Occlusive retinal vasculitis in a patient with West Nile virus.

BACKGROUND: West Nile virus (WNV) was first identified in the United States in 1999. In addition to a spectrum of systemic manifestations, several ocular conditions secondary to the virus have been reported, including chorioretinitis, uveitis and optic neuritis. Age and diabetes mellitus (DM) have been reported to be associated risk factors for the more severe forms of the systemic disease. Only seven cases of occlusive retinal vasculitis have been reported in patients with WNV infection. CASE HISTORY: A 60-year-old Asian male presented with complaints of decreased vision in his left eye. He

UK retinopathy of prematurity treatment survey.

BACKGROUND: Since the CRYO-ROP study showed the benefit of treating babies with ROP, a number of different treatment modalities have become available. Solid-state lasers have been extensively used in the treatment of ROP since their introduction some 10 years ago. Diode, argon and frequency-doubled YAG (FD-YAG) lasers have all been used to perform photocoagulation in ROP. We aimed to assess current treatment practices of ROP treatment in the UK. METHODS: A questionnaire survey was performed in June 2004 and sent to all consultants known to be involved in the treatment or screening of ROP pati

Large interface particles from LASIK surgery.

BACKGROUND: Down syndrome is a common chromosomal anomaly. Few reported studies make reference to the ocular status in Asian children with Down syndrome. The purpose of this study was to determine the visual and binocular status of a sample of Down syndrome children in Malaysia. METHODS: A total of 73 Malaysian children with Down syndrome (38 boys and 35 girls) in the Kuala Lumpur area aged one to 12 years were examined. Cycloplegic refraction was performed on each eye and binocular visual acuity was assessed using the Cardiff acuity card or the LogMAR chart. Binocular functions were assessed

Non-accidental subdural haemorrhage in Hong Kong: incidence, clinical features, management and outcome.

OBJECTIVE: We aim to identify the incidence and the characteristics of non-accidental subdural haemorrhage in Hong Kong in children under the age of 5 years. METHOD: Eighteen children aged below 5 years presented with subdural haemorrhage without a history of significant trauma were studied. Clinical features and the work-up process for probable child abuse were analysed. RESULTS: Sixteen combined case conferences were held, and eleven cases were concluded to be genuine child abuse cases of non-accidental head injury (incidence=1.5 per 100,000 children <5 years old per year). Among these cas

Paroxysmal cough injury, vascular rupture and 'shaken baby syndrome'.

It is widely assumed that subdural and retinal haemorrhage in infants can only result from traumatic rupture of vulnerable blood vessels. An alternative aetiology, that of vascular rupture resulting from excessive intraluminal pressure, is presented in three disease conditions. (1) Perlman et al., studying premature neonates requiring mechanical ventilation for respiratory distress syndrome, observed "cough-like" fluctuations in oesophageal pressure greater than 18 cms H2O, whose timing matched fluctuations in anterior cerebral artery flow. When 14 out of 24 neonates were paralysed (to prevent

[Telemedical-supported screening of retinal vessels ("talking eyes")].

BACKGROUND: Cerebral and retinal vessels behave similarly under the influence of vascular risk factors. Several groups have shown that retinal microvascular abnormalities represent an independent risk factor with regard to strokes and heart attacks. AIM OF THE STUDY: The aim of this study was to perform a prospective screening examination with regard to retinal microvascular abnormalities as well as an extended vessel diagnosis in a subgroup of patients with lower arteriovenous risk values. METHODS: In the course of a prospective cross-sectional study ("Talking Eyes") between 1.9.2001 and 1.

Use of antiplatelet therapy in a diabetic outpatient service of a large urban public hospital.

BACKGROUND AND AIM: Cardiovascular disease is the most important cause of mortality in type 2 diabetes. Aspirin treatment is effective in diabetic patients with cardiovascular disease and it does not significantly increase the risk of retinal haemorrhage, gastrointestinal bleeding or hemorrhagic stroke. The American Diabetes Association (ADA) recommends the use of aspirin in all adult patients with diabetes and macrovascular disease, and suggests to start treatment with aspirin for primary prevention in diabetic patients >or=40 years of age and with one or more other cardiovascular risk factor

Visual outcomes of pan-retinal photocoagulation in diabetic retinopathy at one-year follow-up and associated risk factors.

PURPOSE: To assess the visual outcomes at one-year follow-up after pan-retinal photocoagulation (PRP) in type 2 diabetes mellitus subjects with proliferative diabetic retinopathy (PDR) and associated risk factors. MATERIALS AND METHODS: A retrospective study, using data from medical records of 5000 Type 2 diabetic patients who underwent a retinal examination between 1995 and 1999 at a diabetic centre. Ocular, clinical and biochemical parameters were assessed at baseline and at one-year follow-up after PRP. Diabetic retinopathy (DR) was documented by colour photography and PRP was performed ac

The 'Sutured Skull' and intracranial bleeding in infants.

