A cell fate specification and transition mechanism for human foveolar cone subtype patterning.

In the central region of the human retina, the high-acuity foveola is notable for its dense packing of green (M) and red (L) cones and absence of blue (S) cones. To identify mechanisms that pattern cones in the foveola, we examined human fetal retinas and differentiated retinal organoids. During development, sparse S-opsin-expressing cones are initially observed in the foveola. Later in fetal development, the foveola contains a mix of cones that either coexpress S- and M/L-opsins or exclusively express M/L-opsin. In adults, only M/L cones are present. Two signaling pathway regulators are highl

Retinal electrophysiology in mood disorders: Toward clinically relevant biomarkers. A systematic review and meta-analysis.

Mood disorders, including major depressive disorder (MDD) and bipolar disorder (BD), are associated with substantial disability and frequent misdiagnosis, particularly in early stages and in the differential diagnosis between unipolar and bipolar depression. Current diagnosis remains largely based on clinical assessment, highlighting the need for objective biomarkers to improve early detection and diagnostic accuracy. The retina, sharing structural and neurochemical properties with the brain, is a promising candidate. To assess its function, Electroretinography (ERG) is widely used. This study

Remote Ischemic Conditioning for Non-Proliferative Diabetic Retinopathy (RIC-NPDR): study protocol for a randomized controlled trial.

BACKGROUND: Diabetic retinopathy (DR) is a leading cause of blindness in diabetic patients. However, effective treatments for non-proliferative DR (NPDR) are limited, especially in the early stage. Remote ischemic conditioning (RIC) has shown great promise in protecting retinal tissues from ischemic damage, but its therapeutic potential in NPDR remains unclear. This study aims to evaluate the safety and efficacy of RIC in patients with mild to moderate NPDR and to provide preliminary data for the design of future multicenter clinical trials. METHODS: A single-center, randomized, controlled tri

Age at diabetes onset modifies the association between hyperuricemia and diabetic retinopathy: a real-world cross-sectional study.

BACKGROUND: The role of hyperuricemia in diabetic retinopathy (DR) remains controversial, with limited evidence on how age at diabetes onset modifies this association. This study aimed to investigate the differential association of elevated uric acid (UA) with DR prevalence stratified by age at diagnosis. METHODS: We analysed cross-sectional data from 6,996 patients with type 2 diabetes mellitus, grouped by age at diabetes onset (younger-onset, < 65 years; older-onset, ≥ 65 years) and UA status (normal vs. elevated: ≥420 µmol/L for men; ≥380 µmol/L for women). Single Large-Scale Mitochondrial Deletion Syndromes: Neuroimaging Phenotypes and Longitudinal Progression in Pediatric Patients.

BACKGROUND AND PURPOSE: Single large-scale mitochondrial deletion syndrome (SLSMD) comprises devastating mitochondrial diseases often classified into 3 major clinical syndromes: Kearns-Sayre syndrome (KSS), chronic progressive external ophthalmoplegia (CPEO), and Pearson syndrome (PS). Nevertheless, there remains large clinical variability and overlap among these SLSMD groups. Therefore, further stratification is required for more precise prognostication and clinical management. Through detailed description and analysis of longitudinal neuroimaging changes, we sought to determine the neuroradi

[Optical coherence tomography angiography (OCT-A) and the (non-glaucomatous) optic nerve].

Optical coherence tomography angiography (OCTA) is a non-invasive, reliable, repeatable exam. As opposed to fluorescein angiography, OCTA allows analysis of the capillary bed without interference from diffusion phenomena. It is now widely used for both the diagnosis and ongoing care of retinal and choroidal diseases. However, its use is still limited for evaluation of the optic nerve. Recently, new data quantification software can add value to macular and papillary vascularization images. This review details OCTA results in the most important optic nerve conditions, whether acquired or congeni

Home-monitoring for neovascular age-related macular degeneration in older adults within the UK: the MONARCH diagnostic accuracy study.

BACKGROUND: Most neovascular age-related macular degeneration treatments involve long-term follow-up of disease activity. Home monitoring would reduce the burden on patients and those they depend on for transport, and release clinic appointments for other patients. The study aimed to evaluate three home-monitoring tests for patients to use to detect active neovascular age-related macular degeneration compared with diagnosing active neovascular age-related macular degeneration by hospital follow-up. OBJECTIVES: There were five objectives: Estimate the accuracy of three home-monitoring tests to

Artificial intelligence for diagnosing exudative age-related macular degeneration.

BACKGROUND: Age-related macular degeneration (AMD) is a retinal disorder characterized by central retinal (macular) damage. Approximately 10% to 20% of non-exudative AMD cases progress to the exudative form, which may result in rapid deterioration of central vision. Individuals with exudative AMD (eAMD) need prompt consultation with retinal specialists to minimize the risk and extent of vision loss. Traditional methods of diagnosing ophthalmic disease rely on clinical evaluation and multiple imaging techniques, which can be resource-consuming. Tests leveraging artificial intelligence (AI) hold

MOGAD: A comprehensive review of clinicoradiological features, therapy and outcomes in 4699 patients globally.

Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is one of the most common antibody-mediated CNS disorders. Optimal diagnostic and prognostic biomarkers remain unclear. Our aim was to clarify these biomarkers and therapeutic outcomes internationally. We reviewed articles from 2007 to 2022 and identified 194 unique cohorts encompassing 4699 paediatric and adult patients from 31 countries. Where phenotypes were specified, the most common initial presentation overall was optic neuritis (ON; paediatric 34 %; adults 60 %), during which 71 % had papilloedema on fundoscopy. The
