The authors reviewed the incidence of hospital-linked postoperative endophthalmitis at the Bascom Palmer Eye Institute between January 1, 1984 and June 30, 1989. After 30,002 intraocular surgical procedures, the following incidence of culture-proven endophthalmitis was observed: (1) extracapsular cataract extraction (ECCE) with or without intraocular lens (IOL) implantation--0.072% (17 of 23,625 cases); (2) pars plana vitrectomy--0.051% (1 of 1974 cases); (3) penetrating keratoplasty (PKP)--0.11% (2 of 1783 cases); (4) secondary IOL--0.30% (3 of 988 cases); and (5) glaucoma filtering surgery--

Primary cysts of the iris usually arise from the pigment epithelial layers and are almost always benign, stationary lesions which require no treatment. Those cysts which are secondary to such agents as trauma or inflammation are, however, characterized by progressive enlargement and visual complications. The major clinical significance of iris cysts lies in their similarity to iris melanomas. Cysts should be monitored for enlargement and/or other complications such as inflammation or secondary glaucoma.

Intraocular inflammation of exogenous origin, which may lead to the loss of visual function in both eyes, i.e. SO confronts ophthalmologists much less rarely than might be expected. Epidemiological studies comprising ergophthalmological aspects underscore this. Questions with regard to therapy should consider the fact that the T cell-mediated cytotoxic disease which causes the transition from the initially unilateral subthreshold exogenous uveitis into the bilateral progressive phase is by no means a rare disorder, especially in secondary surgical operations on predamaged eyes with fresh intra

Fifty children, 2 months to 14 years of age underwent 66 penetrating keratoplasties. Included were 18 eyes with congenital corneal opacification, 16 with corneal decompensation (either from congenital glaucoma, trauma, or multiple surgeries), 10 with corneal hydrops secondary to keratoconus, and 10 with failed grafts. There were 32 clear grafts with a 1- to 10-year follow-up, 30 failed grafts, and 4 lost eyes. Acquired corneal scars, corneal decompensation, older children, and phakic eyes had the best prognosis. Corneal perforations, active inflammation or infection, and infants with multiple

A 57-year-old man presented with proptosis of the right eye and findings consistent with bilateral angle-closure glaucoma. Subsequent evaluation revealed severe bilateral uveal effusions and associated nonrhegmatogenous retinal detachments, which were felt to be related to orbital pseudotumour and associated scleritis. Hematologic studies were consistent with a diagnosis of myelodysplastic syndrome. Although myelodysplastic syndrome has been reported in one patient with orbital inflammation and myositis, to our knowledge ciliochoroidal effusion and secondary angle closure have not previously b

A 67-year-old woman presented with signs of severe intraocular inflammation and secondary glaucoma. The initial diagnosis was uveitis, and an anterior chamber paracentesis with cytological study of the aspirate failed to establish an aetiological diagnosis. After three trabeculectomies had failed to control the intraocular pressure, the blind eye was enucleated. On histopathological examination a mucinous adenocarcinoma was found to cover diffusely the iris surface and to involve the ciliary body and peripheral choroid. The patient subsequently developed evidence of widespread metastatic disea

Ocular symptoms of Behçet's syndrome in 16 patients and the results of cytostatic and immunosuppressive therapy with cyclosporin A (6 patients), chlorambucil (6 patients), and a combination of cyclophosphamide and procarbacin (4 patients) are described. The clinical course and therapeutic outcome were compared to the patients' visual acuity and the duration of the disease prior to institution of cytostatic therapy. Average follow-up was 4.6 years, maximum 9 years. The principal ocular symptom in all patients was hemorrhagic, occlusive periphlebitis. Other symptoms, in descending order of frequ

Due to continued improvement in lens design, lens manufacturing, and surgical technique, the incidence of complications of cataract surgery with intraocular lens (IOL) implantation have decreased in recent years. Complications may be categorized into operative and postoperative, both early and late onset. Operative complications include posterior capsule rupture with or without vitreous loss, loss of all or part of the lens nucleus, iris damage, and stripping of Descemet's membrane. Early postoperative complications include pupillary block, hyphema, elevation of the intraocular pressure associ

Implantation of a retropupillary artificial intraocular lens into the capsule of the lens is from the anatomical aspect the most physiological type of pseudophakia. The authors compare the early results of implantation of two models of retropupillary intraocular lenses, i.e. an intraocular lens made from one piece (PMMA loop) and an intraocular lens with polypropylene loops Intraocular lenses with PMMA loops are more difficult to insert in the capsule, they keep, however, better in position than intraocular lenses with a polypropylene loop. Greater traumatisation of surrounding tissues during

A 67-year-old woman examined 12 months following extracapsular cataract extraction had a massive pseudophakic pigment dispersion associated with diffuse corneal epithelial edema, mild uveitis, and secondary glaucoma. She underwent penetrating keratoplasty following removal of a posterior chamber intraocular lens (IOL), anterior vitrectomy, capsulectomy, and iris biopsy. Histopathologic examination revealed a pigmented iris nevus and signs of iris erosion by the IOL loop. Because 3 months later the eye developed streptococcal endophthalmitis and had to be eviscerated, we had the opportunity to