It is known that retinal haemorrhages can result in adults when elevated intrathoracic pressures due to coughing, cardiopulmonary resuscitation, etc., force blood into the head. In infants under one year of age retinal and intracranial haemorrhage commonly occur together, but the same is not true for the older child and adult. The role of the elasticity of the infant skull (resulting from suture and fontanelle stretching) compared to the rigid mature skull, was investigated in a computer aided method. This showed that although in the event of Valsalva-like situations very high lumen pressures

Retinal haemorrhage in vivax malaria.

Retinal haemorrhage is often observed in patients with Plasmodium falciparum, especially when combined with cerebral malaria. However, few cases of retinopathy have been reported in P. vivax malaria. Benign tertian malaria has re-emerged among soldiers in the South Korean demilitarized zone since 1993. We report an indigenous case of retinal haemorrhage caused by P. vivax and review the relevant literature.

Infantile subdural haematoma in Auckland, New Zealand: 1988-1998.

AIM: To review the Auckland experience of traumatic subdural haematoma (SDH) in infants under 2 years of age, with particular regard to features which might help to differentiate accidental from non-accidental injury (NAI). METHODS: Retrospective review of the medical records of children under 2 years of age, coded for subdural haematoma (SDH) and retinal haemorrhage (RH) over a 10-year period. RESULTS: Sixty-four cases of SDH were identified. Forty-one of these were due to non-accidental trauma, and 23 cases were accidental. Differences between these two groups related to the age of present

Violence is not necessary to produce subdural and retinal haemorrhage: a reply to Punt et al.

In this article we reply to the recent critique by Punt et al. in Pediatric Rehabilitation. Our hypothesis about the pathogenesis of intracranial bleeding in infants has three important implications. First, in the case of an infant with a swollen brain, subdural and retinal haemorrhage but no objective evidence of trauma, the findings by themselves are not certain evidence of abuse; second, violence is not necessary to produce subdural and retinal haemorrhage; and lastly, non-traumatic events producing apnoea with a catastrophic rise in intracranial pressure could produce a clinical picture id

Is screening for interferon retinopathy in hepatitis C justified?

BACKGROUND/AIM: In the treatment of hepatitis C, the National Institute for Clinical Excellence advocates use of a combination of interferon alfa and ribavirin for selected patients. Retinopathy is a well recognised side effect of interferon therapy and is characterised by retinal haemorrhages, cotton wool spots, and macular oedema. The aim of this study was to document the incidence and natural history of the retinopathy in patients treated with a long acting (pegylated) interferon and ribavirin for hepatitis C and to assess the need to screen for retinal complications. METHODS: All patients

[Lumbar puncture and early neuroimaging in complex febrile seizures. Report of a case of shaken infant syndrome].

INTRODUCTION: Febrile convulsions are one of the most frequent pathologies seen in paediatric emergencies. The diagnosis of febrile seizures is clinico evolutionary and is easily established once the acute process is overcome and a normal state is restored in the child. The differential diagnosis is established with the processes that associate fever and convulsions in children between the ages of 1 month and 6 years, many of which require specific treatment. Certain complementary examinations, essentially a blood test, lumbar puncture and neuroimaging, are needed to identify them. Shaken infa

Forensic web watch--medicolegal aspects of paediatric pathology.

The trials over the deaths of Matthew Eappen and Victoria Climbie have highlighted the importance of forensic evidence in cases of suspected child abuse. The debate as to whether bruises, fractures or head injuries have been sustained as a result of previous trauma or non-accidental injury is central to these, and other, cases. A variety of subjects are encountered in forensic paediatric pathology, including Shaken Baby Syndrome, non-accidental injury, retinal haemorrhage, skeletal injury, Sudden Infant Death, sexual abuse and Munchausen Syndrome by Proxy. The coverage of these areas on the in

Avidity progression of dietary antibodies in healthy and coeliac children.

BACKGROUND: Klebsiella pneumoniae is a Gram-negative bacterial species well known for its capacity to cause infections in humans, and to carry and spread a wide variety of resistance genes including extended-spectrum beta-lactamase genes, carbapenem resistance genes, and colistin resistance genes. Recently, our real-time laboratory-based surveillance system MARSS (the Marseille Antibiotic Resistance Surveillance System) allowed us to observe a intringing dramatic decrease in the beta-lactam resistance level of the K. pneumoniae strains routinely isolated from patients hospitalized in our setti
