Community awareness, symptom recognition, and perceived causes of skin neglected tropical diseases in selected regions of Ghana.

BACKGROUND: Leprosy and other skin neglected tropical diseases, including Buruli ulcer and yaws, remain significant public health challenges in Ghana. Despite control efforts, these conditions continue to cause disability, stigma, and social exclusion, particularly in rural and underserved communities. Misconceptions rooted in cultural and spiritual beliefs further delay care-seeking and reinforce discrimination. OBJECTIVE: This study assessed community awareness, symptom recognition, and perceived causes of leprosy and other skin NTDs across four regions of Ghana, and examined how these vari

Yaws: a review of clinical features, diagnosis and treatment.

Yaws, a neglected tropical disease caused by Treponema pallidum subsp. pertenue, primarily affects children in impoverished rural areas. It is spread through direct skin contact. The disease progresses through clinical stages. In the primary stage, patients develop a papilloma or ulcer, usually painless, often on the lower extremities, that is highly contagious. If untreated, this lesion heals spontaneously but may progress to secondary yaws, characterized by disseminated skin lesions, scaly papules and painful hyperkeratotic plaques on the palms and soles. Tertiary cases occur in 10% of untre

The etiology of exudative cutaneous ulcers in a yaws-endemic community prior to receipt of antimicrobials is similar to that found in communities after mass treatment with azithromycin.

Mass drug administration (MDA) of azithromycin (AZ) and case finding and treatment of children with cutaneous ulcers (CUs) have been trialed as strategies to eliminate yaws. Metagenomic sequencing of CU swabs obtained from children 3 to 4 years after the initiation of a yaws elimination campaign on Lihir Island showed enrichment for Treponema pallidum subsp. pertenue (TPE), Haemophilus ducreyi (HD), Streptococcus pyogenes (SP), and several anaerobic bacteria. Whether these results were influenced by AZ pressure or are generalizable to other populations is unknown. Here, we performed quantitati

Integrated serosurveillance to assess disease elimination in coastal Ecuador: onchocerciasis, yaws, and trachoma.

BACKGROUND: Testing blood samples with multiplex bead assays can assess elimination and identify residual foci of transmission for multiple pathogens simultaneously. In Ecuador, onchocerciasis and yaws are presumed eliminated, and the status of trachoma is unknown. We assessed their elimination status by measuring IgG antibodies in children 6-24 months. METHODS: In a birth cohort of 404 children measured between 2021-2024 in Esmeraldas province, Ecuador, we tested dried blood spots at ages 6, 9, 12, 18, and 24 months using a multiplex IgG assay that included antigens to Onchocerca volvulus (O

External quality assessment for yaws elimination in low- and middle-income countries using plasmid-based proficiency test items.

BACKGROUND: We aimed to establish an external quality assessment (EQA) programme for the yaws eradication campaign that would meet the needs of reference and district-level laboratories in low- and middle-income countries. METHODOLOGY/PRINCIPAL FINDINGS: We designed proficiency testing items (PTIs) using a plasmid containing gene target sequences for Treponema pallidum (TP) and Haemophilus ducreyi (HD). The storage stability of the plasmids under different environmental conditions was then tested. A proficiency testing panel of seven swabs loaded with different concentrations of plasmids in d

Integrated mass drug administration for yaws eradication: evidence from a comparative observational study in Papua New Guinea and a systematic review with network meta-analysis.

BACKGROUND: Yaws is a chronic infectious disease that disproportionately affects under-reached communities. In 2023, Papua New Guinea (PNG) reported 100 165 suspected yaws cases, accounting for 45% of cases reported worldwide. The WHO targets yaws eradication by 2030, primarily through mass drug administration (MDA) of azithromycin. This study evaluates the impact of an integrated MDA programme in PNG and reviews global evidence on optimal MDA strategies through a network meta-analysis (NMA). METHODS: A comparative observational study was undertaken analysing outpatient department (OPD) atten

Strengthening frontline capacity for skin neglected tropical diseases: Findings from a global survey.

