The authors present a problem secondary posttraumatic glaucoma caused by perforative eye injuries or contusions of the eyeball. Posttraumatic glaucoma after perforative injuries appears in high percentage 25-67%, with the postcontusion glaucoma is rare, and appears in 2-5% cases. It can appear near after injury but also after 10 years. This type of glaucoma has specific clinic finding, inversive type of tonometry, curve, retention type of curve and long interval without rise intraocular pressure. Prognosis of this glaucoma is very difficult and unsure. Most cases finish with blindness and enuc

A bleb infection and subsequent endophthalmitis developed in the left eye of a 68-year-old man who had had a trabeculectomy. Vitreous injections of vancomycin and gentamicin were given, and vitreous cultures grew alpha-Streptococcus. Postoperatively, as the intraocular inflammation resolved, intraocular pressure (IOP) markedly decreased secondary, in our judgment, to ciliary body ischemia. A pars plana vitrectomy relieved the tractional bands and restored the IOP.

We describe a 49-year-old white man with sympathetic ophthalmia following contact neodymium-YAG laser cyclotherapy. There was no invasive surgery, and no perforating injuries preceded cyclodestructive therapy. The eye had hemorrhagic secondary glaucoma due to Coats' disease. It was treated three times with contact cyclophotocoagulation after cyclocryotherapy failed to lower intraocular pressure. Subsequently, the intraocular pressure slowly decreased and the eye became phthisical 15 months after the last surgery. Intraocular inflammation developed in the fellow eye and sympathetic ophthalmia w

Corticosteroid are the main treatment in both systemic and ocular sarcoidosis. Most cases of ocular sarcoidosis, such as iridocyclitis, retinal periphlebitis, optic disc inflammation, vitritis and snowballs can be managed with topical or subconjunctival injections of corticosteroids. However, only rarely cases are systemic steroids indicated. High doses of systemic steroids are effective in unusual manifestations of retinal vein occlusions, retinal and optic disc neovascularization and vitreous hemorrhage. Systemic corticosteroids starting with moderately high doses (40-60 mg/day) with slow ta

The clinicopathological data of 1146 enucleated eyes obtained from 1146 patients (485 females and 661 males; mean age 57.4 (SD 21.6) years) between 1980 and 1990 were reviewed. The most common underlying diseases included trauma (37.4%), malignant tumours (19.6%), systemic diseases (diabetes, vascular diseases) (17.1%), surgical diseases (retinal detachment, glaucoma, cataract, corneal dystrophy) (14.1%), infection and inflammation (7%). The most frequent indications for enucleation were secondary angle closure glaucoma (34.9%), ocular malignant tumours (21.7%), atrophia or phthisis bulbi (18.

Children uveitis represents 2-6% of all cases of uveitis in an ophthalmology clinic. Uveitis can be due to a specific ophthalmic disease or to a systemic disease: juvenile chronic arthritis. Sarcoidosis, Behçet's disease, and connective tissue diseases. Visual function, in these young patients, may be compromised by severe complications such as band keratopathy, posterior synechiae, cataract, glaucoma, and retinal oedema. In some cases, delayed diagnosis and severity of the uveitis are due to its insidious onset, and to the absence of any complaint from the child. Therapeutic approach based on

PURPOSE: In this study we wanted to gain experiences with a new black diaphragm IOL concerning surgical postoperative difficulties in traumatic aniridia.
PATIENTS: Since June 1991 we have implanted the IOL prototype and the modifications into 13 eyes with traumatic aniridia. The IOL was transsclerally sutured in 11 eyes without capsular support and it was positioned in front of capsular remnants in two eyes. In eight eyes penetrating keratoplasty was necessary. The mean follow-up period was 17 (1-34) months.
RESULTS: The seventh IOL modification can be implanted safely now. Persistent intrao

A thirty-year-old patient irradiated five years previously on account of a cerebral tumour was admitted for pars plana vitrectomy with the diagnosis of bilateral chronic uveitis and secondary glaucoma. The intraocular inflammation was characterized by corneal precipitates, pseudohypopyon, circulating large cell conglomerates in the anterior chamber and a dense whitish infiltration of the vitreous cavity. Because an intraocular malignant lymphoma was suspected, repeatedly a diagnostic pars plana vitrectomy was performed on the right eye and once on the left eye and bilateral actinotherapy was a

Dislocated lenses of 218 patients (230 eyes) were performed lens extraction and followed up 1-120 months, averaging 12 months. The results were satisfactory in that the visual acuity was improved in 160 eyes (69.6%), including 145 eyes (63.0%) of visual acuity 0.1 or over and 96 eyes (41.7%) 0.3 or over. Among the 70 eyes (30.4%) of poor results, 45 (64.3%) were due to preoperative conditions, and 25 (35.7%) due to surgical complications. It is important that intraocular hemorrhage, vitreous loss, inflammation and secondary glaucoma be guarded against during and after the operation.