Posterior scleritis is a granulomatous inflammation occurring either within the scleral tissue or in the vascular networks adjacent to it. This inflammatory process can cause leakage of fluid from the choroid into the suprachoroidal space leading to ciliochoroidal effusion and an exudative retinal detachment. The following is a case report of a 64 year old white male, who presented with an acute angle closure attack and myopic shift in one eye, and later developed an exudative choroidal and retinal detachment. Posterior scleritis was diagnosed based on ultrasonography and the orbital CT scan f

The authors describe a new surgical approach used in six consecutive patients referred to us between August 1986 and August 1988 with massive suprachoroidal hemorrhage (MSCH) during or after cataract extraction (4 patients), glaucoma filtering surgery (1 patient), or scleral buckling (1 patient). All patients had large hemorrhagic choroidal detachments with five eyes showing "kissing" detachments. Secondary surgery was delayed 7 to 25 days (mean, 14 days) to allow liquefaction of the blood clot and reduce intraocular inflammation. All eyes underwent posterior drainage sclerotomies under consta

A 44-year-old man had a blind painful right eye secondary to neovascular glaucoma. Cyclocryotherapy was performed because medical therapy was ineffective in lowering the intraocular pressure or reducing the pain. The eye became comfortable, but 5 months later vision decreased in the left eye. Examination revealed a marked posterior uveitis suggestive of sympathetic ophthalmia. Enucleation of the right eye and corticosteroid therapy were effective in eliminating inflammation in the left eye. Histopathologic examination of the exciting right eye revealed classic signs of sympathetic ophthalmia.

Secondary anterior chamber implantation has become relatively simple since the advent of viscoelastic materials. Still, glaucoma, cystoid macular edema, endophthalmitis, and astigmatism remain vision-threatening complications. We studied intraocular pressures (IOPs) following this surgery in 102 patients (124 eyes) over 6 years. Elevated IOP was noted in 32 eyes (25.8%), but only 14 (11.3%) needed long-term medical treatment. None, however, required laser iridectomy, trabeculoplasty, or trabeculectomy. Patients should be selected for secondary anterior chamber implantation only after more cons

Because of their many advantages, posterior chamber (PC) intraocular lenses (IOLs) were sutured to the posterior iris (in the absence of a lens capsule) in combination with penetrating keratoplasty for aphakic and pseudophakic corneal edema in 53 cases. Secondary PC IOL implantations were performed in aphakic corneal edema (nine cases), and IOLs were exchanged for PC IOLs in pseudophakic corneal edema (44 cases). Ninety-six percent of the grafts remained clear. Forty-three percent of the eyes had 20/40 or better visual acuity, 38% between 20/50 and 20/200, and 19% worse than 20/200, with an av

A series of 17 patients with stromal atrophy, hypoperfusion, and microneovascularisation of the iris investigated in the Glaucoma Investigation and Research Unit are described, and their iris angiograms were compared with those of normal irides of patients in the same age group seen in general clinics. In all but one of the 17 cases this iris atrophy was associated with glaucoma or ocular hypertension, which appeared to be secondary to the iris changes. The condition was bilateral and presented a typical slit-lamp appearance, with subtle evidence of microneovascularisation. There was neither h

Following ocular trauma, it is not uncommon for many patients to develop some form of secondary glaucoma. Studies have indicated a 2-10% incidence of late onset glaucoma following blunt trauma. The trauma can be either penetrating or nonpenetrating in nature. Penetrating injuries can lead to severe intraocular damage and inflammation which may cause anterior synechiae, a pupillary block or a trabeculitis. These processes can ultimately produce a glaucomatous condition. Severe blunt ocular trauma (an ocular contusion) can also lead to a glaucomatous state by producing an angle recession, hyphem

Complications attributable to large conjunctival blebs after glaucoma filtering surgery range from annoying foreign-body sensation to devastating endophthalmitis. We used the argon laser to shrink large complicated blebs in four eyes of four patients who had previously undergone glaucoma filtering surgery. The eye is first anesthetized with a topical agent. The bleb surface is lightly abraded with a sterile cotton swab and then "painted" with a tissue-staining dye. Diffuse laser burns applied over the surface resulted in sufficient shrinkage to allow resolution of the respective symptoms or se

The various forms of secondary glaucoma are discussed on the basis of their different pathogenesis. Elevation of IOP is most frequently associated with inflammation of the uveal tract. Vascular changes in the uvea, in particular rubeosis iridis but also angiomatoses of the retina, can cause secondary glaucoma. Tumors of the uvea and retina often lead to increased intraocular pressure; and lastly, blunt and perforating injuries are frequently followed by glaucoma. Treatment of all these different forms of secondary glaucoma is discussed.

Anterior chamber lenses are currently much less popular than posterior chamber lenses. They are mainly used in conjunction with intracapsular cataract extraction, as a secondary lens implant, and as a back up lens in case of intraoperative complications of extracapsular cataract extraction. A major disadvantage of anterior chamber lenses is improper sizing. Rigid lenses cause the greatest ocular tenderness and present serious problems with errors in sizing but they are the easiest to explant. Semiflexible and flexible lenses show a greater tendency to develop peripheral anterior synechiae. Thi
