[Pseudophakic retinal detachment is frequently a sequela of surgical complications].

In a retrospective study we analyzed the records of 18 patients with pseudophakic retinal detachment. Cataract operation had been performed by extracapsular (n = 14) and intracapsular procedures (n = 6). In 14 of 18 cases, intra- or postoperative complications preceded the retinal detachment: capsular rupture with vitreous loss and anterior vitrectomy during cataract operation (n = 5), vitreous wick (n = 5), and explantation of the intraocular lens because of chronic intraocular inflammation (n = 3). At the time of admittance for retinal surgery, 14 patients (78%) had a detached macula. In 8 c

Peeling technique for cataracta secundaria associated with posterior chamber lenses.

A new method for removing a secondary cataract with a posterior chamber lens in situ was developed using a specially designed irrigation/aspiration handpiece. In two cases, a focal rupture of the posterior capsule without vitreous prolapse occurred. There were no other serious complications observed in 367 patients who were treated.

Axial myopia increases the risk of retinal complications after neodymium-YAG laser posterior capsulotomy.

We conducted a retrospective analysis of Q-switched neodymium-YAG laser capsulotomies performed in 122 eyes between April 1984 and June 1987. Retinal complications occurred in 3 (2.5%) of 121 eyes followed up for 1 year and in 2 (3.6%) of 55 eyes followed up for 2 years. Four eyes developed rhegmatogenous retinal detachments and 1 developed an acute symptomatic retinal tear. No patients developed clinical cystoid macular edema (visual acuity 20/30 or worse). These retinal complications were significantly correlated with axial myopia, preexisting vitreoretinal disease, male gender, younger age,

Traumatic wound dehiscence in pseudophakia.

In a series of 11 pseudophakic patients (11 eyes) who sustained wound dehiscence as a result of blunt trauma, five intraocular lenses were dislocated, one was fractured, and two were expulsed. Wound rupture occurred from three days to one year after surgery; the most common cause of injury was a fall. Iris prolapse was observed in all patients, with hyphema observed in seven. Vitreous prolapse occurred in four patients, and three patients developed retinal detachment. Six of the 11 patients had a visual acuity of 20/40 or better after repair. The limbal wound of a pseudophakic eye can dehisce

Vitreoretinal changes associated with rise in intraocular pressure after Nd:YAG capsulotomy.

In a chart review of 100 consecutive Nd:YAG laser capsulotomies, six presumed risk factors of the rise in intraocular pressure (IOP) after capsulotomy were identified. A group of 15 patients in which the pressure rose to at least 10 mmHg above baseline postoperatively showed a significant (P less than or equal to 0.01) association with myopia, vitreoretinal disease, and vitreous prolapse into the anterior chamber. The previously known association of high pressures with glaucoma, a large capsular opening, or absence of a posterior chamber lens were also confirmed. There was no correlation betwe

Pseudophakic keratoplasty: posterior chamber lens implantation in the presence of ruptured capsule.

A technique is described for implantation of a posterior chamber intraocular lens during keratoplasty for bullous keratopathy when a large capsulotomy and vitreous prolapse are present. Seven cases of pseudophakic or aphakic bullous keratopathy with anterior chamber, iris-fixated, or no intraocular lens in place are reported. A completely intact posterior capsule is not necessary to support safely a sulcus-fixated posterior chamber lens. Anterior segment reconstruction, careful removal of existing intraocular lenses (except a posterior chamber lens), vitrectomy through the ruptured posterior c

A comparison of intracapsular and extracapsular lens extraction combined with keratoplasty.

The clinical outcome and final visual acuities of 29 patients with intracapsular lens extraction and keratoplasty to 38 patients having extracapsular cataract extraction and keratoplasty were compared. Grafts remained clear in 89% of the intracapsular group and 82% of the extracapsular group over an average follow-up of 38 months and 21 months, respectively. Ninety-one percent of eyes with intracapsular lens extraction and 70% of eyes in the extracapsular group achieved a final visual acuity of 6/21 (20/70) or better. We found a significantly higher incidence of additional surgery in the extra

[Stress on the eyeball caused by mechanical vibration. Studies of patient transport and experimental models].

The acceleration forces which occur in the eye and in the head region in transport of patients were investigated. It was to be established whether the finding often deteriorates appreciably in perforating eye injuries between the accident site and the ophthalmological operating theater. The investigation showed that smaller acceleration forces become active in the eye in the sitting position than in the lying position in patient transport. In transport in the hospital area, e.g., in the patient lift and on a stretcher, the maximum acceleration amplitudes which occur are greater than those occu

[Indications and techniques for removal of subluxated lenses (author's transl)].

Extraction only appears indicated in cases of anteriour luxation and phakolytic glaucoma. Visual disturbances due to a dislocated lens should be considered relative indications for removal, since functional improvement cannot be expected as a rule. Expression of the lens through a corneoscleral incision is the method of choice in cases of anterior luxation (Fig. 1) as well as in subluxations without vitreous prolapse (Fig. 2) Open-sky vitrectomy allows for clearing of the route of extraction in cases of subluxation with vitreous in the anterior chamber (Fig. 3). [Secondary cataract in aphakic children (author's transl)].

