Tobacco smoking is not associated with primary open-angle glaucoma: a systematic review and meta-analysis.

PURPOSE: This systematic review explores the relationship between tobacco smoking and the development of primary open-angle glaucoma (POAG). Glaucoma is the leading cause of irreversible blindness worldwide. Prior literature investigating the link between tobacco smoking and glaucoma has reported contradictory findings on the association between tobacco smoking and POAG. METHODS: Systematically EMBASE, MEDLINE, and Web of Science were searched, encompassing articles published up until April 2025. The inclusion criteria comprised observational and randomised controlled studies that provided a

Deep-learning-derived glaucoma-related endophenotypes enable novel genome-wide genetic and functional discovery.

The genetic architecture of primary open-angle glaucoma (POAG), a leading cause of irreversible blindness, remains largely unexplained due to the reliance of previous genome-wide association studies (GWAS) on imprecise phenotypes from electronic health records. Here, we overcome this with a disease-trained, task-transfer machine learning (ML) framework that learns glaucoma-related damage patterns from a large clinical repository of 8,323 glaucoma patients. We showed that ML optical coherence tomography (OCT)-derived endophenotypes trained on 18,985 OCT scans from these patients identified nove

Revealing the role of regulatory microglial IRF7-NLRP3 interactions in optic nerve damage of normal-tension glaucoma based on single-cell RNA sequencing.

Normal-tension glaucoma (NTG) is a subtype of primary open-angle glaucoma (POAG). Patients with NTG still experience significant optic nerve damage despite maintaining normal intraocular pressures. The mechanism of optic nerve damage in glaucoma with normal pressure is still unclear. Research has shown that OPTN (E50K) mutations exacerbate the inflammatory response of retinal microglia. However, there is still a lack of evidence on how OPTN (E50K) mutations directly regulate their inflammatory pathways through key molecules. This study explores the role of microglial inflammation caused by the

Development of deep learning model to screen for primary open-angle glaucoma in African ancestry individuals.

Primary open-angle glaucoma (POAG) screening using artificial intelligence (AI) has emerged as a transformative method to identify undiagnosed disease. African ancestry individuals are under-represented in current datasets for AI models, despite being disproportionally affected by this blinding disease. We developed a deep learning model that screens for POAG using fundus photography from Primary Open-Angle African American Glaucoma Genetics (POAAGG) subjects (n = 64,129 images, including 42,914 images from 1782 cases and 21,215 images from 682 controls). Our final diagnosis pipeline is as fol

From first generation to the next: evolution and research trends in iStent technology.

The iStent series constitutes a range of trabecular bypass minimally invasive glaucoma surgery (MIGS) devices, which offers intraocular pressure (IOP) reduction with favourable safety profiles, in patients with open-angle glaucoma (OAG). Having undergone significant evolution since its initial US FDA-approval in 2012, successive generations address previous limitations, while enhancing IOP-lowering efficacy through device and delivery system design iterations. Longer-term and real-world iStent data demonstrate the durability of IOP- and medication-lowering outcomes with minimal complications,

Prognostic factors of optical coherence tomography angiography in intermediate and advanced primary open-angle glaucoma: A systematic review and meta-analysis.

Conventional optical coherence tomography (OCT) has a floor effect in patients with severe visual field loss, such as seen in advanced primary open-angle glaucoma (POAG). OCT angiography (OCTA) does not suffer from such a floor effect. However, which OCTA parameters are most useful for monitoring longitudinal progression is unclear. We conducted a systematic review and meta-analysis to investigate the clinical use of OCTA in monitoring the progression of intermediate and advanced POAG by searching four databases (Medline, Embase, Web of Science and Cochrane Database of Systematic Reviews) for

Segmental outflow and trabecular meshwork stiffness in an ocular hypertensive mouse model.

PURPOSE: Elevated intraocular pressure (IOP) due to increased outflow resistance through the trabecular meshwork (TM) is a major risk factor for primary open-angle glaucoma. Outflow through the TM is segmental, consisting of high flow (HF) and low flow (LF) regions. Here, we investigate how ocular hypertension impacts segmental outflow using a dexamethasone (DEX) mouse model and compare TM stiffness between HF and LF regions. METHODS: Nanoparticles containing DEX or vehicle were injected twice weekly in 2-4-month-old C57BL/6J mice (n=14), and IOP was measured weekly. At week 4, mouse eyes wer

Longitudinal Association Between Disc Hemorrhage and Primary Open-Angle Glaucoma Progression: A Systematic Review and Meta-Analysis.

PURPOSE: Optic disc hemorrhages (DH) are one of the hallmark signs of primary open-angle glaucoma (POAG). However, due to the scarcity of longitudinal reports, the influence of baseline DH on the progression of POAG and its subtype, normal tension glaucoma (NTG), remains unclear. We conducted a systematic review with meta-analysis to determine the association of baseline DH with progression of POAG and NTG. METHODS: PubMed, Embase, Cochrane, SCOPUS, and Web of Science were searched from inception till July 1, 2025. POAG and NTG progressions were defined based on functional or structural chang
