INTRODUCTION: Neuromyelitis optica spectrum disorder (NMOSD) and MOG-associated disease (MOGAD) present high risk of disability. Early testing for anti-aquaporin-4 (AQP4) and anti-myelin oligodendrocyte glycoprotein (MOG) antibodies is crucial for timely diagnosis and better outcomes. However, patient awareness of these antibodies and their understanding about their disease remains poorly characterized.
OBJECTIVES/AIMS: To assess the frequency of antibody serostatus awareness and diagnosis recognition among patients with NMOSD and MOGAD in a tertiary outpatient clinic.
METHODS: In this cross

We report a 36-year-old healthy woman with 2 days of right-sided blurry vision, headache, and pain with eye movement. Exam showed decreased visual acuity, focal retinal pallor, and abnormal cranial nerve II findings. CTA revealed right vertebral artery dissection at C5-C6 with concurrent ophthalmic artery narrowing. She received anticoagulation, intravenous steroids for optic neuritis, and improved with follow-up. This rare presentation underscores the need to consider cervical artery dissection in young patients with painful vision loss and subtle fundoscopic changes, as timely vascular imagi

OBJECTIVE: Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is an immune-mediated demyelinating disorder of the central nervous system. It has a higher incidence in children than in adults, carries a relatively high risk of relapse with unclear mechanisms, and significantly impacts patient prognosis. This study aimed to investigate the clinical characteristics of MOGAD and identify independent risk factors for relapse, to provide a basis for early intervention and individualized treatment.
METHODS: A total of 108 children diagnosed with MOGAD at the Children's Hospital

The eye serves as an important "window" that sensitively reflects neurological lesions. This article reviews the basic anatomy of the eyeball and optic nerve, and explains simple methods for evaluating visual acuity, pupils, ocular movements, color vision, visual fields, and the fundus that can be performed by neurologists in outpatient clinics and at the bedside. It also focuses on optic neuritis, which is closely associated with neurological disorders, outlining examination methods and key clinical precautions that can be undertaken by neurologists, as well as major ophthalmic adverse effect

BACKGROUND: Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) has a broad spectrum of clinical phenotypes, including acute disseminated encephalomyelitis (ADEM), optic neuritis (ON), transverse myelitis (TM), and cerebral cortical encephalitis (CCE). Although these conditions exhibit diverse outcomes and require different treatment options for relapse prevention, no biomarkers have been identified to distinguish the clinical phenotypes. This study aimed to investigate the properties of cytokines and chemokines in the serum and cerebrospinal fluid (CSF) of patients with MO

BACKGROUND: The disease course of neuromyelitis optica spectrum disorder (NMOSD) presenting with isolated optic neuritis (ON) is heterogeneous. Predictive factors for progression to other core phenotypes or bilateral ON (BON) remain poorly defined.
OBJECTIVE: To delineate disease evolution patterns and identify risk factors for phenotypic conversion and BON in a long-term cohort of ON-onset NMOSD, with a specific focus on the impact of antinuclear antibody (ANA) and the timing of immunosuppressive therapy (IMT).
METHODS: This single-center retrospective cohort study enrolled 231 AQP4-IgG-ser

Mitochondrial complex III (CIII) deficiency, resulting from abnormalities in its subunits or assembly factors, presents with diverse clinical manifestations. LYRM7-associated CIII deficiency is rare and typically presents with progressive neurodegeneration. We report a case series of LYRM7-associated CIII deficiency in two brothers, highlighting inflammatory demyelinating-like presentations, intrafamilial variability, and atypical disease progression. We present an investigational case series highlighting continuing challenges in diagnosing and managing LYRM7-associated mitochondrial complex I

BACKGROUND: Neuromyelitis optica spectrum disorder (NMOSD) is an autoimmune astrocytopathy characterized by recurrent inflammatory attacks within the central nervous system. Beyond optic neuritis, diffuse retinal and cerebral alterations may occur. Optical coherence tomography angiography (OCTA) enables the non-invasive assessment of retinal microvasculature and may provide insight into longitudinal retinal changes and their association with cerebral white matter hyperintensities (WMHs).
METHODS: In this longitudinal observational study, 45 aquaporin-4 antibody-positive NMOSD patients and 45

BACKGROUND: The real-world performance of the 2022 international consensus criteria for optic neuritis (ICON) in antibody-predominant Chinese practice is uncertain. We evaluated diagnostic yield and an antibody-stratified adjunct for settings with high antibody prevalence and timely antibody testing.
METHODS: We retrospectively studied 151 consecutive patients admitted with acute optic neuritis between 2020 and 2023. All cases were reclassified using strict 2022 ICON. We then evaluated an assumed-dyschromatopsia scenario in which dyschromatopsia was assumed unless normal color vision was expl

Neuromyelitis optica spectrum disorder (NMOSD) is a rare autoimmune CNS disease that frequently causes severe optic neuritis, yet the molecular mechanisms driving retinal damage remain incompletely understood, especially across different NMOSD subgroups. Müller glial cells, which maintain retinal water-ion homeostasis through AQP4 and Kir4.1 channels, may represent a primary retinal target of circulating NMOSD-related autoantibodies and serum factors. We investigated how sera from AQP4-IgG+, MOG-IgG+, and double-seronegative (DSN) NMOSD patients affect the expression and localization of key Mü

Paediatric optic neuritis is a rare inflammatory demyelinating condition that frequently presents with bilateral involvement and a generally favourable prognosis compared to adult-onset forms. We report the case of a seven-year-old boy with a history of thalassemia trait and mild autism who presented with a one-day history of sudden bilateral painful vision fluctuation, headache, and fever. Initial visual acuity (VA) was 6/12 in the right eye and 6/30 in the left eye, with normal optic nerve function test and fundoscopic examination indicating retrobulbar involvement. While initial computed to

Autoimmune glial fibrillary acidic protein (GFAP) astrocytopathy is a recently identified autoimmune disorder of the central nervous system. It commonly manifests as meningitis, encephalitis, myelitis, or optic neuritis. However, presentations resembling stroke are exceedingly rare. Here, we report a case of autoimmune GFAP astrocytopathy initially misdiagnosed as acute ischemic stroke. The correct diagnosis was later established through the detection of GFAP antibodies in both the patient's serum and cerebrospinal fluid (CSF) using a cell-based assay (CBA). The patient initially responded to

PURPOSE: Optic neuritis (ON) associated with acute retinal necrosis (ARN) has traditionally been considered a complication resulting from direct viral infiltration of the optic nerve. Although it is currently treated with intravenous acyclovir, the appropriate dosage and duration of treatment in the pediatric population remain unclear, and no standardized effective treatment has been established in clinical guidelines.
METHODS: A 14-year-old boy was referred for panuveitis in the right eye (RE), with a visual acuity (VA) of hand motions. Fundus examination revealed intense vitritis, and PCR t
