AQP4 and MOG Characterize the Autoantibody Landscape of Checkpoint Blockade-Induced Optic Neuritis.

OBJECTIVE: The objective of this study was to investigate the autoantibody profile in immune checkpoint inhibitor (ICI)-induced optic neuritis (CBON) and identify key autoantibodies for diagnostic and therapeutic purposes. METHODS: In this multicenter retrospective study (January 2020-June 2025), we screened 327 neuro-ophthalmic patients from a bio-repository of 2,321 ICI-treated individuals, identifying 88 patients with CBON across 3 independent cohorts (n = 25, 29, and 34). Longitudinal serum samples (pre-ICI, prodromal, onset, and follow-up) were available for 12 patients. A matched contro

Association of Cerebrospinal Fluid-Restricted Oligoclonal Bands With Relapsing Pediatric Myelin Oligodendrocyte Glycoprotein Antibody-Associated Disease.

BACKGROUND: Cerebrospinal fluid (CSF)-restricted oligoclonal bands (OCBs) are observed in a minority of patients with pediatric myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD). They indicate chronic intrathecal immune activation and thus may be associated with relapsing disease. We aimed to assess the association of CSF-restricted OCBs with relapsing versus monophasic disease in children with MOGAD. METHODS: A retrospective single-center cohort study of children diagnosed between 2017 and 2023 meeting the 2023 international consensus criteria for MOGAD was performed. A

Myelin oligodendrocyte glycoprotein antibody-related autoimmune encephalitis misdiagnosed as acute cerebral infarction: A case report.

RATIONALE: Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is an inflammatory demyelinating autoimmune disorder of the central nervous system that exhibits a broad spectrum of known clinical phenotypes, including optic neuritis, myelitis, acute disseminated encephalomyelitis, encephalitis, meningoencephalitis, and brainstem encephalitis. However, new evidence indicates that a small subset of patients may present with cortical cerebral encephalitis (CCE). When CCE manifests as acute unilateral limb weakness, it may closely mimic acute cerebral infarction (ACI), resulting

Acute Unilateral Vision Loss in a 61-Year-Old Woman: Distinguishing Non-arteritic Anterior Ischemic Optic Neuropathy (NAION) From Optic Neuritis and Papilledema.

Within the population over age 50, non-arteritic anterior ischemic optic neuropathy (NAION) represents a primary etiology for acute, unilateral visual impairment. It shares common features with conditions like optic neuritis and papilledema, making accurate diagnosis difficult. A 61-year-old woman came to AKFA Medline University Hospital with sudden, painless vision loss in her left eye that began four days prior. Her medical history included high blood pressure. An eye examination revealed swelling of the optic disc with blurred edges and decreased vision in her left eye. Evaluation via optic

Enhanced detection of optic neuritis with contrast-enhanced 3D FLAIR in orbital MRI.

PURPOSE: Diagnosing optic neuritis can be challenging, particularly in atypical cases where conventional MRI may be inconclusive. This study evaluates whether adding CE FS 3D FLAIR to standard orbital MRI improves diagnostic performance. METHODS: This retrospective single-center diagnostic accuracy study included 48 patients (mean age, 60 years ± 18; 30 women) clinically suspected of acute optic neuritis who underwent orbital MRI between September 2016 and July 2024. All underwent FS 2D T2WI, CE FS 3D T1WI, and CE FS 3D FLAIR. Definite optic neuritis was defined by combined clinical and parac

Neutrophil extracellular traps exacerbate blood-brain barrier disruption in neuromyelitis optica spectrum disorder.

Neuromyelitis optica spectrum disorder (NMOSD) is an autoimmune inflammatory disease of the central nervous system (CNS) characterized by recurrent optic neuritis and myelitis. Blood-brain barrier (BBB) disruption is a key pathological feature of NMOSD, but the role of neutrophil extracellular traps (NETs) in this process remains poorly defined. Serum proteomic profiling in patients with acute NMOSD revealed enrichment of the NET formation pathway. Neutrophils from these patients showed an increased propensity for NETosis, accompanied by elevated myeloperoxidase-DNA complexes, neutrophil elast

Intravascular large B-cell lymphoma mimicking seronegative neuromyelitis optica spectrum disorder.

Intravascular large B-cell lymphoma (IVLBCL) is a rare aggressive extra-nodal lymphoma characterised by malignant B-cells proliferating within small to medium blood vessels. Often termed the 'great mimicker', it can affect almost any organ with heterogeneous clinical features. Up to half of cases have central nervous system involvement, resembling inflammatory, autoimmune or vascular disease. We report a man in his 60s presenting with saddle paraesthesia, faecal incontinence and bilateral foot drop. MR scan of the spine showed longitudinally extensive transverse myelitis, multifocal supratento

Early rituximab and structured rehabilitation drive rapid functional restoration in steroid- and PLEX-refractory NMOSD: a case report.

INTRODUCTION AND IMPORTANCE: Neuromyelitis optica spectrum disorder (NMOSD) is an antibody-mediated central nervous system disorder, typically presenting with optic neuritis (ON) and longitudinally extensive transverse myelitis (LETM). Detection of anti-aquaporin 4 (AQP4) antibodies establishes the diagnosis, even in patients without classic clinical features. CASE PRESENTATION: A 32-year-old female presented with progressive ascending quadriparesis and sensory deficits. Neurological examination revealed significant hypotonia and weakness, more pronounced in the lower limbs. Spinal magnetic r

Diagnostic Challenges and Clinical Management of Area Postrema Syndrome: A Case Report on Persistent Nausea and Vomiting.

