BACKGROUND: Neuromyelitis optica (NMO) and its related forms, collectively referred to as neuromyelitis optica spectrum disorders (NMOSD), are rare but often severe autoimmune diseases of the central nervous system (CNS). NMOSD predominantly affects women, most commonly between the ages of 30 and 40 years. Aquaporin-4 antibodies (AQP4-IgG) are detectable in approximately 80% of patients with NMOSD.The cardinal clinical manifestations include transverse myelitis, optic neuritis (ON), as well as brainstem and diencephalic involvement. Most NMOSD cases follow a relapsing-remitting course without p

Optic neuritis is an acute inflammatory disease that can cause acute visual loss. We present a case of a 22-year-old female in postpartum with acute headache, retroocular pain, and bilateral amaurosis. Orbit MRI revealed inflammatory changes in both neurovascular optical bundles. The patient required 5-day steroid therapy, presenting complete recovery.

OBJECTIVE: This scoping review will examine the available evidence to identify the main outcomes, their definitions, and how they were measured, along with the values, preferences, and experiences, of adult patients with aquaporin-4 immunoglobulin G antibody (AQP4-IgG)-seropositive neuromyelitis optica spectrum disorder (NMOSD).
INTRODUCTION: NMOSD encompasses a group of inflammatory central nervous system disorders that cause optic neuritis and myelitis. Therapies have different mechanisms of action, but their efficacy has not been fully compared due to variability of outcomes used in random

This article reports two elderly patients with Neuromyelitis Optica Spectrum Disorder (NMOSD) associated with gastric cardia cancer and pancreatic cancer, respectively each presenting distinctive clinical manifestations and diagnostic challenges. Case 1 involved a 69-year-old male who developed stroke-like symptoms after surgery for gastric cardia cancer, which later progressed to optic neuritis, confirmed by positive aquaporin-4 (AQP4) antibodies in both serum and cerebrospinal fluid. Case 2 was a 64-year-old female whose initial presentation included optic neuritis and myelitis. Early cervic

BACKGROUND: Multidrug-resistant tuberculosis (MDR-TB) remains a major global health challenge due to the limited availability of effective therapeutic options. Among second-line agents, linezolid has demonstrated excellent efficacy in both MDR-TB and extensively drug-resistant tuberculosis (XDR-TB), owing to its high oral bioavailability, good tissue penetration, and low resistance profile. However, its long-term use is frequently limited by adverse effects-most notably peripheral neuropathy, along with optic neuritis and myelosuppression-which often lead to early treatment discontinuation and

Heterozygous germline variants in CBL disrupt its function as a negative regulator of the Ras/MAPK pathway, classically predisposing to Juvenile myelomonocytic leukemia (JMML) and moyamoya. We describe two affected siblings carrying a paternally inherited CBL variant (c.1210 T> C, p. Cys404Arg) who exhibited markedly divergent phenotypes, uncovering novel neuroinflammatory and systemic vascular fragility manifestations. The older sibling, a previously healthy 4-year-old girl, presented with acute, rapidly progressive acute disseminated encephalomyelitis (ADEM)-like neuroinflammatory disease po

BACKGROUND: Canine parvovirus (CPV) is a major cause of haemorrhagic enteritis in juvenile dogs, yet its potential ocular effects have not been systematically investigated.
OBJECTIVES: To evaluate whether naturally acquired CPV infection in juvenile dogs is associated with alterations in tear production, intraocular pressure, corneal integrity, fundoscopic findings, and ocular ultrasonographic measurements.
METHODS: Seventy client-owned dogs under 6 months of age were included: 35 with confirmed CPV infection and 35 healthy controls. Before treatment, all animals underwent standardised ophth

Neuromyelitis optica spectrum disorder (NMOSD) may coexist with other autoimmune diseases, including primary Sjögren's syndrome. We report a 52-year-old female presenting with sicca symptoms leading to the diagnosis of primary Sjögren's syndrome based on a positive Schirmer's test, grade III sialadenitis, and anti-SSB antibodies. Further evaluation revealed a history of optic neuritis and paroxysmal tonic spasms involving the limbs, associated with intractable hiccups and acute myelopathy. Magnetic resonance imaging showed brain and spinal lesions suggestive of NMOSD, with negative aquaporin-4

