Trends in Ophthalmic Hospital Transfers and Consultations Across US Academic Centers.

IMPORTANCE: Academic medical centers increasingly serve as a safety net for urgent and complex ophthalmic conditions, yet trends in the demand for hospital-based ophthalmic services remain poorly characterized. OBJECTIVE: To characterize multi-institutional trends in ophthalmology interhospital transfer requests and emergency department (ED) and inpatient ophthalmology consultation requests (hereafter, consultations) from other departments within the same hospital, such as emergency medicine or internal medicine, at multiple academic medical centers in the US. DESIGN, SETTING, AND PARTICIPAN

Ocular Manifestations in Rheumatic Diseases.

Ocular involvement is common in systemic rheumatic diseases and may precede or reflect systemic activity, serving as an important diagnostic clue. Inflammation can affect any ocular structure, with presentations ranging from mild to vision-threatening. Early recognition is critical, as ocular findings can guide diagnosis and management. A systematic approach combining clinical evaluation, imaging, and laboratory testing is essential to identify disease-specific patterns. Advances in immunomodulatory and biologic therapies have improved outcomes. Optimal care requires a multidisciplinary approa

Masquerade Syndrome Presenting as Optic Neuritis and Retinitis Due to Pituitary Germinoma.

Masquerade syndrome is a collective term for various ocular diseases presenting with clinical features resembling intraocular inflammation. Delays in definitive diagnosis and treatment can not only lead to irreversible vision loss but also impact the patient's prognosis regarding survival. We report the case of a 26-year-old male patient who presented with impaired vision in the right eye, which had persisted for six months; neuroretinitis was diagnosed. Although infectious neuroretinitis was suspected, tests for pathogens such as Mycobacterium tuberculosis, Toxoplasma, Toxocara, and Bartonell

[Research progress of artificial intelligence in the diagnosis and treatment of Fuchs endothelial corneal dystrophy].

Fuchs endothelial corneal dystrophy is an inherited corneal endothelial disease that primarily affects middle-aged and elderly individuals. Due to the absence of obvious clinical symptoms in its early stages, it is often missed in diagnosis. If such patients are not properly diagnosed prior to cataract or other intraocular surgeries, postoperative corneal edema can easily lead to medical disputes. Therefore, early diagnosis and grading are crucial for clarifying patients' corneal status, reducing the risks of intraocular surgeries, and improving prognosis through early intervention. With advan

Angiotensin (1-7)-based Proteomimetic Polymers as Topicals for the Remediation and Repair of the Cornea following Injury.

The cornea is the outermost layer of the eye, most vulnerable to injury including from chemical exposure, either environmental, industrial or chemical weapons. For example, exposure of the eye to sulfur mustard (SM) can cause mustard gas keratopathy, resulting in devastating consequences for vision. Available treatments for corneal mustard keratopathy are either ineffective, or if used for a prolonged time, have deleterious side effects. Here, we aimed to harness the anti-inflammatory and anti-angiogenic peptide angiotensin-(1-7), known as Ang-(1-7), that is derived from the renin-angiotensin

Probable vestibular paroxysmia mimicking benign paroxysmal positional vertigo of the superior semicircular canal in the presence of a neurovascular conflict: a case report and review of the literature.

BACKGROUND: Positional vertigo is a frequent complaint in otology and vestibular physiotherapy, arising from functional or structural causes affecting the peripheral or central vestibular systems. Although benign paroxysmal positional vertigo (BPPV) is the most common form, other etiologies such as vestibular paroxysmia (VP), central positional vertigo, or vertebrobasilar insufficiency should be considered. Recognizing the specific features and differential diagnosis of these conditions is crucial for accurate assessment. CASE PRESENTATION: We describe the case of a 52-year-old French woman p

Systemic Diseases and the Eye: A Translational Perspective on Diabetes.

Diabetes mellitus is a global chronic disease that affects the whole body and not just glucose metabolism. In diabetes, non-invasive ocular changes can help understand microvascular, neurodegenerative, and inflammatory processes. This analysis looks at the eye as a biomarker platform for diabetes-related disease, with retinal imaging informing systemic pathophysiology and the emergence of AI-assisted approaches. With improvements in optical coherence tomography (OCT) and OCT angiography, it is possible to observe neurovascular changes that can occur before the clinical onset of diabetic retino

Prevalence of proprioception deficits in dogs visiting veterinary clinics for non-neurologic reasons: a cross-sectional study.

