Metformin and incidence of age-related macular degeneration in people with diabetes: a population-based 5-year case-control study.

OBJECTIVE: Metformin has been identified as a potential treatment for age-related macular degeneration (AMD). Photographic screening for diabetic retinopathy provides an opportunity to conduct a case-control study with systematic AMD grading. We aimed to investigate associations between metformin use and incidence and progression of AMD at different grades. METHODS AND ANALYSIS: We randomly sampled 2600 participants from 10 336 people aged ≥50 years with diabetes who attended retinopathy screening in 2011 (baseline) and were enrolled to the Individualised Screening for Diabetic Retinopathy st

How Do Optometrists Approach Directed Questioning on Smoking Status in a Centrally Co-Ordinated, Publicly-Funded National Primary Care Eye Health Service?

BACKGROUND: Tobacco use is a leading cause of preventable death worldwide, linked to various health issues, including cancers, cardiovascular diseases, and respiratory illnesses. It also adversely affects ocular health, increasing the risk of conditions like Age-Related Macular Degeneration and cataracts. The socioeconomic burden of smoking in the UK is significant, costing the National Health Service between £2.7 and £5.2 billion annually. Smoking cessation improves health outcomes and reduces healthcare costs. Optometrists are well positioned to identify smoking behaviours and offer cessatio

Enhancing mouse fundus imaging with OCT embedding medium: Prolonging imaging duration and improving image quality.

Fundus diseases are common and serious ophthalmic conditions, including macular degeneration, diabetic retinopathy, glaucoma, and others, which can lead to severe vision loss. Mice, widely used as experimental animal models, play a crucial role in ophthalmic disease research. However, existing mouse fundus imaging techniques encounter several challenges during prolonged, high-precision imaging, such as corneal whitening, poor optical quality, and motion artifacts. In this study, we propose an innovative method based on an OCT embedding medium that, when applied to the mouse corneal surface, im

The Heterogeneity of the Neurovascular Unit in the Optic Nerve Head During Glaucomatous Neurodegeneration Based on Single-Cell RNA Sequencing.

Glaucoma is a multifactorial neurodegenerative disease, and the dysfunction of the neurovascular unit (NVU) in optic nerve head (ONH) has been gradually recognized to contribute to the neurodegeneration progression, yet its heterogeneity features remain poorly understood. Here, we established rat models of high intraocular pressure (HIOP) and lipopolysaccharide-induced ocular inflammation (LPS), and performed single-cell RNA sequencing (scRNA-seq) of ONH-centered tissues to characterize the cellular and molecular characteristics of NVU and its main cell components. Distinct alterations in the

Crosstalk Between R-Loops, RNA/DNA Modifications, and Cell Death Dynamics in Canine Models of Retinitis Pigmentosa.

IMPORTANCE: Repeated transfusions in preterm neonates with anemia of prematurity replace fetal hemoglobin (HbF) with adult Hb (HbA), which has a low oxygen affinity. The reduction of HbF is associated with a higher incidence of retinopathy of prematurity (ROP). OBJECTIVE: To assess whether HbF and HbA are differently associated with cerebral tissue oxygenation in preterm neonates. DESIGN, SETTING, AND PARTICIPANTS: This cohort study was a single-center, pilot study on cerebral oxygenation kinetics in preterm neonates with a gestational age between 24.0 weeks and 27.9 weeks who were admitted

Laterally spreading AAV.SPR enables safe and efficient RS1 delivery to the macula after peripheral subretinal injection.

Mutations in RS1 are associated with X-linked retinoschisis (XLRS), a common cause of juvenile macular degeneration in males. Schisis cavities in the central retina of these patients have hampered submacular delivery with conventional adeno-associated viruses (AAVs). Clinical trials employing intravitreally injected AAVs showed a lack of efficacy and inflammation. Here, we demonstrate, in non-human primate (NHP) retina, that AAV.SPR, a laterally spreading capsid, transduced photoreceptors in the macula/fovea without the need for central retinal detachment, enabling transgene expression multipl

CCR1 signaling as a common injury pathway in retinal degeneration.

Inflammation is a key driver of atrophic Age-Related Macular Degeneration (aAMD), and also plays a role in Inherited Retinal Degenerations (IRDs), two major causes of irreversible visual loss. We previously demonstrated that the C-C chemokine receptor type 1 (CCR1) is upregulated in monocytes from AMD patients, and that it mediates the recruitment of neurotoxic macrophages and activates Müller glial cells in rodent model of photic retinal injury. Here we report that CCR1 is expressed in Müller glia in eyes affected by AMD and that variants in CCR1 are potentially associated with the rate of ma

Bilateral Vortex Keratopathy in a Patient Receiving Long-Term Ribociclib Therapy.

We report a case of bilateral vortex keratopathy associated with long-term ribociclib therapy in a 67-year-old woman with metastatic breast cancer. The corneal deposits were identified during routine ophthalmic examination after 4 years of continuous treatment. Slit-lamp biomicroscopy revealed bilateral whorl-like epithelial deposits with a subtle morphological appearance compared with classic amiodarone-associated cornea verticillata. The patient had no history of tamoxifen, amiodarone, or chloroquine use, and there were no clinical features suggestive of Fabry disease. The combined severity

The chemical oxidation of albumin affects its capability to bind amyloid beta 1-40 peptide and to inhibit its aggregation.

Human Serum Albumin (HSA) is the most abundant protein in the human body, exerting vital antioxidant and carrier functions. Beyond its systemic roles, HSA is clinically employed as a therapeutic tear supplement to restore the ocular surface proteome in severe dry eye and corneal diseases. However, the functional integrity of HSA is frequently compromised by oxidative stress, a hallmark of several neurodegenerative proteinopathies. While elevated levels of oxidised HSA have been documented in the blood and cerebrospinal fluid of Alzheimer's Disease patients, similar oxidative modifications are

Yingmu San ameliorates pathological myopia by protecting retinal pigment epithelium and enhancing mitophagy-associated responses.

BACKGROUND: Pathological myopia is associated with progressive axial elongation and retinal pigment epithelium (RPE) injury, in which oxidative stress, inflammation, and mitochondrial dysfunction are important pathological contributors. Yingmu San (YMS) is a defined multi-component formulation composed of peony seed oil (PSO), lutein, and zeaxanthin at fixed proportions. However, whether YMS protects the RPE during myopia progression and whether mitophagy-associated responses are involved in this effect remain unclear. PURPOSE: This study aimed to determine whether YMS protects RPE cells and

The eyes and Parkinsonian syndromes.

Parkinsonian degenerative syndromes, including Parkinson disease (PD), progressive supranuclear palsy (PSP), multiple system atrophy (MSA), corticobasal degeneration (CBD), and dementia with Lewy bodies (DLB), are frequently associated with ophthalmic manifestations that contribute substantially to impaired quality of life, reduced functional independence, and increased falls risk. These visual disturbances remain under-recognised in routine neurological practice despite their diagnostic and therapeutic relevance. This review summarises the spectrum of ophthalmic manifestations across Parkinso

Biologics-device combinations: Enabling prolonged therapies in the posterior segment ocular disease.

Posterior segment ocular diseases (e.g., age-related macular degeneration and diabetic retinopathy, etc.) often necessitate frequent intravitreal (IVT) injections of biologics, due to the rapid drug clearance and formidable ocular barriers. While molecular engineering strategies and high-concentration protein formulations could extend the administration intervals to a certain extent, they are confronted with critical challenges, protein aggregation, high viscosity, and limited duration. This has spurred the development of innovative biologics-device combination products, which represent a para