Extracapsular extraction of infantile cataract often leads to the development of secondary cataract. The object of this paper is to describe conditions leading to secondary cataract. Between 1960 and 1976, 251 children up to 12 years of age with bilateral congenital cataract were treated and followed up. Surgery was performed on 305 eyes (171 children). The total number of surgical interventions amounted to 707. In 125 cases, the occurrence of secondary cataract necessitated a second operation. Only through the posterior capsulotomy we were able to provide permanently clear optic media. In 85%

Experimental posterior penetrating eye injury in the rabbit. I. Method of production and natural history.

A technique has been developed which produces an experimental posterior penetrating eye injury that reproducibly results in traction retinal detachment in rabbit eyes. The standard injury is an incision through the pars plana with vitreous prolapse and incarceration; the wound is then carefully closed with microsurgical techniques. It appears that blood in the vitreous is an essential factor in the development of vitreous traction and of traction retinal detachment. Method of production and natural history of experimental posterior penetrating eye injury in the rhesus monkey.

We developed an experimental model for a posterior penetrating eye injury that resulted in traction retinal detachment in 21 rhesus monkey eyes. The standard injury was an incision through the pars plana with vitreous prolapse and incarceration; the wound was then carefully closed with microsurgical techniques. At one to two weeks after injury, detachment of the posterior vitreous occurred and detachment of the retina occurred between seven and 11 weeks. The development of retinal detachment was related to traction on the peripheral retina over the vitreous base; and to the contraction of epir

Depth of anterior chamber after cataract extraction.

A total of 80 patients with senile cataract had the anterior eye chamber depth measured optically by means of Haag-Streit's attachment II. The distance to the pupillary border was 2.59 +/- 0.05 mm (mean +/- SEM) preoperatively. It increased gradually after cataract extraction to 3.33 +/- 0.04 mm, measured 4 months after the operation. The increase of depth was the greatest in patients with a flat chamber and in elderly patients. The central chamber depth decreased gradually after the operation (from 2.82 +/- 0.05 mm preoperatively to 1.95 +/- 0.13 mm 4 months postoperatively). The number of vi

[Results of transpupillar vitrectomy (author's transl)].

Vitreous prolapse in cataract surgery or during the extraction of a dislocated lens; corneal dystrophy in aphacic eyes due to corneo-vitreous contact and vitreous invading the anterior chamber following perforating injury proved to be valid indications for partial transpupillary vitrectomy. In malignant glaucoma, in postoperative wound rupture following cataract extraction, in open-sky-surgery of the anterior segment of aphacic eyes and in other particular conditions, transpupillary vitrectomy may also be considered. The transpupillary approach is not indicated in retinal detachment, unless pr

Experimental cataract fragmentation:instrumentation and laboratory evaluation.

A newly designed mechanical phacofragmentator powered by compressed air was tested on 200 human hard nuclear cataracts and ten traumatic owl monkey cataracts. The device, which consists of a turbine-driven diamond burr, successfully fragmented hard human cataracts, both in vitro and when implanted in the anterior chamber of rabbits and monkeys. Iatrogenic damage to the iris was noted in the early cases, but no collapse of the anterior chamber or gross damage of the cornea occurred. Iris damage can be avoided, once the operator gains sufficient skill in manipulating the instrument. The ten trau

[The two-way syringe. A comparison to modified aspiration methods in cataract surgery, and more recent indications for its use (author's transl)].

After description of construction, indication, and application of the two-way syringe in cataract surgery, this instrument is compared to some of its numerous modfications. The original device has the advantage of simplicity, and enables the surgeon to perform simultaneous irrigation and aspiration under zero pressure conditions without an assistant. More recent indications for the use of the two-way syringe are vitreous prolapse, and retinal detachment. These operations are described in detail.

[Echography of the vitreous body in case of aphakia and malignant aphakic glaucoma (author's transl)].

The senile destructions of the vitreous body which can be seen with the slit-lamp (formation of visible fibres, vacuoles and posterior vitreous detachment) cause, if sufficiently advanced, small echoblips visible at echographic examination. We reexamined aphakic patients who had no complications during cataract cryo-extraction. In aphakic eyes without postoperative vitreous prolapse into the anterior chamber the echograms usually consisted of uninterrupted series of small amplitude echoes in the vitreous area. In aphakic eyes with postoperative vitreous prolapse into the anterior chamber echo-

[On iris reposition in case of extraction of lens (author's transl)].

A description is given of a pair of iris retractors which avoid certain shortcomings of other instruments or of other methods of retracting the iris in connection with cryo-extraction, particularly in case where the pupil cannot be dilated or only insufficiently. This pair of iris retractors has proved useful. In addition, information is given about our own technique to achieve a round functional pupil, after complicated lens-extractions with vitreous prolapse, by peripheral iridotomy parallel to the limbus and discission of the body.
