Association between risk-adjusted mortality and severe morbidity in very low birth weight infants: a multicenter study from the NEOCOSUR network.

BACKGROUND: Very low birth weight infants (VLBWI) are at high risk of mortality and severe morbidities. While many neonatal networks monitor risk-adjusted outcomes separately, the relationship between mortality and major morbidities across centers remains unclear. OBJECTIVE: To evaluate the association between risk-adjusted in-hospital mortality and six major morbidities in VLBWI across 20 South American NICUs participating in the NEOCOSUR Network between 2011 and 2023. METHODS: Retrospective cohort study including 14,100 VLBWI (500-1500 g) from 20 NICUs in Argentina, Chile, Paraguay, Peru,

Use of Aflibercept to Treat Retinopathy of Prematurity in Children with Extremely Low Birth Weight.

Background: Aflibercept is one of the anti-VEGF drugs used, among others, for the treatment of retinopathy of prematurity, alongside the widely used bevacizuab and ranibizumab. It is a recombinant fusion protein composed of the human VEGFR-1 and VEGFR-2 domains combined with the Fc part of human IgG, called VEGF-TRAP. The paper describes a group of premature infants treated with aflibercept due to retinopathy of prematurity at the Regional Specialized Children's Hospital in Olsztyn, Poland, in the years 2017-2019. Methods: Eleven children (22 eyes) with extremely low birth weight and type 1 RO

Polyunsaturated Fatty Acid Balance Modulates Microglial State in a Murine Model of Oxygen-Induced Neovascularization.

Background/Objectives: The retina is enriched in polyunsaturated fatty acids (PUFAs) which are indispensable for normal vision, and recent clinical studies have shown that dietary supplementation of ω-6-and ω-3-polyunsaturated fatty acids (PUFAs) can provide a protective role against retinopathy of prematurity (ROP). Our study aims to understand the mechanisms by which altering ω-6-and ω-3-polyunsaturated fatty acids (PUFAs) in the eye can protect against pathologic retinal neovascularization (NV). Methods: We interrogated the effects of endogenous ω-3-PUFA enrichment using transgenic fat-1 mi

Understanding the Molecular Basis of Pathological Retinal Angiogenesis: Roles of Lipids, Non-Coding RNAs, and Cellular Crosstalk.

Pathological retinal neovascularization is a prevalent cause of blindness, impacting millions of individuals across all age groups, from infants to older adults. It is a key clinical feature of several retinal diseases, such as diabetic retinopathy (DR), retinopathy of prematurity (ROP), age-related macular degeneration (AMD), and Coats disease. Researchers have extensively investigated the involvement of vascular endothelial growth factor (VEGF) signaling, hypoxia inducible factor (HIF) signaling, and inflammation in the regulation of pathological retinal neovascularization. Recent breakthrou

From Science and Technology to Eye-Health Policy: A Review on Global and Asia-Pacific Strategies on the Prevention of Blindness from Retinal Diseases.

The World Health Organization (WHO) is making a concerted effort to enhance health, including eye health and overall well-being. This global effort has resulted in a reduced age-standardized prevalence of avoidable blindness and no increase in avoidable moderate and severe vision impairment. However, the absolute number of cases has increased for both avoidable blindness and moderate-to-severe vision impairment. Globally, the leading causes of visual impairment include uncorrected refractive errors and cataract, though many retinal conditions equally contribute. All causes of blindness and mod

Development and validation of the Prognostic Respiratory Intensity Scoring Metric (PRISM).

AIMS: Invasive respiratory interventions save infant lives, yet have negative consequences for their health and neurodevelopment. Here we present the Prognostic Respiratory Intensity Scoring Metric (PRISM) as an accurate, summative tool of respiratory support needs in hospitalized neonates well suited for predicting an infant’s future neurodevelopmental outcomes. METHODS: 218 (112 male) infants were included in this study. We trained a classification and regression tree model using PRISM scores to classify respiratory diagnoses, and compared performance to the Neonatal Sequential Organ Failure

Risk factor ranking of retinopathy of prematurity using classification and regression tree analysis.

PURPOSE: Numerous risk factors for ROP have been identified, with gestational age and birth weight being the most established. Ongoing improvements in neonatal care may change the critical thresholds for these factors associated with ROP development. We applied a classification and regression tree (CART) analysis to predict the likelihood of developing ROP using a contemporary database. METHODS: This cross-sectional study included 1296 premature infants who completed ROP screening by pediatric ophthalmologists from a children's hospital. Relevant infant and maternal parameters were obtained f

Urinary biomarkers and serum VEGF in preterm infants with retinopathy of prematurity treated with ranibizumab.

BACKGROUND AND OBJECTIVE: Intravitreal injection of anti-vascular endothelial growth factor (VEGF) antibodies is widely used to treat retinopathy of prematurity (ROP), yet, its systemic effects remain unclear. In adults, intravitreal anti-VEGF therapy has been associated with proteinuria and acute kidney injury. This study aimed to evaluate the renal effects of anti-VEGF antibody (Ranibizumab) by longitudinally measuring urinary biomarkers and serum VEGF levels in preterm infants with ROP. METHODS: Preterm infants were categorized into two groups based on ROP treatment status: those receiving

360-Degree Sclerosed Vessels and Candle Wax-Like Deposits as an Atypical Exudative Presentation of Aggressive Retinopathy of Prematurity in a Neonate With Severe Anaemia.

We report a finding of 360-degree completely sclerosed vessels presenting in aggressive retinopathy of prematurity (A-ROP) in a neonate with severe anaemia. A one-month-old baby with a history of neonatal intensive care unit (NICU) admission for respiratory distress and multiple blood transfusions for systemic anaemia was referred to our hospital with A-ROP. Posterior pole examination revealed candle wax-like exudative deposits along the retinal vessels, with areas of early vascular sclerosis. Ultrawide field fundus photo documentation for both eyes at various presentations, including after th

Impact of a Structured Brain Care Bundle on Severe Germinal Matrix Hemorrhage-Intraventricular Hemorrhage in Very Preterm Neonates: A Retrospective Cohort Study.

BACKGROUND: Germinal matrix hemorrhage-intraventricular hemorrhage (GMH-IVH) remains a leading cause of neurologic morbidity and mortality in very preterm infants. Evidence supports the assertion that standardized neuroprotective "Brain Care Bundles" (BCBs) can reduce GMH-IVH risk, but data from Middle Eastern Level III neonatal intensive care units are limited. METHODS: We performed a retrospective cohort study comparing pre-BCBs (January-December 2021; n = 80) versus post-BCB (January-December 2022; n = 77) cohorts of inborn infants ≤30 weeks' gestation and or birth weight ≤1500 g.
