Impact of Glucagon-Like Peptide 1 (GLP-1) and Gastric Inhibitory Polypeptide (GIP) Receptor Agonists on Intraocular Pressure in Glaucoma: A Case Series.

PURPOSE: Glucagon-Like Peptide 1 Receptor Agonists (GLP-1RAs) are antidiabetic and weight management medications that bind to GLP-1 receptors throughout the body. Growing interest in this class has led to several retrospective cohort studies reporting a decreased occurrence of glaucoma in diabetic and non-diabetic patients with the use of GLP-1RAs, suggestive of anti-inflammatory and neuroprotective effects. Here we report five real world cases evaluating the impact of GLP-1RA and dual GLP-1/Gastric Inhibitory Polypeptide (GIP) receptor agonist use on Intraocular Pressure (IOP) measurements an

Refractory malignant glaucoma after secondary IOL implantation in an eye with 17-year-old vitrectomy: A case report.

RATIONALE: Malignant glaucoma (MG) results from aqueous misdirection and entrapment in the vitreous cavity. This process drives anterior displacement of the lens-iris diaphragm, causing uniform shallowing of the central and peripheral anterior chamber, typically with elevated intraocular pressure (IOP). Clear lens extraction with anterior vitrectomy remains the definitive intervention. PATIENT CONCERNS: We report a rare case of MG following combined pars plana vitrectomy (PPV) and intraocular lens (IOL) implantation and detail its management. A 49-year-old Chinese male presented with a histor

Inhibition of connexin hemichannels protects retinal ganglion cells against ocular nerve injury.

Retinal ganglion cell (RGC) degeneration caused by optic nerve injury and diseases such as glaucoma leads to irreversible vision loss, yet effective neuroprotective treatments remain elusive. Secondary degeneration driven by astrocytic gliosis and neuroinflammation contributes substantially to neuronal death. Connexin 43 (Cx43), a gap junction protein abundantly expressed in astrocytes, is a key mediator of these secondary responses. Using an optic nerve crush (ONC) mouse model that recapitulates traumatic optic neuropathy, we found that Cx43 haploinsufficiency significantly preserved visual f

Orbital exenteration 14years after evisceration for misdiagnosed choroidal melanoma.

We present a case of orbital recurrence of choroidal melanoma 14years post-evisceration. Initially misdiagnosed as trauma-related, the patient's condition progressed, leading to neovascular glaucoma and retinal detachment. Subsequent pain and complications prompted evisceration without a pathological evaluation. Fourteen years later, an orbital mass was identified, leading to an orbital exenteration. This case demonstrates the potential for long-term local-regional recurrence of choroidal melanoma and highlights the importance of thorough examination and pathological evaluation in cases of a p

Impact of Ocular Massage on Intraocular Pressure and Schlemm Canal Dimensions in Healthy Adults: Protocol for a Randomized Controlled Trial.

BACKGROUND: Ocular massage has been reported to lower intraocular pressure (IOP) temporarily. This effect could be related to enhanced aqueous humor outflow; however, the mechanism is unclear. OBJECTIVE: This study aims to examine the impact of digital and EyePeace ocular massage on IOP fluctuations and investigate whether the observed reduction in IOP is attributable to morphological changes in the Schlemm canal (SC). METHODS: Participants were randomly assigned in a 1:1:1 ratio to 1 of 3 groups: digital ocular massage, EyePeace ocular massage, or no ocular massage. The primary outcome meas

Case report: The diagnostic and surgical value of UBM in phacolytic glaucoma secondary to traumatic lens subluxation.

This case report highlights the diagnostic and surgical value of ultrasound biomicroscopy (UBM) in managing phacolytic glaucoma with traumatic lens subluxation, a rare combination requiring differentiation from other lens-induced glaucomas. UBM precisely identified three pathognomonic features: (1) subluxated lens with reduced thickness yet open angles, (2) cortical liquefaction, and (3) zonular insufficiency, thus overcoming the limitations of conventional imaging modalities. The patient underwent successful phacoemulsification with capsular tension ring implantation, achieving 20/20 vision a

Acute Angle-Closure Glaucoma Secondary to Vitreous Prolapse Following Ocular Trauma.

Secondary acute angle-closure glaucoma (AACG) caused by vitreous prolapse in a phakic patient is exceedingly rare. We report a case of AACG secondary to vitreous prolapse, occurring 2 weeks after orbital trauma in a 34-year-old male healthcare worker. The patient presented with acute left eye pain, nausea, and vision loss after performing cardiopulmonary resuscitation (CPR) at his workplace. Two weeks earlier, he sustained a nonoperative orbital floor fracture and commotio retinae from a motor vehicle accident. Examination revealed decreased vision and an intraocular pressure (IOP) of 59 mmHg.

Systemic cardiac migration of perfluorocarbon liquid following unsuccessful endoresection of choroidal melanoma: a case report.

Choroidal melanoma (CM) is the most common intraocular malignant tumor in adults, with a high rate of metastasis and mortality. This study reports a case of systemic cardiac migration of perfluorocarbon liquid (PFCL) following an unsuccessful endoresection of CM. A 61-year-old woman underwent gamma knife radiosurgery and pars plana vitrectomy endoresection for CM in her left eye. The procedure was complicated by intraocular bleeding, leading to incomplete tumor removal, with the eye filled with PFCL and blood before closure. The patient refused further surgery and later developed secondary gla

Atypical Cause of Headache: A Potential Diagnostic Pitfall in Acute Medicine.

Acute headache is a common clinical presentation with a broad differential diagnosis, ranging from benign to life-threatening conditions. Early recognition of atypical causes is essential, particularly when initial investigations are inconclusive. The etiology of acute headache can be broadly classified into primary and secondary causes. Primary headaches occur in the absence of identifiable structural pathology, whereas secondary headaches arise from an underlying organic cause. In this case presentation, a 65-year-old woman presented with a severe headache, which was triggered by an episode
