The relationship between glaucoma and myopia: the Blue Mountains Eye Study.

OBJECTIVE: To quantify the relationship between myopia and open-angle glaucoma, ocular hypertension (OH), and intraocular pressure (IOP) in a representative older population. DESIGN: Cross-sectional population-based study of 3654 Australians 49 to 97 years of age. METHODS: Subjects with any myopia (> or =-1.0 diopter [D]) were identified by a standardized subjective refraction and categorized into low myopia (> or =-1.0 D to <-3.0 D) or moderate-to-high myopia (> or =-3.0 D). Glaucoma was diagnosed from characteristic visual field loss, combined with optic disc cupping and rim thinning, with

The relationship between glaucoma and pseudoexfoliation: the Blue Mountains Eye Study.

OBJECTIVE: To quantify the relationship between pseudoexfoliation (PXF) and open-angle glaucoma, ocular hypertension, and intraocular pressure (IOP) in a defined older population. METHODS: A cross-sectional study of 3654 people aged 49 to 97 years identified subjects with PXF during slitlamp examination. The IOP was measured by applanation tonometry. Glaucoma was diagnosed from characteristic visual field loss combined with optic disc cupping and rim thinning, without reference to IOP. Ocular hypertension was diagnosed if IOP was greater than 21 mm Hg in either eye, without field and disc cha

Screening for glaucoma in the Chinese elderly population in Singapore.

AIM: To determine the performance of different tests in screening for glaucoma. METHOD: The results from a glaucoma prevalence study of 479 Chinese elderly Singaporeans were analysed. Glaucoma screening tests evaluated in this study included Goldmann applanation tonometry (GAT), non-contact tonometry (NCT), optic disc assessment, screening Humphrey visual field and gonioscopy. The sensitivity, specificity and positive predictive values of the various screening tests in detecting glaucoma were calculated. RESULTS: The sensitivity of GAT and NCT at a specificity of at least 90% was 26% and 30%

Brimonidine. A review of its pharmacological properties and clinical potential in the management of open-angle glaucoma and ocular hypertension.

Brimonidine is a highly selective alpha 2-adrenoceptor agonist which reduces intraocular pressure (IOP) by reducing aqueous humour production and increasing aqueous humour outflow via the uveoscleral pathway. Brimonidine is indicated for the topical management of open-angle glaucoma or ocular hypertension. In 3 large comparative studies in patients with open-angle glaucoma or ocular hypertension, the ocular hypotensive efficacy of brimonidine was maintained during treatment periods of up to 1 year. Mean reductions in peak (measured 2 hours after the morning dose) and trough (measured 12 hours

Reduced intraocular pressure and increased ocular perfusion pressure in normal tension glaucoma: a review of short-term studies with three dose regimens of latanoprost treatment.

Currently used ocular hypotensive agents do not effectively lower intraocular pressure (IOP) in some normal-tension glaucoma (NTG) patients. The prostaglandin F2 alpha analogue, latanoprost, has been shown to reduce IOP in normal subjects and ocular hypertensive glaucoma patients by increasing uveoscleral outflow. This mechanism is expected to be particularly effective in the lower IOP range that is typical of NTG. To date, three dose regimens of latanoprost have been shown to reduce IOP significantly in NTG. The IOP reductions of 14.2% and 15% obtained with twice-daily application of 0.0015%

Circadian intraocular pressure management with latanoprost: diurnal and nocturnal intraocular pressure reduction and increased uveoscleral outflow.

Based on their mechanism of action, the most frequently used ocular hypertensive agents, the beta-blockers, cannot be assumed to reduce IOP during sleep. The need for drugs that reduce IOP around-the-clock is underscored, however, by the fact that inadequate nocturnal ocular perfusion pressure is considered to be one of the likely causes of glaucomatous optic neuropathy especially in some cases of normal tension glaucoma. The studies reviewed here demonstrate that latanoprost, a new ocular hypotensive prostaglandin F2 alpha analogue, applied once a day at a concentration of 0.005%, maintains a

Latanoprost as a new horizon in the medical management of glaucoma.

Latanoprost is a new prostaglandin F2 alpha analogue specifically developed for the treatment of glaucoma. Latanoprost is a selective FP receptor agonist, with a primary mode of action of increased uveoscleral outflow of aqueous humor. A dose of 50 micrograms/mL (0.005%) once daily has been found optimal in clinical trials. Latanoprost reduces the nocturnal intraocular pressure in addition to the diurnal, and has been shown to be additive to other glaucoma medication. In long-term phase III clinical trials, latanoprost 0.005% once daily has been proven to be at least as effective as timolol 0.

Adverse pulmonary effects of topical Beta blockers used in the treatment of glaucoma.

We conducted a case-control study to measure the risk of pulmonary side effects following the use of topical glaucoma medications, particularly the beta blockers, and to identify patients at highest risk for such side effects. Patients over age 55 who were active users of the New Jersey Medicaid system during the years 1981-1989 composed the study population. Two case groups were identified: (a) 21,096 patients who began new use of bronchodilator medications and (b) 3,382 patients who were users of a xanthine bronchodilator or inhaled steroid whose regimen was intensified through addition of a

[Treatment of glaucoma chronicum simplex with a combination of 0.5 percent timolol with 0.5 percent adrenaline plus 0.3 percent guanethidine].