Area postrema syndrome (APS) is a rare neurological syndrome that presents a significant diagnostic challenge due to its nonspecific clinical manifestations, most commonly persistent nausea and vomiting, which frequently lead to initial misdiagnosis as a gastrointestinal disorder. This case report illustrates the diagnostic complexity of APS and underscores the importance of considering central nervous system etiologies in patients with unexplained, refractory gastrointestinal symptoms, particularly in younger individuals. Early recognition through brain magnetic resonance imaging (MRI) and se

Diagnostic dilemma in visual loss due to diabetic papillopathy masquerading as optic neuritis.

Diabetic papillopathy (DP), a rare complication of diabetes mellitus, involves benign changes in the optic nerve head due to metabolic stress or immune dysfunction. This report describes a middle-aged male patient with type 2 diabetes mellitus for 1 year, presenting with painless, progressive blurring of vision in his right eye for 2 weeks. His best corrected visual acuity was 6/18 in the right eye and 6/6 in the left. Examination revealed a sluggish pupillary reaction, grade 1 relative afferent pupillary defect, optic disc hyperaemia, obscured margins and venous tortuosity in the right eye. H

Ophthalmic Immune-Related Adverse Events in Cancer Immunotherapy: Tissue-Specific Mechanisms, Clinical Phenotypes, and Consensus-Based Management.

Immune checkpoint inhibitors (ICIs) have transformed cancer therapy by restoring anti-tumor immunity, but immune activation can disrupt ocular immune homeostasis and induce ophthalmic immune-related adverse events (OirAEs). Although uncommon, OirAEs may involve nearly all ocular compartments and can cause irreversible visual impairment or interruption of effective anticancer therapy. The 2025 international consensus criteria now provide a standardized framework for defining and classifying OirAEs. This review integrates current evidence on ICI-associated ocular toxicity, with emphasis on tissu

Structure function relationships differ between optic neuritis and glaucoma with comparable optical coherence tomography findings.

This retrospective study compared structure-function relationships between patients with optic neuritis (ON) and primary open-angle glaucoma (POAG), focusing on the extent of retinal nerve fiber layer (RNFL) and ganglion cell-inner plexiform layer (GCIPL) damage and its correlation with visual field (VF) defects. We included 194 patients (ON: 47; POAG: 147) referred to Yonsei University Severance Eye Hospital between 2017 and 2023. RNFL and GCIPL thickness, VF indices, and the relationship between structural and functional measures were assessed. Despite comparable RNFL and GCIPL thinning, ON

Genetically predicted association between immune cells and optic neuritis: evidence from a Mendelian randomization study.

PURPOSE: Optic neuritis (ON) is a common inflammatory disease of the optic nerve that usually precedes serious neurological disorders. The purpose of this study was to explore the causal relationship between immune cells and ON. METHODS: We adopted a bidirectional Mendelian randomization (MR) approach, analyzing genetic variants associated with 731 immune cells from a Sardinian cohort and ON data from the FinnGen consortium. Instrumental variables (IVs) for each phenotype were stringently selected based on linkage disequilibrium and genome-wide significance levels. Inverse-variance weighted (

Effectiveness and Safety of Plasmapheresis in MOGAD Attacks: A Systematic Review and Meta-Analysis.

BACKGROUND AND OBJECTIVES: Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is an inflammatory demyelinating disorder characterized by attacks of optic neuritis, myelitis, and brain inflammation. High-dose intravenous methylprednisolone (IVMP) is the most commonly used first-line therapy for acute attacks. However, recovery after IVMP varies, and a substantial proportion of patients require treatment escalation. Thus, we aim to evaluate the effectiveness and safety of plasmapheresis in individuals with acute MOGAD attacks. METHODS: PubMed, Embase, Web of Science, and th

Erdheim-Chester Disease Presenting as Optic Neuritis: A Case Report and Review of the Literature.

We report a case of Erdheim-Chester disease in a patient who presented to the ophthalmology clinic with unilateral vision loss, optic disc swelling, and cluster-type headaches. Erdheim-Chester disease is a rare form of non-Langerhans cell histiocytosis characterized by a xanthogranulomatous process with multisystem organ involvement. The condition can be fatal due to renal or cardiovascular complications. Diagnosis is challenging due to the highly variable presentations and rarity of the disease, as demonstrated in this case. Our patient did not have systemic involvement and tested negative fo

Anti-NMDA-receptor encephalitis and MOGAD associated optic neuritis: a case series.

INTRODUCTION: Anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis is a well-recognized autoimmune condition that often presents with neuropsychiatric symptoms and seizures. Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) often manifests as optic neuritis and, less frequently, as acute demyelinating encephalomyelitis or transverse myelitis. The co-occurrence of anti-NMDAR encephalitis and MOGAD is becoming increasingly recognized, but clinical series remain limited. DESCRIPTION OF CASES: We present three patients with anti-NMDAR encephalitis and MOGAD optic neuritis

Myelin Oligodendrocyte Glycoprotein Antibody-Associated Disease Mimicking Central Nervous System Infection in an Adolescent: A Case Report.

Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) can resemble central nervous system infection in children because fever, headache, vomiting, cerebrospinal fluid pleocytosis, and multifocal brain lesions may coexist. We report a previously healthy 13-year-old boy who developed headache, fever, vomiting, and binocular horizontal diplopia. Examination showed transient exotropia with apparent adduction limitation but no encephalopathy, seizures, limb weakness, pupillary abnormality, optic neuritis, or spinal cord lesion. Cerebrospinal fluid showed mononuclear-predominant pl
