Serum NfL predicts visual recovery and neuronal loss after optic neuritis.

Optic neuritis, a common manifestation of multiple sclerosis, involves acute axonal injury and variable visual recovery. We evaluated whether serum neurofilament light chain predicts visual and structural outcomes after first-episode optic neuritis. Fifty-one patients (mean age = 33 years, 71% women) were followed for 24 ± 6 months with low-contrast letter acuity testing and optical coherence tomography. Adjusting for age and sex, higher baseline serum neurofilament light chain predicted greater inter-eye differences in retinal nerve fibre layer (-4.65 µm per SD, p = 0.02), ganglion cell/inner

Effects of Pre-MRI Multimodal Psychosocial Nursing Intervention on Anxiety and Examination Compliance in Patients With Optic Neuritis.

AIMS/BACKGROUND: Magnetic resonance imaging (MRI) is essential for diagnosing and monitoring optic neuritis. However, the procedure itself may induce significant anxiety in patients, potentially compromising examination compliance and image quality. This study retrospectively evaluated the effects of a structured, multimodal pre-MRI psychosocial nursing intervention on anxiety and compliance in patients with optic neuritis. METHODS: A total of 106 patients diagnosed with optic neuritis who underwent MRI at Jiaozhou Central Hospital of Qingdao between June 2021 and June 2024 were included. Pat

Synthetic MRI for Detecting Abnormal Signals in the Optic Nerves: An Exploratory Study.

OBJECTIVES: Synthetic magnetic resonance imaging (MRI) is a quantitative imaging technique that has shown promise in brain imaging but has not yet been evaluated for assessing the optic nerves. Our study aimed to investigate its diagnostic performance in this context. MATERIALS AND METHODS: We retrospectively evaluated synthetic MRI's performance in detecting optic nerve hypersignals in 65 patients who underwent synthetic MRI covering the optic nerves from March 2023 to February 2025 in a single tertiary center. Diagnostic performance for optic nerve hypersignals was assessed using convention

Immune checkpoint inhibitor-associated bilateral optic neuritis.

Optic neuritis is a rare immune-related adverse event of immune checkpoint inhibitor use and may result in poor visual outcomes. We describe an elderly woman with metastatic malignant melanoma treated with nivolumab, a programmed cell death protein 1 immune checkpoint inhibitor. After 2 months, she developed bilateral painless loss of vision due to optic neuritis. Although there is limited evidence, expert opinion favours early corticosteroid use and/or plasma exchange, as recommended for neuromyelitis optica spectrum disorders. Optic neuritis is increasingly recognised as an immune-related ad

Building the cancer research workforce through implementation of the RACE21 undergraduate cancer research and education program.

BACKGROUND: Cancer remains the second leading cause of death in the state of Alabama and disproportionately affects communities that are rural, of lower socioeconomic status, and individuals with high-risk, predisposing conditions such as obesity, cardiovascular disease, advanced age, and diabetes. One method to address the increased burden of cancer is to expand the cancer research and clinical workforce. The Roadmap for America's Cancer Explorers for the 21st Century (RACE21) is a National Cancer Institute-funded Youth Enjoy Science program at the University of Alabama at Birmingham (UAB) an

Myelin oligodendrocyte glycoprotein-antibody disease (MOGAD) with leukodystrophy-like presentation.

Myelin oligodendrocyte glycoprotein-antibody disease (MOGAD) is an immune-mediated demyelinating disorder, distinct from MS, which typically affects the optic nerve, spinal cord, brain and/or brainstem. Diffuse white matter involvement that resembles a leukodystrophy has been reported only in children. We present a woman aged 29 years with new right optic neuritis on a background of previously unexplained, longstanding, bilateral, reduced visual acuity. MR scan of the brain and orbits showed confluent, bilateral, posterior predominant white matter changes reminiscent of a leukodystrophy, along

Eculizumab as a rescue therapy in acute-phase aquaporin-4 (AQP4) antibody-positive neuromyelitis optica spectrum disorder.

Eculizumab as C5 complement protein inhibitor has been approved for the maintenance treatment of neuromyelitis optica spectrum disorder (NMOSD). However, evidence regarding its efficacy in the acute phase remain limited. This study enrolled six patients with aquaporin-4 (AQP4)-IgG-seropositive neuromyelitis optica, including five cases of optic neuritis and one case of long-segment transverse myelitis. These patients exhibited no or partial symptomatic improvement following glucocorticoid pulse therapy, and two patient experienced further deterioration. Subsequently, eculizumab was added to th

[How I explore…optic neuritis].

Optic neuritis is an inflammation of the optic nerve most often caused by autoimmune or systemic disease. The diagnostic management is transdisciplinary. The clinical approach should be global first and then system-oriented. It determines the choice of additional investigations whose purpose is the characterization of the type and the cause of the optic neuritis. The exploration initially involves neuroimaging techniques, visual electrophysiology and biological analyses. Optical coherence tomography is of considerable interest in specifying the differential diagnosis. Diagnostic accuracy influ

Cerebellar tumefactive demyelination in MOGAD: a case report on diagnostic challenges and immunotherapeutic strategy.

Tumefactive demyelination is a rare phenotypic subtype of myelin oligodendrocyte glycoprotein (MOG) antibody-associated disease (MOGAD) and poses significant diagnostic challenges due to substantial clinical and radiological overlap with intracranial neoplasms and other demyelinating conditions. This mimicry frequently leads to misdiagnosis and subsequent inappropriate therapeutic interventions and delay in treatment, thereby increasing the risk of adverse clinical outcomes. We present the case of a 31-year-old male with a prior history of MOG-associated optic neuritis (ON) who developed acute

Thinking Outside of the MOGAD Box: When Does CNS-Hemophagocytic Lymphohistiocytosis Make More Sense?

Neuroinflammatory syndromes in children often present with overlapping clinical, radiological and immunological features, posing significant diagnostic challenges. We describe two pediatric patients with acute neuroinflammatory presentations who fulfilled criteria for a well-recognized antibody-associated demyelinating disorder but also demonstrated clinical and laboratory features suggestive of a broader hyperinflammatory process. These cases underscore the diagnostic complexity when systemic and central nervous system inflammation coexist and highlight the importance of maintaining a broad d

Neuromyelitis Optica Spectrum Disorder Initially Presenting With Meningitis-Like Symptoms: A Case Report.

We report a case of neuromyelitis optica spectrum disorder (NMOSD) relapse in a 45-year-old woman with a known 11-year history of NMOSD, who initially presented with meningitis-like symptoms, including fever, headache, and neck stiffness. Physical examination revealed neck stiffness and positive jolt accentuation, without any other neurological abnormalities. Cerebrospinal fluid (CSF) analysis revealed mononuclear-predominant pleocytosis and hypoglycorrhachia, raising concerns of meningitis. On hospital day 2, brain magnetic resonance imaging (MRI) revealed new hyperintense lesions in the righ

Neuroinflammatory regulatory role of microglia in optic nerve injury: from pathological mechanisms to therapeutic targets.

Optic nerve injury, encompassing conditions such as glaucoma, optic neuritis, and traumatic optic neuropathy, is a major cause of irreversible vision loss. Traditional broad-spectrum anti-inflammatory treatments have shown limited efficacy, highlighting the need for precision-based therapeutic approaches grounded in the underlying pathological mechanisms. As the primary immune cells of the central nervous system (CNS), microglia play a crucial role in regulating neuroinflammation following optic nerve injury. This review provides a comprehensive overview of the mechanisms governing microglial
