Multisystem sarcoidosis is uncommon and presentation as a stroke mimic with severe renal impairment and concurrent haemolysis has not previously been described. Evidence for the use of plasma exchange (PLEX) in sarcoidosis is limited. This case describes a patient with multisystem sarcoidosis who presented with encephalopathy, acute tubulointerstitial nephritis, optic neuritis and autoimmune haemolytic anaemia. Extensive immunological investigations were non-diagnostic. The patient's neurological and haematological manifestations improved rapidly with intravenous methylprednisolone, intravenou

PURPOSE: To investigate the origin of P100 double peaks occasionally observed in patients with optic neuritis by experimentally inducing timing asynchronies between visual field locations and modulating stimulus strength via contrast reduction in transient and steady-state VEPs (trVEP and ssVEP).
METHODS: The trVEPs and ssVEPs were recorded using a checkerboard stimulus vertically split into nasal and temporal half-fields. Timing asynchrony was induced by delaying the temporal half-field by 24, 35, or 47 ms; stimulus strength was modulated by reducing the temporal half-field contrast to 20% o

Optic neuritis (ON) is a neuroinflammatory autoimmune disease harboring autoreactive lymphocytes. Effector memory CD4+ T cells (CD4+ Tem) represent a prominently expanded and pro-inflammatory subset within this compartment, yet their pathogenic metabolic programs remain poorly defined. B cell dysfunction also contributes to ON pathogenesis. However, whether T cell-intrinsic metabolic rewiring directly fuels this B cell dysregulation awaits elucidation. Here, through single-cell transcriptomic profiling of peripheral blood mononuclear cells (PBMCs) from ON patients, we conceptualized a patholog

INTRODUCTION: Vogt-Koyanagi-Harada (VKH) syndrome is an idiopathic immune-related disorder that affects multiple organ systems, particularly targeting melanocytes in the uvea, ear, and meninges. It can sometimes present as subacute vision loss and or headache without other systemic features mimicking optic neuritis or Idiopathic intracranial hypertension, leading to an initial misdiagnosis.
CASE REPORT: A 36-year-old male presented with an acute onset and progressive diminution of vision in both eyes for 5 days. Fundus examination revealed papillitis. Brain MRI and CSF examination were normal

A woman in her 40 s presented with recurrent retro-orbital pain over several years, initially attributed to migraine. Later, she was diagnosed with glaucoma in the context of suspected intermittent angle closure. Despite controlled intraocular pressure (IOP) with topical pressure-lowering therapy, progressive visual loss persisted.Optical coherence tomography showed diffuse retinal nerve fibre layer (RNFL) thinning, and visual fields demonstrated severe defects. However, pronounced optic disc pallor, rapid progression despite IOP control and diffuse rather than sectorial RNFL loss raised suspi

INTRODUCTION: Myelin oligodendrocyte glycoprotein antibody-positive optic neuritis (MOG-ON) is an autoimmune inflammatory demyelinating disorder of the optic nerve that often results in severe visual impairment and recurrent attacks. Although immune dysregulation is recognized as a central feature of MOG-ON, the contribution of post-transcriptional regulatory mechanisms, particularly adenosine-to-inosine (A-to-I) RNA editing, to disease pathogenesis and visual outcomes remains poorly understood. In this study, we characterized A-to-I RNA editing in MOG-ON, investigated molecular signatures and

The involvement of both the central and peripheral nervous systems in Mycoplasma pneumoniae infection is rare. We report a patient who developed bilateral optic neuritis and Guillain-Barré syndrome (GBS) following a Mycoplasma pneumoniae infection. A 32-year-old man presented with progressive weakness, ataxia, and dysphagia after a recent respiratory infection. Clinical, cerebrospinal fluid, and neurophysiological findings were consistent with GBS, and treatment with intravenous immunoglobulin was initiated. Two days later, the patient developed visual loss, and post-infectious autoimmune opti

Systemic lupus erythematosus (SLE) may overlap with Sjögren's syndrome (SS), leading to complex systemic involvement, including neurological and pulmonary manifestations. We report a 60-year-old woman with a 10-year history of SLE-SS overlap presenting with headaches and bilateral visual impairment. Hydroxychloroquine (HCQ) had been discontinued 4 months prior due to HCQ-related optic neuropathy confirmed on optical coherence tomography (OCT). Ophthalmologic evaluation revealed bilateral optic neuritis. Immunological testing showed high-titre antinuclear antibodies (ANAs) in both serum (1:640)