BACKGROUND: Skin-related neglected tropical diseases (skin NTDs) remain a major source of morbidity in resource-limited settings. These conditions include Buruli ulcer, chromoblastomycosis and other deep mycoses, cutaneous leishmaniasis, post-kala-azar dermal leishmaniasis, leprosy, lymphatic filariasis, mycetoma, noma, onchocerciasis, scabies, tungiasis, and yaws. Recent efforts have focused on developing training materials to help frontline health workers diagnose and treat skin NTDs. However, little is known about how these materials are perceived and utilized in endemic regions. We perform

Challenges to 'Last Mile' Surveillance: Result of Programmatic Review of Integrated Skin NTDs Surveillance in Three Indonesian Districts.

Indonesia is approaching the 'last mile' of elimination for several skin-related neglected tropical diseases (skin NTDs): notably, leprosy, yaws and lymphatic filariasis (LF). However, persistent transmission in selected districts highlights systemic weaknesses in surveillance. This paper aimed to analyse the health system, operational and sociocultural barriers to integrated skin NTDs surveillance in Indonesia. A descriptive analysis of the national programmatic review of integrated skin NTDs was conducted in 2024, using a mixed-methods descriptive evaluation based on routine data and themati

Investigating a Skin Disease in Guinea Baboons (Papio papio) Using Non-invasive Methods.

Infectious skin diseases in nonhuman primates are of importance due to their zoonotic transmission potential, especially in biodiversity rich areas where human-wildlife interactions are frequent. Here, we report the presence of a skin disease in unhabituated wild Guinea baboons (Papio papio) inhabiting an agroforest landscape in Cantanhez National Park, Guinea-Bissau, exclusively using non-invasive surveillance methods. We combined 107 georeferenced baboon presence points, camera trap footage obtained during 2224 camera trap days, with molecular analysis of 272 biological samples collected opp

Molecular and serological evidence of systemic Treponema pallidum infection in wild western chimpanzees.

Human treponematoses are chronic diseases caused by closely related Treponema species. Increasing evidence of yaws-like Treponema pallidum subsp. pertenue infections in nonhuman primates raises concerns about pathogen persistence in natural ecosystems. Here, we investigated treponemal infection in western chimpanzees (Pan troglodytes verus) using molecular and serological analyses of postmortem skeletal remains from three deceased individuals. Anti-T. pallidum antibodies were detected in all chimpanzees using validated serological assays, while molecular assays identified T. pallidum DNA in a

Mental health in skin neglected tropical diseases.

Neglected tropical skin diseases (SNTDs) include at least nine conditions affecting more than one billion people worldwide. While their physical manifestations have been studied in detail, the mental health consequences are highly under-researched and underreported, often limited to specific conditions. This article summarizes existing literature on mental health aspects of individual SNTDs ranging from depression, anxiety, stigma, social isolation, post-traumatic stress disorder, epilepsy, and developmental impacts on growing children. While evidence is more for leprosy, lymphatic filariasis,

Effect of repeated mass drug administration on the transmission of yaws: a retrospective genomic epidemiology study.

BACKGROUND: Yaws, a neglected tropical disease caused by Treponema pallidum subspecies pertenue (T p pertenue), has evaded eradication, in part due to a high proportion of asymptomatic cases. Repeated mass drug administration (MDA), whereby an entire population is repeatedly treated irrespective of disease, could provide a solution. Here, we aimed to investigate the effect of MDA on the genomic epidemiology of T p pertenue. METHODS: We conducted a retrospective genomic epidemiology study on samples collected during a cluster-randomised trial of mass administration of azithromycin for yaws era

Assessment of the role of Haemophilus ducreyi coinfection on outcomes of Yaws treatment in the southwestern part of Ghana.

