Hydrogels as Promising Carriers for Ophthalmic Disease Treatment: A Comprehensive Review.

Ocular disorders such as keratitis, glaucoma, age-related macular degeneration (AMD), diabetic retinopathy (DR), and dry eye disease (DED) are highly prevalent worldwide and remain major causes of visual impairment and blindness. Conventional therapeutic approaches for ocular diseases, such as eye drops, surgery, and laser therapy, are frequently hampered by limited drug bioavailability, rapid clearance, and treatment-related complications, primarily due to the eye's unique anatomical and physiological barriers. Hydrogels, characterized by their three-dimensional network structure, high water

A Novel, Low-Cost, 3D-Printed Motorized Injector for Retinal Sheet Transplantation.

Retinal transplantation offers promise for restoring vision in advanced retinal degeneration. However, manual delivery of retinal sheets is often hindered by imprecise placement and collateral tissue damage resulting from instrument instability. We introduce a novel, 3D-printed, motorized retinal sheet injector designed to enhance placement accuracy and minimize tissue injury. The motorized injector features an Arduino-controlled foot pedal with three discrete actuator positions ("Min", "Mid", "Max"). When compared via frame-by-frame motion analysis, the motorized system reduced tip variance b

Computational Modeling of Parkinson's Disease Across Scales: From Mechanisms to Biomarkers, Drug Discovery, and Personalized Therapies.

Parkinson's disease (PD) is a multifactorial neurodegenerative disorder characterized by complex interactions across molecular, cellular, circuit, and behavioral scales. While experimental and clinical studies have provided critical insights into PD pathology, integrating these heterogeneous data into coherent mechanistic frameworks and translational strategies remains a major challenge. Computational modeling offers a powerful approach to bridge these scales, enabling the systematic investigation of disease mechanisms, candidate biomarkers, and therapeutic strategies. In this review, we surve

Altered Ocular Surface Temperature in Congenital Aniridia with PAX6 Pathogenic Variants: Impact of Age, Salzmann Nodules and Ocular Surgery.

PAX6 haploinsufficiency-related congenital aniridia is frequently associated with ocular surface disease, including meibomian gland dysfunction (MGD), dry eye, limbal stem cell deficiency (LSCD), aniridia-associated keratopathy (AAK), and inflammation. This study measured ocular surface temperature (OST) at the corneal center and four paracentral points (2 mm from center) in patients with congenital aniridia and examined factors influencing OST. Forty-five eyes from 26 aniridia patients (55.6% female; 26.29 ± 17.78 years) with PAX6 pathogenic variants and 47 eyes from 25 controls (68.1% female

Prevalence of Polypoidal Choroidal Vasculopathy Beyond Recalcitrant Macular Neovasculopathies in a European AMD Cohort and Therapeutic Response to Brolucizumab.

Background: Polypoidal choroidal vasculopathy (PCV) may be underdiagnosed in Europe due to the limited use of indocyanine green angiography (ICGA), in particular, in patients with occult or poorly responsive neovascular age-related macular degeneration (AMD). Based on this, we aimed to assess the prevalence of PCV beyond clinically typical recalcitrant nAMD using OCT criteria, and to report on the anatomical and functional outcomes following a switch to brolucizumab (bro). Methods: This retrospective case series used recently established optical coherence tomography (OCT)-based criteria to dif

Acid sphingomyelinase inhibition restores RPE homeostasis and photoreceptor function in preclinical Stargardt macular degeneration models.

Stargardt disease, which destroys central high-resolution vision in over 2 million people globally, lacks effective therapies. The primary site of damage in Stargardt disease is the retinal pigment epithelium (RPE), which safeguards photoreceptor health and function. Progressive loss of RPE integrity precedes visual deficits, yet insight into mechanisms driving RPE dysfunction and how this influences disease pathogenesis remains elusive. Here, we addressed this in cell-based and pigmented Abca4-/- Stargardt mice models using super-resolution imaging, bioinformatics, and biochemical approaches.

Gut inflammation affects retinal fatty acid composition.

Inflammation and altered lipid metabolism - particularly bioavailability of polyunsaturated fatty acids of the omega-3 series (n-3 PUFAs) - are associated with age-related macular degeneration (AMD). Growing evidence suggests that the gut plays a significant role in regulating retinal physiology. Recent studies have reported that patients with chronic inflammatory bowel diseases (IBD) may have an increased risk of developing AMD. We investigated the influence of colitis on retinal fatty acids, by using a dextran sodium sulfate (DSS)-induced colitis model in mice. Compared to control mice, the

Mimicking Nature, Modulating Vision: Peptidomimetic Neurotrophin Agonists and Emerging Regenerative Strategies in Ocular Disease.

