in retinal vascular changes was studied in this report. The study was made on 160 patients of age 54 and above. They were divided into four groups depending upon their clinical background with or without the presence of diabetes mellitus (DM) and cerebral infarction (CI). The retinal vascular status of the patients was studied by ophthalmoscopic examination and the findings were graded according to Scheie's classification. We found that all the patients with retinal arterial hyperte

[Fundus-controlled microperimetry with the scanning laser ophthalmoscope in macular diseases].

BACKGROUND: Scanning laser microperimetry allows focal sensitivity testing of any localization of the retina under direct visual control. Physiological or involuntary fixation shifts can be corrected by manual fundus-tracking. METHODS: For static fundus-controlled microperimetry we used a confocal scanning laser ophthalmoscope equipped with a helium-neon laser as light source for background illumination and for generating the stimuli by acousto-optic modulation. Fundus imaging was achieved by an infrared diode laser scanning synchronous with the helium-neon laser. In comparison computerized s
