Telemedicine-based diabetic retinopathy screening in patients over 70 Years: a French cohort study within the OPHDIAT network.

AIM: To assess the outcomes of teleophthalmology-based diabetic retinopathy (DR) screening in individuals over 70 years within the OPHDIAT network and to compare them with those of patients aged 18-69 years. METHODS: A cohort of 16,459 diabetic patients, without known DR or with mild non-proliferative DR (NPDR), screened in 2024 in 32 OPHDIAT centers, was included and divided into two groups: < 70 years (n = 13,639) and ≥ 70 years (n = 2,820). Two non-mydriatic retinal photographs per eye were analyzed by certified ophthalmologists. RESULTS: Among patients aged ≥70 years, 21.3% (95% CI: 19.8

Outer retinal tubulation associated with photoreceptor degeneration.

Outer retinal tubulation (ORT) is a distinct structural manifestation of chronic photoreceptor degeneration, observed across a broad spectrum of retinal diseases. Initially described histologically as rosette-like formations, ORT has gained clinical relevance with the advent of high-resolution imaging modalities such as spectral-domain optical coherence tomography (SD-OCT) and adaptive optics scanning laser ophthalmoscopy (AO-SLO), which enable in vivo visualization of its tubular architecture. ORT arises from sustained photoreceptor and retinal pigment epithelium (RPE) injury, leading to the

Synaptopathy in cone and rod cells after retinal detachment and reattachment.

Using the powerful technique of stimulated emission depletion (STED) confocal microscopy in combination with 3D imaging, we describe the degeneration of cone and rod synapses after one week of retinal detachment in a porcine model. Synaptic invaginations are lost, ribbons change size and shape, and bipolar dendrites sprout. Spontaneous reattachment after hours up to 2 days after detachment appears to restore cone pedicles but does not repair rod spherules one week later. Disjunction of rod synapses remains, both at the point of initial detachment and in areas that were not detached. Moreover,

PINN-EM: Physics-Guided Disease Progression Model of Geographic Atrophy.

OBJECTIVE: To construct a personalizable spatio-temporal disease progression model of patients with a late dry form of Age-Related Macular Degeneration (AMD), known as Geographic Atrophy (GA). METHODS: From a series of retinal optical coherence tomography (OCT) scans, we infer the coefficients for the parametrized partial differential equation (PDE), such that the parametrized PDE best describes the observed imaging data. Acting as a soft constraint, the recovered PDE helps to extrapolate an implicit neural representation (INR) of the GA segmentation map progression. To enable efficient train

Prosaposin is a dual-action therapeutic target for vascular stabilization and neuroprotection.

The disruption of vascular homeostasis is a key pathological feature of fundus abnormal vascular diseases, including wet age-related macular degeneration, diabetic macular edema, and retinal vein occlusion. While anti-vascular endothelial growth factor therapies are widely used, many patients exhibit limited or suboptimal responses. Through proteomic analysis of 173 aqueous humor samples from fundus abnormal vascular disease patients, followed by validation in an independent cohort of 70 samples, this study identifies prosaposin as a potential therapeutic candidate. In vitro, prosaposin mitiga

Early Rod Dysfunction Influences Cone Development in a Rhodopsin P23H Mouse Model of Retinitis Pigmentosa.

Background/Objectives: The RhoP23H/WT mouse line is a commonly used model to study rhodopsin P23H-associated autosomal dominant retinitis pigmentosa. Previous studies in RhoP23H/WT mice have largely focused on retinal changes occurring at one month of age and later, and have indicated a compensatory thickening of inner retinal layers in response to rod degeneration. However, the effect of disease processes during early postnatal retinal development remains understudied. Methods: In this study, we investigated the retinal response to rod dysfunction during early postnatal developmental ages P8-

[Liquid biopsy proteomics in ophthalmology : A clinical and scientific perspective].

