Identification of key genes associated with butyrate metabolism in keratoconus through Mendelian randomization and transcriptomic analysis.

BACKGROUND: Keratoconus (KC) is a degenerative corneal disease with a complex etiology that severely affects vision. Butyrate is a saturated short-chain fatty acid with anti-inflammatory properties. Accumulating evidence underscores a significant role for inflammation in KC pathogenesis. However, the specific role of butyrate metabolism in KC remained unclear. This study aimed to identify key genes associated with butyrate-related regulatory pathways in KC. METHODS: The KC-related dataset GSE77938, GSE151631, genome-wide association studies (GWAS) data for KC (finn-b-H7_CORNEALDEFORM) and but

Structure-based virtual screening identifies VX-809 as a candidate dual-pathway modulator in fuchs endothelial corneal dystrophy.

Fuchs endothelial corneal dystrophy (FECD) is a progressive degenerative disease of the corneal endothelium and remains a leading indication for corneal transplantation worldwide. FECD is characterized by excessive extracellular matrix (ECM) deposition, disruption of proteostasis with endoplasmic reticulum (ER) stress, and progressive endothelial cell loss; however, no pharmacological therapy is currently available. Given that FECD involves multiple interacting pathogenic pathways, we asked whether a structure-based polypharmacology approach could identify a single small molecule capable of mo

Saffron as a Retinal Neuroprotectant: A Narrative Review of Preclinical Studies and Clinical Results.

The present narrative review reports the main preclinical and clinical results obtained by using supplementation of saffron or its pure components in neurodegeneration, with special emphasis on age-related macular degeneration. Beyond that, this article will address shared pathways between neurodegenerative diseases of the eye and the brain. It will be shown that saffron treatment might counteract oxidative damage in the retina and brain, as well as inflammation and inflammatory mediators that induce neuronal degeneration and death. The ways of action are multiple, and saffron chemical compone

Targeting the OPA1 pathway in Autosomal Dominant Optic Atrophy (ADOA): 25 years from gene discovery to therapeutic strategy.

INTRODUCTION: Autosomal Dominant Optic Atrophy (ADOA) is a rare hereditary optic neuropathy primarily caused by OPA1 mutations. Retinal ganglion cell (RGC) loss results in variable visual impairments, occasionally accompanied by extra-ocular manifestations. ADOA also involves a developmental component consistent with OPA1's essential role in mitochondrial fusion, cristae organization, and quality control. As such, ADOA serves as a paradigm for studying mitochondrial contributions to neurodegeneration. AREAS COVERED: This article provides a comprehensive overview of ADOA, covering genetic and

Bilateral refractory macular holes related to tamoxifen use and full anatomic restoration of the fovea with autologous retinal graft.

BACKGROUND: Low-dose tamoxifen use may cause degenerative changes progressing to full-thickness macular holes, mimicking other degenerative causes for macular holes instead of well-known features of tamoxifen retinopathy. The aim of this report is to present a case of bilateral refractory macular holes related to tamoxifen use and full anatomic restoration of the fovea with autologous retinal graft. CASE PRESENTATION: A 35-year-old female presented with decreased vision in both eyes and was initially diagnosed with a macular hole in the left eye, followed by a subsequent diagnosis in the righ

Identifying potential therapeutic targets for high myopia via a case-control study and Mendelian randomisation analyses of the human blood metabolome.

BACKGROUND: High myopia increases the risk of pathological ocular changes that may lead to irreversible vision loss. Therefore, the identification of potential biomarkers and therapeutic targets for high myopia is essential for early intervention and prevention. METHODS: Summary statistics for 122 blood metabolites were obtained from three genome-wide association studies (GWASs), whereas data on high myopia were derived from a large GWAS conducted with 50,372 participants from the UK Biobank. Mendelian randomisation (MR) analyses were conducted to assess the causal relationships between blood

Artificial intelligence-based retinal imaging for brain health assessment: a scoping review.

Brain health is an emerging and rapidly expanding interdisciplinary field that requires optimisation across the life course of a person. The retina serves as an optically accessible window for studying brain health and related neurodegenerative and cerebrovascular diseases, which impose substantial global burdens. With advances in artificial intelligence (AI) and retinal imaging, the field of oculomics-defined as the identification of ocular imaging-based markers of systemic diseases-has grown considerably and shows promising applications in brain health. In this scoping review, we propose a p

A fatty acid amide activates myeloid cells and improves neurovascular outcomes in retinal degeneration.

Neurovasculoglial cross-talk underlying breakdown of the neurovascular unit is a central, yet poorly understood, component of many neurodegenerative disorders of the CNS, including retinal disease. Primary fatty acid amides have been identified to regulate this cross-talk between vasculature and neuronal tissues, but specific molecules and mechanisms remain unresolved. Here we show, using an unbiased high-resolution metabolomics screen, that erucamide, a 22:1 monounsaturated omega-9 fatty acid amide, is highly dysregulated during photoreceptor degeneration in mice. In vivo delivery of erucamid

Programmed axon degeneration gene variants in human disease.

