A standardised experimental posterior penetrating eye injury in the rhesus monkey with the vitreous-lens admixture resulted in limited fibrous ingrowth from the wound and minimal traction on the peripheral retina. Microscopically inflammation was noted in the iris, ciliary body, and as a perivascular infiltrate of the inner retina. This study reports the histological findings in human eyes after severe penetrating trauma. The findings confirm the high incidence of retinal detachment in eyes with severe penetrating injuries. The retina was detached in 32 out of the 34 eyes examined, with 27 having evidence of traction on the retina. These eyes were characterized histologically by intraocular cellular proliferation producing cyclitic, epiretinal, and retroretinal membranes. B-scan ultrasound of the posterior segment is reported in 154 patients with severe diabetic eye disease. Epiretinal fibrosis, vitreous haemorrhage, vitreous detachment, and retinal detachment were frequently diagnosed. The ultrasonic findings are discussed in relation to the pathological changes in the vitreous in proliferative diabetic retinopathy.

Seven cases of retinal cavernous haemangioma are presented. Three cases have been followed for more than 6 years, and three cases between 1 and 2 years. Six cases had no eye symptoms related to the vascular tumour, while in one case vitreous haemorrhage occurred on two occasions. On both these occasions full vision was regained. None of the vascular tumours were treated. Two patients had grand mal seizures. They also had convulsive disease in the family history. In three cases family members of two generations were found to have normal eyes on examination.

Racemose haegiomas of the retina are rare developmental anomalies. The basic lesion is an abnormal arterio-venous communication. Three new cases with long term follow-up are presented. In two of the cases the first symptom was loss of vision due to retinal and vitreous haemorrhage. In the third case gradual reduction of vision was due to vascular leakage into the macular area. The recent literature is reviewed and the association to midbrain haemangiomas discussed. The ophthalmological differential diagnosis, the prognosis and the present status in treatment is presented.

The results of 100 consecutive cases of pars plana vitrectomy are reported. Vitrectomy was performed on accunt of complications of diabetic retinopathy (37 eyes), complicated retinal detachment (28 eyes), vitreous haemorrhage of various causes (17 eyes), vitreous haemorrhage and complications secondary to injuries (13 eyes) and secondary cataract or vitreous in the anterior chamber creating corneal dystrophy (5 eyes). With an average follow-up time of 14.2 months, vitrectomy resulted in visual improvement in 55 eyes, unchanged visual acuity in 24 eyes and reduced visual acuity in 21 eyes. The

This review of the outcome of the conventional management of magnetic foreign bodies shows that, with the exception of the severe open-eye injuries associated with large foriegn bodies, where immediate lensectomy and vitrectomy is required, conventional magnetic extraction should be performed as close to the foreign body as is feasible. It is important to assess posterior segment damage both before and after extraction of the foreign body, as the subsequent management will depend on the presence of vitreous haemorrhage and retinal damage.

126 cases of retinal breaks treated in our department by photocoagulation during a period of 10 years (1963-1973) were selected for a close annual follow-up. Eliminated from the study were cases of macular holes, breaks which developed in an eye after retinal detachment surgery, and cases in which a close follow-up was impossible for various reasons. Thirteen patients with vitreous opacities were treated by intravitreal injection of urokinase. The central corneal thickness was measured daily on both eyes. Determination of visual acuity and ophthalmoscopy were done before treatment and at each following attendance (longest period of follow-up was seven months). The central corneal thickness increased after urokinase injection and the maximum thickness was reached on the second day. On the 6th day a secondary rise in corneal thickness occurred. A series of 53 eyes that had suffered severe trauma was treated by closed intraocular microsurgery with vitreous cutters. The cases were divided into three categories: (a) anterior segment reconstruction for traumatic cataracts and post-traumatic pupillary membranes, (b) vitreous haemorrhage with and without retinal detachment, and (c) traumatically dislocated lenses.

In the first hours after a vitreous haemorrhage dense enough to obscure the retina, the blood is usually confined to the posthyaloid space in an aqueous phase. Binocular occlusion and elevation provides sufficient immobilization of the eyes in 9 out of 10 patients for the blood cells to settle to the bottom of the space and make the retina available for examination and repair. Is this not done, blood will eventually enter the vitreous gel through holes that develop in the posterior hyaloid membrane. Blood in the gel does not settle and requires months to clear.

Urokinase is a plasminogen activator of human origin which breaks up the fibrin base of blood clots. When given as an intravitreal injection it produces hypopyon and glaucoma, both of which transient. In a series of 27 patients (34 eyes) with unresolved vitreous haemorrhage, this simple and relatively atraumatic treatment has produced marked objective improvement in 10, and greatly improved the life styles of a further 9. Vitreous haemorrhages were induced in rabbits and the echographic appearances of A and B scan were correlated with the gross and histologic appearances over a period of twenty weeks. In all cases, visible vitreous opacities produced distinct echo patterns. As the haemorrhages cleared, the echo amplitudes became progressively weaker. Several patterns of vitreous "membranes" on B scan were also observed. Pathological examination revealed that such membranes were composed of red cells, macrophages and condensed vitreous collagen. In 554 consecutive primary cases, adverse prognostic factors for successful retinal detachment surgery were aphakic retinal detachment, severe vitreous traction creating retinal breaks, especially with lattice degeneration, long-standing retinal separations, choroidal or combined detachments, giant tears, Marfan's syndrome, and preoperative massive vitreous retraction (MVR).