BACKGROUND: For many patients with age-related macular degeneration, diabetic retinopathy and other partially monogenetic retinal diseases as well as for tumors of the eye that are relatively rare but are usually associated with profound consequences for affected patients, there is still no effective treatment available. Metastatic melanoma, for example, remains poorly predictable with respect to disease progression, response to treatment and outcome. This illustrates the urgent need for a deeper molecular understanding of the disease with the goal to develop novel therapeutic strategies. Liqu

Advancements and challenges in ophthalmic microneedles to treat eye diseases.

By 2050, more than 61 million people worldwide are expected to lose their vision due to conditions like age-related macular degeneration, glaucoma, diabetic retinopathy, and uveitis (Bourne et al. 2021). This anticipated rise highlights the urgent need for more effective treatment options. While progress continues in developing new pharmacological agents, treating ocular diseases with these therapies remains particularly challenging due to the eye's unique and complex anatomy. This is largely due to the limitations of current drug delivery methods, including systemic administration, topical de

FGF21 protects retinal pigment epithelium from sodium iodate-induced injury: Association with inhibition of ferroptosis and the NRF2/GPX4 pathway.

Age-related macular degeneration (AMD) is a leading cause of blindness, with retinal pigment epithelium (RPE) cell death representing a central pathological event. Ferroptosis, an iron-dependent form of regulated cell death, has recently been implicated in RPE degeneration. Although fibroblast growth factor 21 (FGF21) has demonstrated cytoprotective effects in various contexts, its specific role and mechanism in RPE protection, particularly concerning ferroptosis, remain unexplored. This study investigated the protective effect of FGF21 against sodium iodate (NaIO3)-induced damage and its unde

Epigenetic reprogramming for ocular aging and disease: Mechanisms, biomarkers, and the road to the clinic.

The eye's visual function relies on retinal neural cells that are long-lived, post-mitotic, and possess minimal regenerative capacity. These combined properties render them exceptionally vulnerable to the cumulative damage that drives age-related functional decline. Accumulating evidence now implicates epigenetic alterations, such as aberrant DNA methylation and histone modifications, not merely as correlates of aging but as fundamental drivers of aging and disease. These changes disrupt the stable gene expression programs required to maintain cellular identity and function, thereby contributi

Pbx1 overexpression delays cochlear hair cells degeneration in an accelerated aging mouse model.

Age-related hearing loss (ARHL), the most prevalent sensory disorder worldwide, arises primarily from cochlear hair cells (HCs) degeneration due to aging. Although the molecular mechanisms driving HC senescence are increasingly understood, effective treatments for ARHL remain lacking. This study explores the therapeutic potential role of Pre-B cell leukemia homeobox 1 (Pbx1), a transcription factor involved in inner ear development and pluripotency, in mitigating ARHL. Our results reveal a striking age-dependent reduction in PBX1 expression within mouse cochlear HCs. Using D-galactose (D-gal)/

Organotypic Retinal Explant Culture as a Model for Neuroretinal Degenerative Disease and Future Applications.

Organotypic retinal explant cultures are a valuable experimental tool for investigating neuroretinal diseases and advancing therapeutic strategies. Ex vivo methods preserve the natural architecture and intercellular interactions of the retina while enabling precise manipulations of the microenvironment, a distinct advantage over traditional in vitro two-dimensional cell models. Compared with in vivo animal and human studies, retinal explants offer ethical and cost-effective advantages, serving as bridge models for both animal to human and preclinical to clinical studies. This review explores t

Recent advances of aggregation-induced emission materials in ophthalmic diseases.

Ocular diseases impose significant therapeutic challenges due to drug delivery barriers and limitations of current treatment procedures. This review explores how aggregation-induced emission (AIE) technology offers advantages over conventional treatments by unifying precision imaging and targeted therapy. AIE luminogens exhibit significantly enhanced fluorescence and reactive oxygen species (ROS) generation in aggregated states, enabling real-time visualization and spatiotemporally controlled intervention. Crucially, AIE platforms address key constraints of existing approaches: their physical