BACKGROUND: Yaws, caused by Treponema pallidum subsp. pertenue (TP) is targeted for eradication by 2030 under the World Health Organization's (WHO) initiative, which relies on mass drug administration (MDA) of azithromycin. Evidence of persistent lesions after treatment has been reported. While the occurrences of Haemophilus ducreyi (HD) in yaws-like lesions has been widely documented, there is limited evidence regarding its influence on treatment outcomes assessed at the WHO-recommended 4-week follow-up. In this study, we sought to detect the occurrence of H. ducreyi in yaws-confirmed cases a

A 5500-year-old Treponema pallidum genome from Sabana de Bogotá, Colombia.

Treponematosis, a bacterial infection caused by Treponema pallidum subspecies and T. carateum (yaws, bejel, syphilis, pinta), has afflicted humans for millennia. Despite paleopathological evidence and emerging genomic data, little is known about the evolutionary history of these pathogens. We report a 5500-year-old Treponema genome (TE1-3) from Middle Holocene hunter-gatherer contexts of the rock shelter Tequendama I in Colombia. Our analyses place TE1-3 as a sister lineage to all known T. pallidum subspecies, positioning this pathogen in the Americas millennia before European contact and befo

A new highly discriminatory typing scheme for Treponema pallidum reveals similar levels of genetic variability across lineages.

UNLABELLED: The global resurgence of treponematoses, particularly syphilis, poses a growing public health challenge. Despite advances in sequencing technologies, obtaining complete Treponema pallidum genome sequences for epidemiological studies remains challenging due to clinical sampling and methodological constraints. There is, therefore, a need for rapid, cost-effective, and accessible typing methods. Based on the analysis of 121 T. pallidum genomes spanning all three subspecies (TPA, TPE, and TEN) from diverse regions, we selected seven highly variable genes (tp0136, tp0326, tp0548, tp0705

Yaws active case detection surveys in 15 districts of Cote d'Ivoire.

BACKGROUND: Yaws is one of the skin-related neglected tropical disease (skin NTD). It is closely linked to poverty and occurs mainly in poor communities in hot, humid, tropical regions. Children in disadvantaged communities in the tropics are most affected. Following the recommendations of the WHO NTD Road map 2021-2030, Côte d'Ivoire planned an evaluation (clinical and serological investigation) to assess the true extent of yaws in the country. Côte d'Ivoire is one of the nine countries currently endemic for yaws in the WHO African Region. The aim of this investigation was to establish the en

A Look Back at Mercury Soap as a Treatment for Yaws.

The centuries-old practice of using mercury as a treatment for syphilis is well-known. The historical use of mercury for other treponematoses is less well-recognized. Afridol was a mercury containing soap produced by Bayer which they promoted as a cure for various cutaneous aliments, including yaws (frambosia). In this article, I briefly outline the development of Afridol and its subsequent marketing with the use of early 20th century medical advertising art.

Formal description of Treponema pallidum subsp. pallidum, comb. nov., Treponema pallidum subsp. pertenue, comb. nov., Treponema pallidum subsp. endemicum, subsp. nov. and emended description of Treponema pallidum.

This article represents the valid publication of the names of three subspecies of Treponema pallidum. These subspecies were first described by Robert M. Smibert in 1984 and correspond with organisms that cause syphilis (subsp. pallidum), yaws (subsp. pertenue) and bejel (subsp. endemicum) in humans. The subspecies nomenclature has been widely used in the literature since that time but has not been validly published due in part to the availability of only limited genetic information. The designation of these subspecies is now supported by a large number of genomic sequences indicating a clear s

Investigation of bacterial agents in yaws-like ulcers in children and their environment in Cameroon: An experience from the Akonolinga Health District.

BACKGROUND: Skin ulcers are health problem in children in tropical areas. Some clinically resemble yaws and this may cause inappropriate allocation of resources from the WHO yaws eradication plan. Studies have shown that bacteria other than the Treponema pallidum subspecies pertenue (bacterium causing yaws) were responsible for these lesions with certain associated risk factors. This study aimed to detect bacteria in yaws-like skin ulcers in Akonolinga Health District and the involvement of some matrices (pets, water, flies). METHODS: A cross-sectional study was conducted from April to June 2