The effect on the intraocular pressure of patients with primary open-angle glaucoma of a combination of timolol 0.5% with epinephrine 0.5% plus guanethidine 3% was studied and compared with the effects of timolol alone, of epinephrine 0.5% plus guanethidine 3%, and of other glaucoma drugs. The combination of timolol 0.5% with epinephrine 0.5% plus guanethidine 3% had a stronger effect on intraocular pressure (reduction 10.9 +/- 1.1 mm Hg) than epinephrine 0.5% plus guanethidine 3% (7.6 +/- 0.9 mm Hg) or timolol alone (5.8 +/- 1.1 mm Hg) or other glaucoma drugs. Molteno implant for control of glaucoma in eyes after penetrating keratoplasty.

Seventeen patients (17 eyes) underwent implantation of a single plate Molteno implant for medically uncontrollable intraocular pressures after penetrating keratoplasty. Most of the eyes had extensive peripheral anterior synechiae, and 16 of 17 (94%) were pseudophakic or aphakic following keratoplasty. Other glaucoma procedures had been performed previously on 13 eyes: argon laser trabeculoplasty (one eye), trabeculectomy (seven eyes), transpupillary argon laser cyclophotocoagulation (three eyes), and cyclocryotherapy (three eyes). Follow-up ranged from 5 to 28 months (mean, 13 months). Three e

Light-induced avian glaucoma as an animal model for human primary glaucoma.

Glaucoma can be induced in domestic chicks at the will of the investigator, by the simple device of rearing the chicks under continuous light. This light-induced avian glaucoma (LIAG) is presented as an animal model system for human open-angle glaucoma. A number of morphological and physiological findings in LIAG are reviewed, and the LIAG system is compared with several other glaucoma model systems, in dogs, rabbits and monkeys. Intraocular pressure in LIAG has been demonstrated to be responsive to several anti-glaucoma drugs, and the system could be used for further drug testing. Thus it is

Glaucoma filtering surgery, a retrospective study of 300 operations.

The results of 300 consecutive glaucoma filtering procedures, which were performed in our clinic between January 1982 and September 1985, are described. The overall combined success rate (complete + qualified) was 66.5% (Heuer's classification). This is in good agreement with other glaucoma referral centers. The peak of the age distribution curve at the time of operation was between 70 and 75 years. Nevertheless, 30% of operations was performed on patients under 50 years of age. Relatively poor success rates were obtained in operations following prior unsuccessful filtering surgery (50.5%) or

Timolol treatment of chronic open-angle glaucoma and ocular hypertension. A 2.5-year multicenter study.

Maintenance effect of topical timolol was investigated for 2 years in a group of 125 glaucomatous and ocular hypertensive patients (231 eyes) who had been successfully treated with timolol alone during a 6-month period preceding this trial. Intraocular pressure (IOP) was controlled with timolol alone in 135 of 183 eyes (74%) that completed the study. At the end of the trial 142 eyes (78%) showed an IOP of less than 22 mmHg. Other glaucoma medication had to be added to timolol treatment in 18% of ocular hypertensive and 35% of glaucomatous eyes because of IOP elevation. Elevation of IOP seemed

[Timolol in combination with other glaucoma drugs (author's transl)].

In 39 selected patients with open-angle glaucoma, in whom pressure regulation with Timolol alone was inadequate, success was achieved by using this drug in combination with various other glaucoma drugs over treatment periods of 12 to 18 months. In all cases, Timolol had had a significant, thought inadequate pressure-lowering effect; only by adding either 0.5%-2% pilocarpine. 0.75-2.25% carbachol, 2% aceclidine, 0.1% epinephrine or 250 mg of acetazolamide could IOP be lowered effectively to an acceptable level.

Long-term drift and continued efficacy after multiyear timolol therapy.

The clinical courses of 41 patients who received timolol maleate for experimental protocols were reviewed. We reevaluated the conditions of 17 patients with the elevated pressures of primary open angle glaucoma who had received timolol alone or in combination with other glaucoma medications for a maximum of 35 months (average, 26 months). Withdrawal of timolol led to an average intraocular pressure rise of 5 mm Hg. Eight patients treated with timolol alone showed a progressive, significant, 3-mm Hg upward rise, or "long-term drift" in IOP during the follow-up period. Pressure rise after timolo

Cyclodialysis with air injection in black patients.

Thirty-two cases are presented in which cyclodialysis with air injections was used to treat glaucoma in black patients. Nineteen of the 32 cases were of aphakic glaucoma and most represented failures of other glaucoma surgery. The success rate was found at 12 months postoperative follow-up to be 75%. Three of 13 eyes with phakic glaucoma developed cataracts. Cyclodialysis may be superior to trabeculectomy for surgical treatment of glaucoma in black patients.

Timolol and pediatric glaucomas.

Thirty-eight eyes were treated by adding timolol to the medical regimen. After a suitable trial, attempts were made to reduce other glaucoma medications. Fifteen eyes with infantile glaucoma treated surgically at birth, experienced elevated intraocular pressure later in life. Another 15 eyes had glaucoma associated with congenital anomalies such as aniridia, Sturge-Weber syndrome, and mesodermal malformations. The group with infantile glaucoma demonstrated an average drop in pressure of 24% and 22% after one and three months, respectively. Six of the 15 eyes were controlled at 22 mm Hg or less