PURPOSE: To characterise the spectrum of clinical presentations of Bartonella-associated uveitis across multiple tertiary uveitis centres and assess the frequency of non-neuroretinitis phenotypes.
METHODS: Multi-centre retrospective case series of patients with serologically confirmed Bartonella infection and associated intraocular inflammation. Clinical phenotypes were classified as classical (isolated neuroretinitis) or non-neuroretinitis based on anatomic and multimodal imaging findings, and their relative frequencies were analysed.
RESULTS: Seventy eyes of 53 patients were included. Medi

A 23-year-old male patient presented to the emergency department with several days of exacerbating headaches, bilateral retrobulbar pain, and blurred vision. Ophthalmologic evaluation was consistent with bilateral optic neuritis (ON). MRI, CSF analysis, and blood tests were normal except for elevated liver enzymes and elevated cytomegalovirus-immunoglobulin G titers. Prompt treatment with intravenous methylprednisolone for acute ON was initiated, despite the concern for an acute systemic viral infection, resulting in rapid improvement in vision. This case report highlights the key differentiat

Background/Objectives: To compare the lesion detection rates of fluid-attenuated inversion recovery with fat saturation (FLAIR-FS) versus T2-weighted short tau inversion recovery (T2WI-STIR) and contrast-enhanced T1-weighted imaging (CE-T1WI) in detecting lesions across antibody-defined subtypes (AQP4-ON, MOG-ON, seronegative ON) and anatomic segments of optic neuritis (ON). Methods: Forty-two patients (56 eyes) aged 5-57 years with acute/subacute optic neuritis underwent orbital MRI (T2WI-STIR, FLAIR-FS, and CE-T1WI). Two blinded neuroradiologists independently reviewed the images to assess l

This study reports a case of MOG antibody-associated optic neuritis masquerading as acute angle-closure glaucoma in which marked discordance between optic disc cupping and visual field loss prompted magnetic resonance imaging (MRI) and led to the correct diagnosis. A 69-year-old woman presented with acute ocular pain, decreased vision, and intraocular pressure exceeding 40 mmHg in the right eye. Humphrey visual field testing demonstrated nearly complete central visual field loss, whereas fundus examination revealed only mild optic disc cupping. Because the severity of visual field loss was dis

Concurrent headache and visual symptoms pose a diagnostic challenge across specialties, ranging from benign to life-threatening etiologies. Overlapping manifestations and terminological confusion often lead to misdiagnosis and delayed care. This narrative review searched eight databases from inception to May 5th, 2026, for articles on headache and visual symptoms. Extracted data were synthesized into a structured diagnostic framework. Key benchmarks were clarified across seven categories. Retinal migraine and migraine with aura are distinguished by monocular/binocular involvement, duration, an

BACKGROUND: Leber's hereditary optic neuropathy (LHON) is a mitochondrial disorder typically presenting with subacute, painless, bilateral vision loss in young individuals. While often recognizable, atypical presentations can complicate diagnosis. Notably, optic nerve lesions on MRI are uncommon in LHON and are typically associated with inflammatory optic neuropathies, increasing the risk of misdiagnosis.
METHODS: We retrospectively analyzed four patients who initially presented with suspected optic neuritis but were later diagnosed with LHON. Demographic, genetic, clinical, and multimodal da

Sarcoidosis is a diagnostically challenging and complex multisystem disease characterised by the formation of non-caseating granulomas. This case describes a young man who developed bitemporal vision loss that rapidly progressed to complete bilateral vision loss. MRI demonstrated chiasmatic and optic neuritis. A history of pulmonary nodules was noted. Via multi-disciplinary team discussion, no safe biopsy targets were identified. A clinical diagnosis of neurosarcoidosis was established. The patient's symptoms responded well to high-dose steroids and plasma exchange. Chiasmitis and optic neurit

BACKGROUND: Monoclonal gammopathy of undetermined significance (MGUS), which can be asymptomatic, is a premalignant proliferative disorder of plasma cells. The peripheral nervous system may be involved in MGUS, causing a monoclonal gammopathy associated peripheral neuropathy. However, involvement of the central nervous system (CNS) in MGUS was rarely reported.
CASE PRESENTATION: Here we report a rare case of recurrent optic neuritis, as the first clinical presentation of MGUS, in an otherwise healthy 54-year-old Iranian male patient. He was treated with intravenous high-dose methylprednisolon

Neuromyelitis optica spectrum disorder (NMOSD) is an autoimmune disease of the central nervous system, characterized by severe attacks. Optic neuritis is the most common clinical manifestation, leading to a rapid and often severe loss of vision, which is sometimes bilateral, with frequent sequelae and a risk of very low visual acuity in the long-term. Magnetic resonance imaging (MRI) reveals extensive lesions of the optic nerve, mostly involving the posterior part of the nerve and sometimes extending to the optic chiasm. Myelitis manifests as longitudinally extensive and centrally located spin
