Indocyanine green-assisted phacoemulsification in cases of complicated or simple advanced cataracts.

BACKGROUND/PURPOSE: During phacoemulsification for advanced cataracts, particularly when complicated by anterior segment abnormalities, capsulorhexis is very difficult and carries a high risk of complications. This study investigated the efficacy and safety of indocyanine green (ICG)-assisted phacoemulsification in complicated or simple advanced cataracts. METHODS: Thirty-two patients (35 eyes) underwent phacoemulsification for complicated advanced cataracts (group 1) or simple advanced (mature/hypermature) cataracts (group 2). Anterior segment abnormalities (corneal opacity, small pupil, or

Descemet stripping with automated endothelial keratoplasty for bullous keratopathies secondary to argon laser iridotomy--preliminary results and usefulness of double-glide donor insertion technique.

PURPOSE: Manipulation of the endothelial donor lamella in the anterior chamber during Descemet stripping with automated endothelial keratoplasty (DSAEK) is often difficult, especially in cases with bullous keratopathies (BK) secondary to argon laser iridotomy (ALI). We developed a novel nonfolding donor insertion technique using a Busin glide and an intraocular lens (IOL) sheet glide and evaluated post-DSAEK outcomes for ALI-induced BK using several insertion techniques. METHODS: Fourteen patients (mean age, 74.2 +/- 4.9 years) with ALI-induced BK underwent DSAEK; simultaneous cataract surger

Double Descemet's membranes after penetrating keratoplasty with anterior segment optical coherence tomography.

An 83-year-old man underwent penetrating keratoplasty for pseudophakic bullous keratopathy in the left eye. Postoperatively, a thin transparent membrane in the anterior chamber was noted. The differential diagnosis included vitreous prolapse, retained capsule, fibrinous anterior chamber membrane, and retained Descemet's membrane in the anterior chamber, but the diagnosis was uncertain due to corneal edema and Descemet's membrane folds. Anterior segment optical coherence tomography was used to determine the diagnosis of retained host Descemet's membrane. This case report demonstrates that anter

Late onset of acute angle-closure glaucoma following neodymium:YAG laser posterior capsulotomy.

Acute angle-closure glaucoma due to vitreous prolapse immediately following neodymium:YAG (Nd:YAG) laser posterior capsulotomy in eyes with posterior chamber intraocular lens implants is rare. Previously described cases were associated with advanced age, excessive amounts of laser energy, and a large-diameter capsular opening. The authors describe a case of acute angle-closure glaucoma occurring 21 days after Nd:YAG laser posterior capsulotomy in a young patient with a small-diameter capsule opening and a small amount of laser energy to stress the importance of close extended follow-up even in

Trypan blue to aid visualization of the vitreous during anterior segment surgery.

We describe the usefulness of intracameral injection of trypan blue to highlight vitreous in the anterior chamber. The technique was used in 3 eyes of 3 patients who had vitreous prolapse in the anterior chamber. Trypan blue 0.06% was injected in the anterior chamber under air to visualize and resect the prolapsed vitreous. Trypan blue stained the clear vitreous and improved visualization for a safe and atraumatic vitrectomy. No complications associated with the dye were found.
