Intraocular lens scaffold to facilitate intraocular lens exchange.

UNLABELLED: We describe a technique of intraocular lens (IOL) exchange in patients with post-refractive surgery IOL power error or patients who are dissatisfied with the optical performance of the IOL. The technique involves the presence of 2 IOLs in the eye: the offending IOL, which is manipulated out of the capsular bag into the anterior chamber, and the corrective IOL, which is inserted into the bag. The anteriorly elevated IOL is transected with the IOL cutting scissors, while the corrective IOL acts as a scaffold for the posterior capsule. The technique provides continuous distension of t

Securing the anterior chamber in penetrating keratoplasty: an innovative surgical technique.

PURPOSE: Conventional penetrating keratoplasty (PK) is an open-sky procedure, carrying a worrisome risk of expulsive suprachoroidal hemorrhage and vitreous prolapse in high-risk cases. The authors propose an innovative surgical technique that secures the intraoperative anterior chamber and should reduce the risk of vitreous complications. METHODS: In the new stabilized procedure, a deep anterior corneal lamella is excised. A small puncture is made in each quadrant along the groove of the stromal bed, and the anterior chamber is maintained by viscoelastic injection. The viscoelastic-coated gra

Traumatic optic nerve avulsion: A case report.

INTRODUCTION: Case report of a traumatic optic nerve avulsion. OBSERVATION: We report the case of a traumatic right optic nerve avulsion in an 11-year-old boy as a result of a contusion with a surfboard. On initial examination, the patient exhibited bilateral mydriasis with a right afferent pupillary defect. Visual acuity was no light perception. A moderate microhyphema was noted along with intraocular pressure of 12mmHg and no open globe. Fundus examination revealed retinal ischemia with white retinal edema, attenuated arteries and segmentally occluded vasculature. In place of the optic nerv

Pseudo anterior capsule barrier for the management of posterior capsule rupture.

UNLABELLED: A technique that uses an implanted intraocular lens (IOL) to create a barrier for the management of posterior capsule rupture is described. When a rupture occurs, surgery is halted and a dispersive ophthalmic viscosurgical device (OVD) injected into the anterior chamber to prevent vitreous prolapse. The remaining nucleus is maneuvered into the anterior chamber away from the pupillary space. The posterior capsule tear is converted into a continuous curvilinear capsulorhexis where possible. Dissociated anterior vitrectomy is performed as indicated, keeping the large nuclear fragments

Vitreous asteroid hyalosis prolapse into the anterior chamber simulating iris metastasis.

Two asymptomatic elderly women who underwent cataract extraction 7 or more years previously and with intraocular lens placement presented with a linear bead-like white multinodular mass in the inferior angle simulating iris metastasis versus large inflammatory precipitates. There was no iris infiltration. In the first case, the posterior lens capsule was intact and there was no evidence of gelatinous vitreous in the anterior chamber, whereas in the second case, the capsule was open and there was gelatinous vitreous prolapse. In both cases, there was asteroid hyalosis in the vitreous. Both pati

Traumatic expulsive iridodialysis with vitreous prolapse.

Traumatic expulsive iridodialysis is a rare complication after trauma by a blunt object in eyes with previous small-incision cataract surgery. The presence of an intact lens capsule with an intraocular lens was thought to protect against damage beyond the posterior capsule. We report a case of dehiscence of the cataract wound and traumatic expulsive iridodialysis that also exhibited vitreous prolapse.