BACKGROUND: Programmed axon degeneration (PAD; also known as Wallerian degeneration) is a conserved pathway controlling axon breakdown following injury or metabolic stress. PAD is driven by the depletion of nicotinamide adenine dinucleotide (NAD) through loss of the pro-survival enzyme NMNAT2 and activation of the pro-degenerative NADase SARM1. Recent genetic studies have identified pathogenic variants in PAD pathway enzymes associated with severe neurodegenerative phenotypes. MAIN BODY: Pathogenic variants in NAMPT, NMNAT1, NMNAT2, and SARM1 have been identified and will be discussed in this

Decreasing Radial Peripapillary Capillary Perfusion Density Correlated With Visual Field Impairment in High Myopia.

PURPOSE: To measure structural and microvascular alterations of the optic nerve head in high myopia and correlate the measured parameters with the visual field mean sensitivity defect. METHODS: Thirty-two control eyes and 53 eyes with high myopia were analysed to quantify the thickness of the peripapillary retinal nerve fibre layer (pRNFL) and density of radial peripapillary capillary (RPC) perfusion. The pRNFL thickness, RPC perfusion density and other parameters were compared between the two groups and correlations between structural/microvascular alterations and visual field mean deviation

Chiral Nanoparticles Suppress Inflammatory Infiltration to Promote Extracellular Matrix Remodeling for Ectopia Lentis Therapy.

Chirality, a fundamental geometric property of matter, plays a pivotal role in biological systems by mediating stereoselective molecular interactions and cellular regulation. Inspired by this principle, chiral nanoparticles have emerged as promising candidates for immunomodulation in inflammation-related diseases. However, whether and how their chiral cues regulate extracellular matrix (ECM) remodeling remains largely unexplored. Herein, we report the design of L-cysteine-configured polyurethane nanoparticles (L-CN) through covalent grafting of naturally chiral L-cysteine, and demonstrate thei

Retinal manifestations of Alzheimer's Disease: Insights from animal models, clinical detection, and future translation.

The retina, as an extension of the central nervous system, shares a common embryological origin with the brain. In Alzheimer's disease (AD), studies of human tissue and animal models have revealed that hallmark AD pathologies, including amyloid-β (Aβ) deposits and pathological tau protein tangles, also appear in the retina. These findings, coupled with advances in high-resolution retinal imaging techniques, suggest the potential to detect and characterize AD-related molecular and structural changes in the retina. However, retinal findings across different AD mouse models have significant discr

Fast nanoDSF Tear Fluid Profiling: Toward Diagnosis of Age-Related Macular Degeneration.

Background: Age-related macular degeneration (AMD) is the leading cause of irreversible vision loss in older adults. An important challenge is the recognition of its early asymptomatic stages and the monitoring of its progression, which requires reliable biomarkers. Growing evidence indicates that AMD-related biochemical changes are reflected in the proteome of tear fluid (TF). Although TF is a non-invasive and easily collectable diagnostic material, its proteomic analysis is complex and costly and therefore has limited clinical value. Methods: In this pilot single-center retrospective cross-s

Transcranial magnetic stimulation as a novel therapeutic approach for severe retinal degenerative diseases: a pilot study.

BACKGROUND: Retinal degenerative diseases, including myopic macular degeneration (MMD) and retinitis pigmentosa (RP), lead to irreversible vision impairment, with limited treatment options. Transcranial magnetic stimulation (TMS), a non-invasive brain stimulation technique for rehabilitation, may hold potential for vision restoration. The pilot study explored whether TMS is associated with short-term functional visual improvements in patients with severe retinal degenerative diseases. METHODS: This prospective, non-randomised, parallel-arm pilot study enrolled 98 patients with bilateral sever

Single-cell spatial transcriptomics reveals rewiring of RPE and Müller glia signaling toward photoreceptors following outer segment disruption in Prph2C213Y mice.

PRPH2 mutations cause inherited retinal dystrophies (IRDs), but how photoreceptor outer segment (OS) disruption reshapes the surrounding retina remains unclear. Using a heterozygous Prph2C213Y/+ mouse model generated by CRISPR/Cas9, we characterized age-related retinal pathology and responses of retinal pigment epithelium (RPE) and Müller glia. Independent age- and sex-matched cohorts were examined at 1, 3, and 6 months by electroretinography, optical coherence tomography, and fundus autofluorescence. Mutant mice showed rod dysfunction from 1 month, RPE dysfunction from 3 months, and cone dysf

The Biology and Cell Engineering of Retinal Pigment Epithelium Cell Transplantation: Developing a Living Drug.

Age-related macular degeneration (AMD) is a multifactorial retinal disease that causes progressive vision loss, with retinal pigment epithelium (RPE) atrophy representing a key initiating event. The RPE monolayer plays a crucial role in maintaining photoreceptor and choriocapillaris health and function. In advanced AMD stages, degeneration extends to the photoreceptors and choriocapillaris, likely as a consequence of preceding RPE atrophy. Existing treatment modalities aim to slow disease progression rather than restore degenerated cells and improve visual function. Pluripotent stem cell-deriv
