Seventy-two keratoplasty eyes obtained surgically from 72 patients were examined histopathologically. The age at the time of enucleation ranged between 2 months and 91 years, with an average of 47.2 years. The brief history of the development of cytopathologic study of ocular fluids has been reviewed. The utilization of cytopathology techniques has become increasingly important with the introduction and widespread use of vitreous surgery. The cytopathologic study of ocular fluids have diagnostic value in infectious and noninfectious ocular inflammatory diseases, neoplastic lesions, some forms of secondary glaucoma, epithelial ingrowth, proliferative vitreoretinopathy, persistent hyperplastic primary vitreous and amyloidosis.

The authors present their experience with the evaluation and follow-up of 62 patients with primary cysts of the iris, discuss their clinical and pathological features, and propose a simple classification for these lesions. This paper describes the clinicopathologic findings in 38 eyes that were surgically enucleated or eviscerated after cataract extraction with implantation of an intraocular lens (IOL). Endophthalmitis and/or complications of wound healing accounted for loss of the eye in 29 cases. Eight of the 38 eyes had complications of IOL insertion that necessitated multiple surgical procedures. These procedures were the removal of an IOL, the secondary insertion of an IOL, and penetrating keratoplasty. Two cases of endophthalmitis were the result of contamination of the solution used to sterilize the lens.

Secondary vascular glaucoma is increased intraocular pressure due to venous obstruction in or outside the eye. Its main aetiological features are lesions of the vortex veins and the anterior ciliary veins and orbital and cavernous sinus obstruction; increase in orbital volume, tenonitis/scleritis posterior and idiopathically elevated episcleral venous pressure are described. A patient had both cataracts removed and a Copeland pseudophakos was implanted in one eye. Both globes were retained at autopsy for histopathologic study. The globe containing the implant did not show evidence of acute or chronic inflammation of the anterior segment, nor did it display signs suggesting secondary glaucoma. Observations based on clinical and histopathologic data from 200 consecutive cases of retrocorneal pigmentation by melanin-containing cells (RCP) and 25 cases of pigmented pupillary membranes (PPM) are reported. The histopathologic and cytopathologic study of ocular fluids obtained for diagnostic purposes is presented. The analysis of fluid specimens with millipore filter, celloidin bag-cell block, and transmission electron microscopy techniques has proved to be effective for establishing and confirming clinical diagnoses. Diagnostic vitrectomy can be readily used to evaluate such conditions as intraocular tumors, lens and blood-induced glaucoma, inflammatory conditions, amyloidosis, persistent hyperplastic primary vitreous (PHPV), epithelial ingrowth, and tractional vitreoretinal membranes.

On the basis of these observations, a classification of iris cyst is proposed. Accordingly, primary iris cysts are divided into epithelial and stromal categories, each having different clinical characteristics. Of 92 enucleated eyes with scleral inflammation and 114 eyes from 81 patients with scleritis, 45 (49%) and 20 (18%), respectively, showed evidence of increased intraocular pressure. Damage to the trabecular meshwork of iridocyclitis, overlying corneoscleral inflammation, or peripheral anterior synechiae was the most common cause of increased intraocular pressure. Other causes included topical corticosteroid use, angle neovascularization, and posterior scleritis with secondary angle closure.

The results of 100 consecutive cases of pars plana vitrectomy are reported. Vitrectomy was performed on accunt of complications of diabetic retinopathy (37 eyes), complicated retinal detachment (28 eyes), vitreous haemorrhage of various causes (17 eyes), vitreous haemorrhage and complications secondary to injuries (13 eyes) and secondary cataract or vitreous in the anterior chamber creating corneal dystrophy (5 eyes). Pupillary block glaucoma developed in 3 children treated with irrigation and aspiration using the Cavitron phacoemulsification system. One had a congenital cataract, 1 had blunt trauma, and 1 had a penetrating injury. Cataract extraction was performed on 734 patients. These were followed up postoperatively for up to 1 year. In 448 cases the extraction was performed in both eyes during 1 operating session (896 eyes) and in 1 eye in 286 cases, a total of 1182 cataract extractions.
