BACKGROUND: This report describes a case of obstructive sleep apnea-hypopnea syndrome (OSAHS) associated with nonarteritic anterior ischemic optic neuropathy (NAION). NAION is an optic neuropathy characterized by painless vision loss, visual field defects, and optic disc edema. It results from hypoperfusion of the short posterior ciliary arteries, leading to acute ischemia and structural and functional impairment of the optic nerve, which ultimately manifests as blurred vision or blindness.
CASE PRESENTATION: We report the case of a 59-year-old Han Chinese male who presented with a 6-day hist

Pattern electroretinography (PERG) is a non-invasive test of macular and retinal ganglion cell function, but its low amplitude and inter-subject variability limit its diagnostic use. This study compares three representations of the transient PERG waveform for the detection of six retinal and optic nerve conditions - Birdshot chorioretinopathy, cone-rod dystrophy, hereditary optic atrophy, macular dystrophy, retinitis pigmentosa and Stargardt disease - against healthy controls, using the publicly available PERG-IOBA dataset. A total of 672 recordings from 336 examinations of 304 participants we

Giant cell arteritis (GCA) is a systemic granulomatous vasculitis of medium and large-sized arteries, which represents an ophthalmic emergency because of the risk of sudden, severe and often irreversible loss of vision. Its most important ocular manifestation is arteritic anterior ischemic optic neuropathy; less frequent manifestations include central retinal artery occlusion, cilioretinal artery occlusion, posterior ischemic optic neuropathy, choroidal ischemia and ocular motor nerve palsies. Giant cell arteritis should be actively considered in every patient over 50 years of age presenting

Mycobacterium avium complex (MAC) lung disease requires prolonged multidrug therapy. We report the case of an 80-year-old woman with MAC lung disease who developed an isolated right abducens nerve palsy after approximately nine months of treatment with moxifloxacin, azithromycin, and ethambutol. The patient presented with horizontal diplopia, and ophthalmological evaluation confirmed sixth cranial nerve palsy. Although ethambutol-induced optic neuropathy was initially considered, clinical findings were not consistent with this diagnosis. Given the temporal relationship with moxifloxacin exposu

Thyroid Eye Disease (TED) often causes proptosis and optic nerve compression, and orbital decompression surgery is commonly performed to relieve these symptoms. However, postoperative anatomical changes are highly nonlinear and patient-specific due to bone removal and orbital content displacement, which makes reliable preoperative prediction both critical and challenging. Existing methods for pre- to postoperative medical image generation often overlook an important clinical prior that, although surgery induces local structural changes, the overall anatomy of the same patient remains largely c

Glaucoma is increasingly recognized as a multifactorial neurodegenerative optic neuropathy in which intraocular pressure (IOP) is one of several stress amplifiers rather than a sufficient etiological explanation. Vascular dysregulation, mitochondrial and metabolic vulnerability, oxidative stress, excitotoxicity, glial dysregulation, and persistent neuroimmune activation can converge on retinal ganglion cell (RGC) and optic nerve injury. This broader framework is particularly relevant to normal-tension glaucoma (NTG), which represents a large proportion of primary open-angle glaucoma in Asian p

Thyroid eye disease (TED) is an autoimmune inflammatory orbitopathy that can threaten vision in severe cases due to dysthyroid optic neuropathy (DON). While TED is primarily diagnosed clinically, orbital imaging with computed tomography (CT) and magnetic resonance imaging (MRI) plays a pivotal role in confirming the diagnosis in atypical presentations, distinguishing TED from other orbital diseases, assessing disease activity, and guiding management. This narrative review examines the role of CT and MRI in the diagnosis and management of TED, with particular focus on their application in diffe

Neuro-ophthalmologic emergencies presenting with acute visual loss pose significant diagnostic and therapeutic challenges, with potential for irreversible vision loss if not promptly recognized. A comprehensive approach that integrates clinical evaluation with advanced imaging modalities is essential for timely diagnosis, prognostic stratification, and management. Magnetic resonance imaging remains the gold standard for demyelinating optic neuritis, enabling differentiation among multiple sclerosis, neuromyelitis optica spectrum disorder, and myelin oligodendrocyte glycoprotein antibody-associ

Optical coherence tomography angiography (OCT-A) has emerged as a promising imaging technique within the field of ophthalmology. This review explores the potential applications of OCT-A in neuro-ophthalmology for both diagnostic and prognostic purposes. To fully realize the benefits of OCT-A in this domain, further investigation into the characteristics and natural history of the optic disc vasculature in neuro-ophthalmic disorders is essential. Future direction includes standardizing OCT-A protocols to facilitate cross-comparison and help integration of OCT-A in the evaluation of individuals

BACKGROUND: Progesterone signalling is a key determinant of meningioma behaviour, particularly in skull base tumours. Cyproterone acetate-associated meningiomas stabilise or regress after withdrawal Whether similar reversibility follows cessation of depot medroxyprogesterone acetate (DMPA), a widely used long-acting injectable contraceptive, is poorly characterised.
METHODS: Retrospective case series of three women with intracranial meningiomas who discontinued DMPA following neurosurgical assessment, assessed with post-contrast CT (Cases 1 and 2) or magnetic resonance imaging (Case 3).

Quantification of retinal ganglion cell (RGC) loss provides a sensitive and reproducible measure of neurodegeneration in mouse models of neurological disease and injury. In experimental autoimmune encephalomyelitis (EAE), a widely used mouse model of multiple sclerosis, inflammation of the optic nerve can lead to optic neuritis, axonal injury, and progressive RGC loss. Therefore, quantification of RGC survival provides a valuable readout of inflammation-mediated neurodegeneration and can be used to evaluate disease severity and neuroprotective interventions. Here, we describe a protocol for r

PURPOSE: Optic neuropathies involve progressive loss of retinal ganglion cells (RGCs) and their axons, leading to irreversible vision deficits. While pretreatment with drugs or gene therapy that upregulate sirtuin 1 (SIRT1), an NAD⁺-dependent deacetylase, has shown neuroprotective effects, its effectiveness when administered after disease onset remains uncertain. We investigated whether adeno-associated virus (AAV)-mediated SIRT1 overexpression in RGCs can provide post-onset neuroprotection in preclinical models of optic neuropathy.
METHODS: RGC degeneration was induced in C57BL/6 mice (8-10/

Normal-tension glaucoma (NTG) is a distinct form of glaucomatous optic neuropathy characterized by progressive retinal ganglion cell (RGC) degeneration and visual field loss despite intraocular pressure (IOP) remaining within the statistically normal range. Although IOP reduction remains the only established disease-modifying strategy, continued disease progression despite adequate IOP control highlights the importance of IOP-independent mechanisms. Increasing evidence implicates vascular dysregulation, mitochondrial dysfunction, oxidative stress, impaired autophagy, neuroinflammation, axonal