Contact cw-Nd:YAG laser transscleral cyclophotocoagulation was performed on 42 patients (44 eyes) with glaucoma who were not responding to medical treatment or had failure of prior surgical procedures. No selection was made as to the type of glaucoma (congenital, primary, secondary due to trauma or inflammation). We applied between 16 and 60 exposures 0.5-2 mm posterior to the limbus. Prior to cyclophotocoagulation the mean intraocular pressure (IOP) was 40.4 mm Hg (+/- 8.0). As a result of the treatment, the mean IOP was lowered to 21.1 mm Hg (+/- 5.2). In eyes with congenital, neovascular, a

The clinical features, treatment, and visual outcome of 52 eyes from 43 patients who developed scleritis following surgery were reviewed. In all patients the scleral inflammation developed adjacent to a surgical wound. Ninety six per cent had necrotising disease and 23% also had evidence of secondary posterior scleritis. Many different types of ocular surgery were implicated and the majority (75%) of the patients had two or more surgical procedures before the onset of the scleritis. Although cataract extraction through a limbal incision resulted in the largest subgroup, scleritis also followed

In this paper ophthalmological and clinical results as well as the literature on lens-induced uveitis (LIU), a relatively unknown pathological syndrome in domestic animals, are presented. Out of all ocular material sent to the Institute of Pathology, GSF, from 1970 until 1990 (n = 864), 40 individual cases of LIU (14 cases in dogs, 13 in cats, 10 in rabbits, 1 in a horse and 2 in birds) were diagnosed. The histology of the eyes of the cases is characterized by a lesion of the lens capsule and a consequent reactive phacogenic inflammation of the anterior segment of the eye in the region of the

Cataract extraction with posterior chamber intraocular lens implantation is rarely complicated by an acute, sterile, anterior segment inflammation. We report three unrelated cases of acute intraocular inflammation following uncomplicated cataract extraction and posterior chamber intraocular lens placement. A constellation of clinical features were found in these cases. The hallmark of this entity was an acute toxic inflammatory reaction occurring in the anterior segment on the first postoperative day. Typically, widespread corneal edema with diffuse corneal endothelial damage occurred, accompa

Results achieved in eyes following a single surgeon's first 200 procedures consisting of intracapsular cataract extraction and implantation of a Choyce-Tennant anterior chamber lens (AC-IOL) (1977 to 1980) are compared with those achieved following the same surgeon's first 200 procedures consisting of extracapsular cataract extraction and implantation of a posterior chamber intraocular lens (PC-IOL) (1980 to 1982). For the AC-IOL eyes, follow up ranged from 11.1 to 14.2 years (mean, 12.0 years); for the PC-IOL eyes, from 9.5 to 10.8 years (mean, 10.0 years). The AC-IOL eyes had many early prob

Topical and systemic steroids have proven to be invaluable agents in the treatment of a wide range of disorders, but their use is not without potential complications. Before initiation of therapy with systemic steroids, a personal or family history of cataracts, glaucoma, hypertension, diabetes, hyperlipidemia, renal stones, peptic ulceration, and current infection or pregnancy should be ascertained, because these patients have an increased risk of complications. Prior to long-term therapy with systemic steroids, blood pressure measurement, tuberculin skin test, and anergy panel are recommende

A case of Enterococcal endophthalmitis developed following an extracapsular cataract extraction. The infection was successfully treated with intravenous and intravitreal ampicillin, but a secondary glaucoma led to a later enucleation. We report a case of postoperative endophthalmitis with an unusual etiology, which did not respond to common treatment.

Glaucoma resulting from secondary congenital, ocular, or systemic conditions represents a formidable diagnostic and management challenge for optometrists. This paper discusses the pathogenesis, diagnosis, and treatment of six common forms of the disease: exfoliation syndrome, pigmentary glaucoma, neovascular glaucoma, lens-induced glaucoma, glaucoma accompanied by ocular inflammation, and trauma-induced glaucoma. For each of these conditions, clinical experience, observational skills, and an understanding of the mechanisms by which elevated intraocular pressure may result are prerequisites for

Six patients with chronic herpes simplex keratouveitis developed a rapidly progressive ocular inflammation unresponsive to corticosteroid and antiviral therapy. Severe secondary glaucoma unresponsive to maximum medical therapy ensued and was treated by cyclocryotherapy in five patients. Ocular ischemia with secondary corneal and scleral calcification subsequently developed in all six patients. All had also received prolonged topical therapy with topical antiviral agents, corticosteroids, beta adrenergic blockers and epinephrine compounds. Three eyes eventually required enucleation for the reli
