Elevated CD45+CD11c+CX3CR1+ Cells in Optic Nerves and Their Relation to Optic Nerve Function in an Experimental Autoimmune Encephalomyelitis Model.

Dendritic cells are antigen-presenting cells that play essential roles in immunity and initiate immune reactions in experimental autoimmune encephalomyelitis (EAE) models. Dendritic cell recruitment was quantified in the optic nerve and retina after inducing the EAE model, and the correlation between functional and structural damage to the optic nerve and retina was determined. EAE was induced in female C57BL/6 mice by injecting murine myelin oligodendrocyte glycoprotein 35-55. All 20 mice were sacrificed sequentially. The dendritic cell infiltration level was analyzed using flow cytometry and

Optic nerve sheath meningioma and optic neuropathy: A clinical and diagnosis crossroad.

Optic nerve sheath meningiomas (ONSMs) are rare, benign tumors of the central nervous system that arise from arachnoid cap cells. They most commonly affect middle-aged women and account for approximately 2% of orbital tumors. ONSMs typically present with slowly progressive, painless, ipsilateral optic neuropathy; however, atypical acute or subacute onset may mimic inflammatory optic neuritis. We describe the case of a 45-year-old woman who presented with a 1-month history of progressive visual blurring in the left eye, accompanied by retro-orbital pain and optic disc swelling. Initial clinical

The supporting role of Visual Evoked Potentials for the diagnosis of Optic Neuritis within the 2022 ICON criteria.

BACKGROUND: Visual evoked potentials (VEPs) are commonly used in the assessment of optic neuritis (ON) but were not included in the International Consortium for ON (ICON) 2022 diagnostic criteria as supporting tests. OBJECTIVES: We aimed to assess VEPs diagnostic performances and their addition to the ICON criteria in a large consecutive cohort of patients with suspected ON. METHODS: We screened 207 patients with suspected ON and included 71 who performed MRI, OCT, and VEPs within 30 days from symptoms onset. We calculated diagnostic performances for each test and for the ICON 2022 criteria

Neuro-Ophthalmic Complications of Immune Checkpoint Inhibitors.

Neuro-ophthalmic complications associated with immune checkpoint inhibitors (ICIs) are rare. These involve adverse events primarily affecting the optic nerve, other cranial nerves, and the neuromuscular junction. Myasthenia gravis-like syndrome (often with concomitant myositis) is the most commonly reported immune-related adverse event, followed by optic neuropathy, of which the most notable is optic neuritis. Symptoms typically present within 6 months of exposure. Prompt and comprehensive evaluation is essential. Diagnostic work-up may include MRI brain and orbits, lumbar puncture, serum, and

Successful treatment with efgartigimod as rescue therapy in acute attack of MOGAD: a case report.

BACKGROUND: Myelin oligodendrocyte glycoprotein (MOG) antibody-associated disease (MOGAD) is an autoimmune inflammatory demyelinating disease typically characterized by recurrent myelitis and optic neuritis. Its pathogenesis is primarily mediated by MOG-immunoglobulin G (IgG), a pathogenic autoantibody targeting central nervous system myelin. Efgartigimod, a human IgG antibody Fc fragment and natural ligand for the neonatal Fc receptor, promotes degeneration of circulating pathogenic IgG, potentially positioning it as a novel therapeutic option for MOGAD. Case presentation CASE PRESENTATION: W

[18F]-FDG-PET/CT detects subclinical optic nerve inflammation in giant cell arteritis.

OBJECTIVE: Vision loss is a feared complication in giant cell arteritis (GCA). However, imaging data characterizing intraorbital inflammation and its potential predictive value for ischemic ocular complications remain limited. Such information might be helpful in clinical phenotyping and risk stratification of patients with GCA. METHODS: This is a retrospective cohort study of patients with GCA, who received at least two [18F]-FDG-PET/CTs. Quantitative assessment included measurement of SUVmax, SUVmean, and the target-to-background ratio (TBR) of both optic nerves (ONs) in the canalicular seg

Myelin oligodendrocyte glycoprotein antibody-associated disease presenting as isolated simultaneous bilateral optic neuritis in a child.

Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is the most recently defined distinct demyelinating disorder of the central nervous system which has heterogeneous clinical manifestations. Here, we present the case of isolated bilateral optic neuritis (ON) in an 11-year-old female who presented with bilateral acute and progressive loss of vision with pain on ocular movement over the past 2 days following mild fever. Neuroimaging revealed the features suggestive of isolated ON more in favor of MOGAD. Spine imaging was within the normal limits. She was seronegative for aqu

Leber hereditary optic neuropathy triggered by the AstraZeneca coronavirus disease 2019 vaccination.

Leber hereditary optic neuropathy (LHON) is a rare mitochondrial disorder primarily affecting young males, characterized by progressive vision loss due to retinal ganglion cell degeneration. LHON is typically associated with specific mitochondrial mutations with potential triggers such as environmental factors and, more recently, postvaccination complications. We present a case of a 35-year-old male who experienced LHON onset 3 weeks following administration of the AstraZeneca Coronavirus disease 2019 vaccine. Despite lacking traditional risk factors, genetic testing confirmed the presence of

A novel optic nerve demyelination mouse model: Insights into secondary impairment of the visual cortical circuit.

Demyelinating optic neuritis (DON) results in significant visual impairment. In the central nervous system (CNS), neurons and neural circuitry spatially separate from the primary lesion site can be compromised in the way of trans-synaptic degeneration. This study aims to investigate the anterograde alterations in the primary visual cortex (V1) induced by DON-related optic nerve (ON) myelin loss and explore potential treatment approaches targeting underlying mechanisms. Here, we developed a novel mouse model of focal optic nerve demyelination (OND) by combined use of a conditional gene knockout

[Chinese expert consensus on the treatment of demyelinating optic neuritis with biologics (2026)].

Demyelinating optic neuritis (DON) is an autoimmune inflammatory demyelinating disease that affects the optic nerve. In recent years, with the rapid advancement of biotechnology, novel targeted biological agents have demonstrated unique advantages in the treatment of DON, and the number of approved targeted biologics for clinical use has gradually increased. However, the risks of infections and adverse effects caused by the long-term use of biologics cannot be ignored. How to use various biologics reasonably, effectively, and safely presents a new challenge that clinical physicians must addres

Bilateral optic neuritis and multiple nerve sheath tumors in a patient with genetically characterized Ehlers-Danlos syndrome: A rare co-occurrence.

Classical‑like Ehlers-Danlos syndrome (clEDS) due to TNXB variants is a hereditary connective tissue disorder that shares clinical features with classic EDS but differs genetically from the COL5A1/COL5A2‑associated cEDS subtype. The coexistence of inflammatory optic neuropathy and multiple nerve sheath tumors in this context is rarely reported. We describe a 26-year-old woman with genetically characterized clEDS (TNXB variant) who presented with subacute bilateral visual loss. Orbital magnetic resonance imaging (MRI) demonstrated bilateral optic nerve thickening and contrast enhancement consis
