The wandering lens: Clear Lens in the anterior chamber following blunt trauma.

Ocular trauma is a common cause of visual morbidity and can lead to various anterior and posterior segment complications. Traumatic dislocation of the crystalline lens in toto into the anterior chamber without open globe injury is a rare entity. We report a 27-year-old male who presented after blunt trauma from a wooden door with sudden visual loss in the right eye. Best-corrected visual acuity was counting fingers at 1 metre. Slit-lamp examination revealed a clear crystalline lens completely dislocated into the anterior chamber with a clear cornea, well-formed anterior chamber, round pupil an

Hunting Firearm Induced Optic Nerve Transection.

Penetrating ocular trauma caused by firearms represents a severe ophthalmic emergency and is frequently associated with poor visual outcomes. We report the case of a 41-year-old man who presented with complete loss of visual acuity (no light perception) after a penetrating injury to the right eye sustained during a hunting accident involving a low-velocity firearm. Initial ophthalmic examination showed a full-thickness nasal scleral perforation with vitreous prolapse, mild corneal oedema, and limited fundus visualization due to dense vitreous haemorrhage. A relative afferent pupillary defect o

Acute angle closure in a pseudophakic eye with pseudoexfoliative syndrome: management when conventional medical and laser therapies fail.

Pseudoexfoliation syndrome is a common cause of secondary open-angle glaucoma but rarely presents with acute angle closure in pseudophakic eyes, where management may be challenging when conventional medical and laser therapies fail. We report the case of an 82-year-old pseudophakic woman with known pseudoexfoliation syndrome who presented with acute right eye pain, visual loss, an intraocular pressure of 65 mmHg, a shallow anterior chamber and vitreous prolapse into the anterior chamber. Medical therapy, anterior vitreous disruption and laser peripheral iridotomy failed to adequately reduce in

Acute Angle-Closure Glaucoma Secondary to Vitreous Prolapse Following Ocular Trauma.

Secondary acute angle-closure glaucoma (AACG) caused by vitreous prolapse in a phakic patient is exceedingly rare. We report a case of AACG secondary to vitreous prolapse, occurring 2 weeks after orbital trauma in a 34-year-old male healthcare worker. The patient presented with acute left eye pain, nausea, and vision loss after performing cardiopulmonary resuscitation (CPR) at his workplace. Two weeks earlier, he sustained a nonoperative orbital floor fracture and commotio retinae from a motor vehicle accident. Examination revealed decreased vision and an intraocular pressure (IOP) of 59 mmHg.

Bilateral Asymmetrical Presentation of Dead Bag Syndrome: A Case Report.

BACKGROUND: Delayed complications after uneventful cataract surgeries may arise, including IOL (Intraocular Lens) dislocation. Among the rarest etiologies is Dead Bag Syndrome (DBS), a condition defined by the presence of a clear, transparent capsular bag accompanied by spontaneous posterior capsular rupture (PCR) occurring many years after cataract surgery. This late-onset PCR represents the most distinctive clinical feature of the syndrome. CASE PRESENTATION: We report a case of healthy 69-year-old male presented to our Emergency Room (ER) complaining of sudden onset painless decreased visi

Suprachoroidal injection of a dexamethasone implant in a case of secondary macular oedema.

BackgroundNon-infectious uveitis (NIU) is a common ocular inflammation that frequently leads to Macular oedema (MO). MO can cause irreversible tissue damage and permanent visual impairment. Dexamethasone implant is approved for intravitreal injection (IV) in the treatment of MO. Despite its proven efficacy, IV poses risks of complications. Recently, suprachoroidal injection has emerged as a novel strategy for targeted drug delivery to the posterior segment of the eye, reducing the risk of complications.Case presentationA 70-year-old woman with no history of diabetes underwent left eye cataract

Intraocular lens subluxation obstructing a glaucoma tube shunt after a Valsalva maneuver: a case report.

BACKGROUND: Late subluxation of an intraocular lens (IOL) within the capsular bag is a rare complication of cataract surgery, primarily associated with progressive zonular weakening in conditions such as pseudoexfoliation syndrome, high myopia, prior vitreoretinal surgery, connective tissue disorders, or ocular trauma. While IOL-capsular bag subluxation is well-documented, its impact on glaucoma drainage devices remains poorly understood. We present a rare case of Valsalva-induced IOL subluxation obstructing an Ahmed glaucoma valve, resulting in acute intraocular pressure (IOP) elevation in a

Traumatic Glaucoma Managed With Ab Interno Trabeculotomy (Trabectome®): A Case Report.

A 55-year-old patient sustained a bungee cord injury to his left eye, resulting in a traumatic cataract, vitreous prolapse into the anterior chamber, and extensive angle recession. Despite cataract surgery with pars plana vitrectomy and maximal medical therapy, intraocular pressure (IOP) remained elevated at 40 mmHg. The patient was diagnosed with mixed-mechanism traumatic glaucoma. Ab interno trabeculotomy with the Trabectome® system (MicroSurgical Technology, Redmond, WA, USA) successfully lowered IOP over 12 months, significantly reducing the medication burden. To the best of our knowledge,

Epithelial ingrowth in descemet membrane endothelial keratoplasty associated with vitreous loss.

BACKGROUND: Epithelial ingrowth is a rare but potentially sight-threatening complication caused by the invasion of corneal or conjunctival epithelial cells into the eye during ocular surgeries. DMEK is emerging as a widely used surgery for endothelial keratoplasty with its improved safety profile. We describe a case of epithelial ingrowth in the graft-host interface after uneventful DMEK associated with vitreous prolapse in the anterior chamber. CASE PRESENTATION: An 81-year-old female with Fuchs endothelial dystrophy underwent DMEK for corneal decompensation following cataract surgery. Durin
