Management of vitreous loss and dropped nucleus during cataract surgery.

The intraoperative management of complication during cataract surgery of vitreous loss and dropped nucleus strongly influences the outcome, and a well-rehearsed plan for both staff and surgeons is needed. Preventative measures, early recognition, damage control, and resolution are discussed in this article. Maintenance of appropriate pressure relationships and tissue planes limits vitreous prolapse and the surgeon's primary goal is to avoid vitreous traction resulting in retinal tears and detachment. A pars plana approach to anterior vitrectomy with staining of the prolapsed vitreous is the mo

Vitreous prolapse and IOL dislocation during intravitreal injection of triamcinolone acetonide.

BACKGROUND: To report on procedure-related anterior segment complications during intravitreal injections. METHODS: In a prospective interventional case series, 614 eyes received a total of 723 intravitreal injections of about 20 mg triamcinolone acetonide (in 0.2 ml) after paracentesis and aqueous humor drainage for various indications. RESULTS: In three eyes (0.49% of all eyes) a vitreous prolapse occurred during the injection. In one eye, the vitreous prolapse was combined with dislocation of the intraocular lens (IOL). All three eyes were pseudophakic, showing an posterior capsule defect,

Acute angle-closure glaucoma caused by vitreous prolapse after neodymium:YAG posterior capsulotomy.

We report the second known case of acute angle-closure glaucoma from vitreous prolapse after neodymium:YAG (Nd:YAG) laser posterior capsulotomy in a patient with a posterior chamber intraocular lens (IOL). This is the first reported case to result in corneal decompensation that required penetrating keratoplasty. The rare complication of vitreous prolapse was unusual in this patient because of the presence of a posterior chamber IOL, a small 4.0 mm capsulotomy opening, and low (26 mJ) Nd:YAG laser energy.

[The cause and management of intraocular lens dislocation into anterior vitreous cavity].

OBJECTIVE: To study the causes and management of the intraocular lens (IOL) dislocation into anterior vitreous cavity. METHODS: The author retrospectively analyzed the causes of the IOL dislocation treated in the past 10 years. Twenty-eight dislocated IOLs were replaced after anterior vitrectomy. RESULTS: The causes of the IOL dislocation were: (1) IOL vertically sank down into the vitreous cavity through the broken inferior zonules. (2) The superior haptics were not placed in the ciliary sulcus and overturned down into vitreous cavity because of the vitreous prolapse closing the ciliary sul

Small-gauge, sutureless pars plana vitrectomy to manage vitreous loss during phacoemulsification.

This technique manages vitreous prolapse associated with posterior capsule rupture during phacoemulsification by performing small-gauge pars plana vitrectomy with a sutureless, self-sealing incision performed in a closed chamber, maintaining normal intraocular pressure. A high-speed cutter exerts minimal traction on the vitreous. Accessibility to the vitreous through the pars plana is better, ensuring more complete removal of the vitreous and restoration of normal anatomy.

[Eye contusions caused by a bottle cap. A retrospective study based on the Erlangen Ocular Contusion Register (EOCR)].

BACKGROUND: Perforating ocular injuries due to exploding beverage bottles are well known. However, bottle caps alone may also induce severe ocular damage due to ocular contusions while opening carbon dioxide-containing beverage bottles. We studied this type of ocular contusion. PATIENTS AND METHODS: Retrospectively, we examined all findings, operations, and tension profiles of 400 consecutive patients with ocular contusion who had been hospitalized in the Department of Ophthalmology, University Erlangen-Nürnberg, using the Erlangen ocular contusion registry (EOCR). RESULTS: Seven patients we

Lens-induced astigmatism after perforating scleral injury.

Within 6 weeks of a penetrating scleral injury that included vitreous prolapse, a 6-year-old boy developed lenticular astigmatism with a regular component of 5.5 diopters (D). Visible indentational folds in the posterior lens capsule, caused by anterior vitreous fibers and anterior hyaloid, were presumed to be the origin of the astigmatism. Because of decreased visual acuity and the suspicion of early amblyopia, a pars plana vitrectomy with removal of the anterior hyaloid and the critical anterior vitreous fibers was performed. Dense fibrotic tissue between the lens equator and the site of the

Posterior chamber intraocular lens supported by an intact vitreous face.

We describe a technique of posterior chamber intraocular lens (IOL) implantation in eyes with inadequate capsule support caused by inadvertent or planned intracapsular cataract extraction (ICCE) or in eyes having secondary IOL implantation after previous ICCE. The procedure is only performed in eyes with an intact anterior vitreous face, no vitreous prolapse into the anterior chamber, and no vitreous loss. The anterior vitreous is pushed back by viscoelastic material or air. The viscoelastic material is injected under the iris to create a free space between the iris and anterior vitreous. A si

Pupil block glaucoma from traumatic vitreous prolapse in a patient with posterior chamber lens implantation.

BACKGROUND: Angle closure secondary to pupil block is an entity known to occur in aphakic and pseudophakic patients. In aphakic patients, typically the cause of the pupil block is vitreous prolapse (aphakic pupil block). In pseudophakic patients, the typical cause of the pupil block is an anterior chamber lens implant, often in the absence of an iridectomy (pseudophakic pupil block). Rarely, a hybrid of these two conditions can occur, in which vitreous prolapse causes a pupil block in a pseudophakic patient with a posterior chamber lens implant. METHODS: The case presented is that of an elder

[Closed anterior manual vitrectomy as a method of treatment in vitreal complications of intraocular lenses].

Fifty-five closed anterior manual vitrectomies were carried out in patients with vitreal complications after cataract extraction with implantation of posterior-chamber intraocular lenses. The results indicate high efficiency of anterior vitrectomy as a method for surgical treatment of sluggish uveitis, keratopathy, macular edema, secondary glaucoma, developing as a result of vitreous prolapse during implantation of the posterior-chamber intraocular lens. The operations were performed through transscleral and corneolimbic accesses. No cases of intraocular lens dislocation occurred.
