[New modified method of phacoemulsification in lenticular subluxation].

A new method for cataract removal by phacoemulsification in impaired lenticular joints consists in fixation during surgery of the anterior lenticular chamber along the edge of the capsulorrhexis by 4 retractor hooks and subsequent implantation of a soft intraocular lens. The operation was performed in 6 patients with cataracts of different types complicated by subluxation of the lens. The operation did not involve such complications as vitreous prolapse or displacement of the incompletely dislocated lens. All operations were finished with complete evacuation of lenticular mass and implantation

[Efficiency of removing a transparent lens in high myopia].

Results of removal of a transparent lens in high myopia are analyzed. The operation was performed in two stages according to V. S. Belyaev and T. S. Ilyina. The second stage of the operation (lens extraction) was performed by the extracapsular method, which prevented vitreous prolapse into the anterior chamber in all cases and ruled out the possibility of detachment of the posterior plate of the vitreous and retina. Simple technique of the operation, good remote results, no need in expensive instruments or equipment recommend this operation for any ophthalmological department performing extrac

Suction posterior capsulorhexis.

A technique for creating a posterior capsulorhexis during phacoemulsification is presented. It can be used in cases with posterior capsule tears or opacities. The free edge of the capsule is grasped with suction using a 2 mL syringe and a 27 gauge Rycroft cannula introduced via the paracentesis. The edge is then manipulated to produce a continuous curvilinear opening in the posterior capsule. The combination of a closed eye plus the use of a viscoelastic agent in the anterior chamber and capsular bag minimizes the possibility of vitreous prolapse during the maneuver. Occlusion of the cannula t

Repair of scleral rupture and total retinal detachment in a self-injuring psychotic patient under local anesthesia: a case report.

PURPOSE: Ocular self-injury by psychotic patients is an uncommon cause of ocular morbidity that poses peculiar problems. This report describes a schizophrenic self-injuring patient with scleral rupture and retinal detachment (RD), treated under loco-regional anesthesia. CASE REPORT: A 65-year-old man presented with scleral rupture, hyphema, traumatic lens luxation, vitreous prolapse, vitreous hemorrhage and total RD after deliberately hitting his head. The fellow eye had been successfully operated with scleral buckle surgery for the same reason two years earlier. The patient underwent a two-s

[Risk factors of pseudophakic detachment].

UNLABELLED: The increase of pseudophakic retinal detachments has raised the question about its risk factors. According to the literature, complicated cataract surgery rarely contributes to pseudophakic retinal detachment. Nevertheless, we noted a high proportion of defects in the capsular barrier among our patients. We therefore reevaluated our charts for perioperative and epidemiologic risk factors for pseudophakic retinal detachment. PATIENTS: Seventy-six patients with pseudophakic retinal detachment after extracapsular cataract operation with intraocular lens implantation were referred to

[Vitreous prolapse during cataract surgery].

Clinical data of 427 cases (458 eyes) of extracapsular cataract extraction and IOL implantation gave the impression that vitreous prolapse due to disruption of the zonula and the posterior capsule was a malpractical complication. Forcible capsulotomy too large or deep and uneven irrigating pressure were the common causes of zonular disruption. Obstructed passage and insufficient separation of the nucleus from the posterior cortical bed leading to difficult nuclear delivery, aspiration of cortex in the posterior chamber, poorly filled anterior chamber, insufficient room for the IOL, improper in

[Surgical aspiration of secondary cataract. Success quotas and complications].

Cataracta secundaria ("Elschnig pearls") following extracapsular cataract extraction with IOL implantation can be treated by Neodymium-YAG laser capsulotomy or by surgical peeling. The major benefit of the peeling technique is the preservation of an intact posterior capsule. We retrospectively reviewed the peeling procedures (n = 224, mean age: 67 years) performed in our hospital from 1987 to 1990. The results were as follows: (1) In almost all cases (214/224) visual acuity was improved by the operation (mean improvement 0.16-->0.54); (2) major complications were: rupture of the posterior caps

[Results of pseudophakic retinal detachment surgery].

Between 8/81 and 9/89 2658 patients underwent retinal detachment surgery at the university eye hospital Erlangen/Nürnberg. From this number there were 30 (= 1.1%) pseudophakic retinal detachments consisting of 14 males and 16 females. Minimal age was 48, maximal 89 years, average age was 70.9 years. There were 4479 extracapsular cataract extractions with implantation of a posterior chamber lens performed in the same interval at our eye hospital. Out of this 4479 extracapsular cataract extractions we found 9 patients (= 0.2%) with pseudophakic retinal detachment. In our investigated group we fo

A prospective study of angiographic cystoid macular edema one year after Nd: YAG posterior capsulotomy.

We prospectively studied 120 eyes with intact opacified posterior capsules in 120 consecutive patients to determine the incidence of new late-onset angiographic cystoid macular edema (CME) after neodynium: yttrium aluminum garnet (Nd:YAG) laser posterior capsulotomy. Fifty-seven of 120 patients (48%) had both precapsulotomy and one-year post-capsulotomy angiograms done. Of these 57 patients, three eyes had angiographic CME before capsulotomy and two of these three cases had persistent CME one year after capsulotomy. In the other 54 patients, three new late-onset cases occurred (5.6%). Because

The torn posterior capsule: its intraoperative behavior, surgical management, and long-term consequences.

A retrospective study of cataract surgery performed between January 1983 and December 31, 1989, disclosed 48 eyes in which the posterior capsule was torn. The overall incidence of this complication was approximately 1% but was further reduced when the technique of capsulorhexis was mastered. Our review of the video tapes of these surgical procedures revealed that the tear occurred most frequently during nucleus removal (41%) and posterior capsular vacuuming (28%). The anatomy of each tear was classified and related to the event responsible for the torn capsule, as well as to the likelihood of
