To investigate the effects of postoperative mitomycin C (MMC) eyedrop as an adjunct to intraoperative MMC in refractory glaucomas, 15 eyes of 14 glaucoma patients (age; 47.0 +/- 15.5 years) in need of trabeculectomy were studied. Preoperative diagnoses were secondary glaucoma to uveitis in seven patients, neovascular glaucoma in four, pseudophakic glaucoma in three, and aphakic glaucoma in one. Six patients who had neovascular glaucoma or history of a failed trabeculectomy with intraoperative 0.2 mg/ml MMC received intraoperative 0.4 mg/ml MMC. The others received 0.2 mg/ml MMC for 3 minutes.

This prospective, randomized clinical trial assessed the effects of mitomycin C and cyclosporin A used as antimetabolites in trabeculectomy on the post-operative IOP and success rate. PURPOSE: To evaluate the criteria of diagnosis and management of secondary glaucoma in young nanophthalmic patients.
METHODS: Considering the anatomic features and clinical characteristics, 22 eyes of 11 patients were identified as bilateral nanophthalmos. Intraocular pressures (IOP) were checked with Goldmann applanation or Keeler pulse-air 2000 tonometer, and Nd-Yag laser was used for iridotomies. Prophylactic V-shaped unsutured sclerectomies over the pars plana were combined with trabeculectomy, and Mitomycin C (MMC) in 0.2 mg/ml concentration was applied.

Trabeculectomy is reportedly less effective in aphakic eyes, secondary glaucoma, juvenile patients, and eyes with a history of unsuccessful glaucoma surgery. Repository corticosteroid was injected into the posterior sub-Tenon space in 29 cases (39 eyes) of cystoid macular edema (CME) secondary to uveitis. There were 12 cases of Behçet's disease, 7 cases of sarcoidosis, one case of tuberculous uveitis, and 9 cases of etiology unknown uveitis. In some of the patients injections were repeated 2 to 7 times at intervals of more than 2 weeks. In the management of an ocular injury, traumatic glaucoma is often overlooked. Prospectively reviewing 100 consecutive patients of traumatic glaucoma we found a very high correlation of postconcussional glaucoma with traumatic cataracts, angle recession of more than 180 degrees, significant injuries to the iris and a displacement of the lens.
