BACKGROUND: Subfoveal choroidal neovascularization is a main cause of blindness. The new surgical technique of macular translocation conceived by De Juan, with scleral shortening and without a retinotomy, allows to move the fovea away from the neovascular membrane with a low rate of complications. The first results obtained with this technique are presented here, in cases of submacular neovascularization due to age-related macular degeneration (AMD) or degenerative myopia.

Visual function deteriorates additionally in high degenerative myopia in consequence of initial cataract causing increase of refractive index and refractive error. This paper presents indications for removal of initial cataract with posterior chamber lens implantation to improve visual acuity, colour perception, contrast sensitivity and to broaden visual field. Particular conditions of cataract surgery in myopic eye, selection criteria of operative technique and anaesthesia, power and diameter of implant and possible postoperative complications are discussed.

A study of all the schools for the blind in the West Bank and Gaza Strip was undertaken during 1991-2. Two hundred and five children had a complete ophthalmic examination. This represented 94% of all children aged 5-15 years attending schools for the blind. According to WHO categories of visual loss, 63% were blind and 21% had severe visual impairment. High or degenerative myopia is associated to abnormalities of the posterior pole of the eye. Their expansion with time progresses to the occurrence of complications, especially neovascular ingrowth in the central macular area. The natural evolution of these new vessels results in loss of central vision at long term. Photocoagulation of the new vessels, sparing initially the center, has delayed the loss of central vision at 2 years in our randomized clinical trial. We present two cases of degenerative myopia with abduction deficiency. Three mechanisms can explain the defect in the abduction: (a) the size of the long globe filling the space of the orbits, (b) the tightness of the medial recti due to long axis of the globe, and (c) longstanding esotropia becoming decompensated later in life. We believe that high myopia is not a well-known cause of abduction deficiency, and it should be considered in the differential diagnosis.

The authors performed a prospective study of 36 eyes affected by pathologic myopia with macular subretinal neovascularization (SRNV) successfully treated with either argon green, dye orange (590 nm), or krypton red lasers.