Therapeutic strategies for ocular diseases are undergoing a transformative shift from symptom management to regenerative and disease-modifying approaches. This review highlights the development of neurotrophin receptor agonists-including recombinant nerve growth factor (NGF) (cenegermin), peptidomimetics (e.g., REC-0559, tavilermide), and synthetic microneurotrophins (BNN27, ENT-A010)-that target tropomyosin receptor kinases (TrkA/TrkB) and the p75 neurotrophin receptor (p75NTR) pathways to promote neuronal survival, synaptic plasticity, and tissue repair in neurotrophic keratitis, dry eye dis

Opportunities and challenges in leveraging multiomics and biobanks for vision science.

PURPOSE OF REVIEW: Emerging biobank resources allow large-scale integration of eye-specific phenotypes with clinical, genomic, and multiomic data. This convergence enables unprecedented opportunities to systematically dissect the genetic architecture, epidemiology, and mechanistic pathways of both rare monogenic and common polygenic diseases. The review aims to critically examine how contemporary data extraction, multiomics, and analytic methodologies are reshaping disease classification, genetic discovery, and translational research in ophthalmology, while highlighting the associated challeng

Combined Expression of hRdCVF and hRdCVFL Through AAV-Mediated Delivery for the Treatment of Retinitis Pigmentosa.

PURPOSE: Retinitis pigmentosa (RP) is an inherited retinal degeneration that progresses from rod to cone degeneration. Several gene-independent adeno-associated virus (AAV)-based strategies have revealed a partial restauration of RP using AAV vectors coding for either rod-derived cone viability factor (RdCVF) or its long form (RdCVFL). The nucleoredoxin-like 1 gene (NXNL1) encodes two proteins, the short isoform RdCVF and the long isoform (RdCVFL). RdCVF acts by stimulating aerobic glycolysis to sustain cone outer segment renewal, and RdCVFL is thought to modulate oxidative stress. In prior st

Prechoroidal cleft in neovascular age-related macular degeneration (AMD): A narrative review of pathophysiology, clinical implications and prognosis.

The prechoroidal cleft (PC) is a hyporeflective space visualized on optical coherence tomography (OCT), typically located within the pigment epithelial detachments (PEDs) in neovascular age-related macular degeneration (nAMD). Although its pathogenesis is not yet fully understood, proposed mechanisms include fluid accumulation secondary to macular neovascularization (MNV), contractile forces from fibrovascular tissue and chronic structural remodelling of the outer retinal layers. This narrative review synthesizes the current evidence regarding the imaging features, pathophysiological mechanism

Bilateral Diffuse Uveal Melanocytic Proliferation Masquerading as Refractory Subretinal Fluid Due to Peripapillary Pachychoroid Syndrome.

We describe a case of peripapillary pachychoroid syndrome (PPS) that appeared resistant to conventional treatment and was subsequently diagnosed as bilateral diffuse uveal melanocytic proliferation (BDUMP) based on fundus autofluorescence (FAF) images. A 79-year-old man presented with vision loss in both eyes and was initially diagnosed with PPS based on optical coherence tomography, fluorescein angiography, and indocyanine green angiography findings. The patient underwent photodynamic therapy in both eyes, photocoagulation in the left eye, and 2 mg aflibercept injections in both eyes. Althoug

Chaperone-mediated autophagy: the Achilles heel of the retinal pigment epithelium during age-related macular degeneration.

Chaperone-mediated autophagy (CMA) is a selective autophagy pathway that targets specific proteins containing a KFERQ-like motif for lysosomal degradation. It has been shown by us and others that CMA decreases during physiological aging in most tissues, and its impairment is associated with increased incidence of age-related pathologies, such as cardiovascular disease, neurodegenerative disorders or sarcopenia. However, its involvement in age-related macular degeneration (AMD), a prevalent progressive maculopathy that leads to bilateral central vision loss, had not been explored. In the early

Long-Term Evolution and Growth Rate Assessment of Non-Exudative Macular Neovascularization: A Multicenter Analysis.

INTRODUCTION: Non-exudative macular neovascularization (neMNV) in age-related macular degeneration (AMD) may remain stable or progress to exudation with vision loss, but its natural history is not well established. This study aimed to assess the prevalence of neMNV, the conversion rate to exudation, and to discern differences between neMNVs that exhibited exudation versus those that remained non-exudative. METHODS: In this retrospective, multicentric, longitudinal study, consecutive patients treated with anti-VEGF between august 2008 and June 2023, presenting exudative AMD in one eye and inte