BACKGROUND: General proprioception deficits (GPD) detected during neurological exams aid in neurologic assessment and steer practitioners to consider neurological differential diagnoses. However, prevalence of GPD in without clinical signs of neurologic disease is limited. HYPOTHESIS/OBJECTIVES: That elderly dogs have GPD that might be undetected in dogs not presenting for neurologic concerns and to determine GPD prevalence in dogs presented to a veterinary teaching hospital. ANIMALS: The study included 277 client-owned dogs presenting to the Cornell University without neurologic complaints.

[Unilateral Lisch epithelial corneal dystrophy discovered after LASIK evaluation: a case report].

A 24-year-old woman was found to have a gray, intermittent, linear opacity in the supertemporal quadrant of the right corneal epithelium during preoperative evaluation for laser in situ keratomileusis (LASIK); all other examinations were within normal limits. After bilateral LASIK, both corneas healed well, and the linear opacity in the right eye had disappeared. More than 3 years postoperatively, a gray band-shaped opacity was detected in the right cornea, and a diagnosis of Lisch epithelial corneal dystrophy was established. Five months after diagnosis, the corneal opacity had significantly

Sjögren's disease and depression.

Depression is a major and under-recognized comorbidity in Sjögren's disease, contributing to disease burden and impaired quality of life. This narrative review summarizes current evidence regarding the prevalence, pathophysiological mechanisms, clinical impact, assessment, and treatment of depression in patients with Sjögren's disease. Reported prevalence rates range widely from 3.2% to 78.6%, largely owing to methodological heterogeneity, although meta-analytic data demonstrate a significantly increased risk of depression in patients with Sjögren's disease than in healthy controls. Etiology i

Morphological subtypes of Chiari malformation type I and clinical outcomes after posterior fossa decompression.

OBJECTIVE: Chiari malformation type I (CM-I) is the most common structural hindbrain defect in humans and causes debilitating headaches and other neurological symptoms frequently requiring surgery. Although patients' brains have heterogeneous shapes, diagnosis is based on a single radiographic measure with only 1 available pathology-directed treatment. The authors sought to determine whether there are distinct morphological subtypes of CM-I and to determine whether patients with different subtypes differ in clinical response to neurosurgical decompression. METHODS: For this cohort study, 186

Self-immolative hydrogen sulfide and ciprofloxacin co-delivery nanosheets for enhanced treatment of bacterial keratitis.

Pseudomonas aeruginosa (PA)-induced bacterial keratitis represents one of the most severe corneal infections usually accompanied by prominent inflammatory infiltration. Clinical fluoroquinolone-based therapies yield unfavorable therapeutic outcomes due to the insufficient ocular bioavailability and compromised anti-inflammatory efficacy. In the present work, we report a molecularly engineering strategy to conjugate ciprofloxacin (Cip) with a self-immolative carbonyl sulfide (COS)/hydrogen sulfide (H2S) donating motif. The resulting prodrug (termed ASCip) could spontaneously self-assemble into

Triple immunotherapy containing telitacicept for systemic lupus erythematosus complicated by multiple autoimmune diseases: a case report.

SLE may occur simultaneously with or subsequently to other autoimmune diseases; such cases are called overlap syndromes. Cases of SLE overlapping with three or more autoimmune diseases are extremely rare, and there is no unified standard or consensus for treatment. We report a clinical case of a patient with SLE, immune-mediated necrotizing myopathy, Sjögren's syndrome, and psoriasis. After treatment with a triple immunosuppressive regimen containing telitacicept, the patient achieved a satisfactory therapeutic response. As the first dual-pathway inhibitor targeting BLyS/APRIL, telitacicept de

Bilateral cranial polyneuropathy associated with an intracranial mass in a cat with concurrent tonsillar lymphoma.

A 14-year-old spayed female Siamese cat was presented with signs consistent with a rapidly progressive cranial polyneuropathy. Magnetic resonance imaging of the brain revealed an extra-axial mass at the calvarial floor, extending caudally to involve bilateral trigeminal nerves and rostrally to involve the pituitary gland and optic chiasm. Additional findings included contrast-enhancing content within the bilateral tympanic bullae, mild mandibular and medial retropharyngeal lymphadenopathy, and a left tonsillar mass. Cytologic and histopathologic findings confirmed tonsillar lymphoma. The concu

The growing burden of myopia in Europe: evidence from population-based cohorts.

Myopia is a childhood-onset condition driven by excessive axial ocular elongation, associated with increased risk of sight-threatening complications and irreversible visual impairment. Despite rising global prevalence, epidemiological data on long-term trends in Europe remain limited. This study examines temporal trends in myopia prevalence and axial length (AL) across European adult birth cohorts. We conducted a comparative birth-cohort analysis using harmonised data from 28 population-based studies (n = 81,828) across nine countries participating in the European Eye Epidemiology (E3) Consort