Anti-CRMP5/CV2 antibody disease is a rare cause of optic neuritis, therefore the clinical findings, results of neuroimaging, CSF studies and treatment outcomes have been described in case reports or small case series. Here, we performed a systematic review including all published cases of optic neuritis with positive anti-CRMP5/CV2 antibodies. Twenty-four articles comprising 32 patients were included. Median age was 67 years (60-71); 87.1% of patients had clinically symptomatic bilateral optic neuritis, with 64% having a visual acuity worse than 20/200 and ocular pain was uncommon (4.2%). An a

BACKGROUND: Neuromyelitis optica spectrum disorder (NMOSD) can cause severe neurological disability after a single relapse. Although relapse rates have markedly decreased with the emergence of targeted biologics, treatment switches are increasingly encountered due to long-term treatment considerations and adverse events. However, safe switching strategies, especially within 12 months after a relapse, which we defined operationally as the "cluster phase", have not been established. We investigated relapse risk and potential preventive strategies when switching from complement inhibitors in AQP4

To evaluate the associations and diagnostic performance of inflammatory markers derived from complete blood counts in patients with optic neuritis (ON) and central nervous system (CNS) demyelinating diseases. This retrospective cross-sectional study included 54 patients with ON, 43 patients with CNS demyelinating diseases without ON, and 50 age- and sex-matched healthy controls. The neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), monocyte-to-lymphocyte ratio (MLR), systemic immune-inflammation index (SII), and systemic inflammation response index (SIRI) were compared

INTRODUCTION: Myelin oligodendrocyte glycoprotein (MOG) antibody-associated disease (MOGAD) is a severe, autoantibody-mediated neuroinflammatory syndrome that disproportionately impacts children. Detection of conformation-specific anti-MOG IgG1 in the serum is central to MOGAD diagnosis, yet the pathogenic role of these antibodies remains unclear. We sought to develop a clinically informed model of pediatric MOGAD to study the immunopathogenesis that recapitulates 1) early age of disease onset, 2) anti-MOG IgG1 in serum, and 3) widespread inflammation in the CNS.

PURPOSE: The aim of this study is to highlight findings of retrograde maculopathy (RM) on optical coherence tomography (OCT) as a sequela of neuromyelitis optica associated optic neuropathy.
OBSERVATIONS: We report two patients with demyelinating lesions involving the visual pathways due to neuromyelitis optica in whom RM was seen, and in whom the location of the RM corresponded to their visual deficits. In one case, we describe a patient who had a demyelinating chiasmal lesion with bitemporal hemianopia and who had corresponding binasal RM. In another case, we describe a patient who had gene

BACKGROUND: Gypenosides are neuroprotective in experimental optic neuritis, but human data are lacking.
METHODS: In this single-center, double-blind, placebo-controlled randomized pilot trial, adults with first-episode optic neuritis in the study eye within 28 days received standard corticosteroids and were assigned to gypenosides (180 mg/day for 10 days) or placebo. Planned enrollment was 28, but the trial stopped after 10 randomizations. The primary outcome was peripapillary retinal nerve fiber layer (pRNFL) thickness at 6 months. The registered secondary structural outcome was total macula

BACKGROUND: Myelin oligodendrocyte glycoprotein antibody-associated optic neuritis (MOG-ON) is a subtype of demyelinating optic neuritis (ON) characterized by a considerable risk of relapse; however, the demographic and clinical factors associated with recurrence remain poorly defined, posing ongoing challenges for patient management.
METHODS: A retrospective analysis was conducted on adult-onset MOG-ON patients diagnosed in the Ophthalmology Department of the Chinese People's Liberation Army General Hospital (PLAGH) from January 2019 to January 2024. Patients were divided into two groups bas
