Two Novel Variants in MT-RNR1 Gene Associated with Hereditary Optic Neuropathy: A Case Report.

INTRODUCTION: Hereditary optic neuropathies are primarily disorders of mitochondrial dysfunction leading to the metabolic failure of the highly energy-dependent retinal ganglion cells. Beyond the canonical variants found in OPA1 or the mitochondrial genome, a growing subset of patients exhibits progressive optic atrophy of unknown origin. Identifying these variants is critical, as they might lead to a diagnostic delay. We report 2 cases with previously unreported MT-RNR1 variants, potentially contributing to a hereditary optic neuropathy. CASE PRESENTATIONS: A 55-year-old woman with painless

Optic Atrophy in Wolfram Syndrome Type 1: A Retrospective Analysis of Visual Outcomes and Biomarker Correlates.

BACKGROUND: Wolfram syndrome type 1 (WS1) is a rare autosomal recessive disorder classically associated with diabetes mellitus (DM) and optic atrophy (OA). We aimed to characterize OA in WS1 and evaluate optical coherence tomography (OCT) and genetic biomarkers as tools for disease monitoring and prognostication. METHODS: We conducted a retrospective chart review of genetically confirmed patients with WS1 seen at Washington University or Indiana University neuro-ophthalmology clinics between July 2017 and 2024. Data included demographics, clinical history, best corrected visual acuity (BCVA),

BLOC1S1 variants cause lysosomal and autophagic defects resulting in a hypomyelinating leukodystrophy with epileptic encephalopathy.

BLOC1S1 encodes a subunit shared by the BLOC-1 and BLOC-one-related complex (BORC) hetero-octameric complexes that regulate various endolysosomal processes. Here, we report the identification of seven distinct variants in BLOC1S1 in 11 individuals from seven independent families presenting with early psychomotor delay, hypotonia, spasticity, epileptic encephalopathy, optic atrophy, and leuko-axonopathy with hypomyelination. A subset of the affected individuals also have features of hypopigmentation and ocular albinism that are similar, although milder, than those of individuals with BLOC-1-rel

Frequency and Hearing Loss Phenotypes of OPA1 Variants in a Cohort of 18,475 Patients with Hearing Impairment.

BACKGROUND/OBJECTIVES: The OPA1 gene encodes a dynamin-related GTPase essential for mitochondrial fusion. Variants in OPA1 are a major cause of autosomal dominant optic atrophy (DOA). A subset of DOA patients exhibits hearing loss, often manifesting as auditory neuropathy spectrum disorder (ANSD). In this study, we aimed to describe the frequency of OPA1-related hearing loss in a large cohort of patients with hearing loss and to explore the genotype-phenotype correlations and appropriate interventions. METHODS: A total of 18,475 Japanese patients with hearing loss were recruited. Targeted mas

Natural history of 15 patients with autosomal dominant WFS1 pathogenic variants associated with sensorineural hearing loss and optic atrophy.

OBJECTIVE: Autosomal dominant pathogenic variants in WFS1 cause a spectrum of disorders with phenotypic manifestations including low-frequency sensorineural hearing loss, optic nerve atrophy accompanied by low- to mid-frequency sensorineural hearing loss, non-syndromic diabetes mellitus, and early-onset cataracts. These conditions are generally milder than autosomal recessive Wolfram syndrome, except for Hattersley-Urano syndrome. Despite growing recognition of these disorders, data on clinical progression and long-term outcomes remain limited. This study aims to expand knowledge on disease se

Irreversible blindness from hydrocephalus in recurrent pediatric pilocytic astrocytoma: a case report.

BACKGROUND: Pilocytic astrocytoma (PA) is the most common pediatric brain tumor, typically associated with favorable outcomes following surgical resection. However, irreversible vision loss due to elevated intracranial pressure (ICP) in posterior fossa PA is rare and often underrecognized. This case highlights the consequences of delayed detection of tumor recurrence and the critical importance of long-term surveillance in pediatric brain tumor survivors. CASE PRESENTATION: A 14-year-old male with a history of posterior fossa PA resection at age 4 presented with progressive headache, vomiting

Early hypotonia and visual regression as presenting features of peroxisome biogenesis disorder: an Egyptian case report.

INTRODUCTION: Peroxisome biogenesis disorders (PBDs) are rare autosomal recessive neurodegenerative diseases caused by variants in Peroxin (PEX) genes, leading to defective peroxisome assembly and multisystem dysfunction. The PEX12 variant NM_000286.3: c.1047_1049del (NP_000277.1: p.Gln349del) has been described almost exclusively in Egyptian patients, suggesting a founder effect. We present the first Egyptian case with this variant showing bilateral optic atrophy, expanding the known clinical phenotype of PEX12-related PBD. OPA1 in MC4R Neurons Regulates Dietary Fat Intake and Body Weight in Mice.

Melanocortin 4 receptor (MC4R) neurons in the hypothalamus regulate appetite and energy balance, and mitochondrial dynamics are important for neuronal function. However, how dietary fat intake affects mitochondrial regulation in the hypothalamus has not been fully clarified. Here we examined the effects of soybean oil intake on hypothalamic mitochondrial gene expression and generated MC4R neuron-specific mitochondrial fusion factor OPA1 (Optic atrophy-1) knockout mice. Voluntary ingestion of soybean oil increased hypothalamic OPA1 expression in wild-type male mice but not in female mice. In co

OPA1 Requires ATP Synthase Activity and Oligomerization to Mitigate Prion-Induced Mitochondrial Dysfunction, Oxidative Stress, and Neuronal Apoptosis.

Prion diseases are lethal neurodegenerative disorders marked by mitochondrial impairment, oxidative stress, and neuronal apoptosis. Our previous work identified optic atrophy protein 1 (OPA1) as a promising point of intervention, yet the underlying mechanisms are poorly understood. In the present work, we showed that OPA1-mediated protection depends on ATP synthase activity and oligomerization. In N2a cells, PrP106-126 exposure reduced the protein expression levels of OPA1 and key ATP synthase subunits, including the catalytic subunit ATP5A and the oligomerization-associated subunit ATP5k, whe

The NR2F1-Related 5q14.3-q21.1 deletion causing periventricular heterotopia with cerebral visual impairment: a longitudinal case report and genotype-phenotype analysis.

Large interstitial deletions spanning chromosome 5q14.3 to q21.1 and encompassing NR2F1 are rare. Existing descriptions have largely focused on structural features and presenting manifestations, with fewer reports examining functional neurodevelopmental outcomes over time, particularly in individuals with large interstitial deletions. Here, we present a 16-year longitudinal analysis of an individual with a de novo 13.58 Mb interstitial deletion of 5q14.3 to q21.1 encompassing NR2F1 (ClinVar accession SCV007328941), consistent with NR2F1-related neurodevelopmental disorder, historically describ

Intracranial Pressure Reduction Is Associated with Mitochondrial OPA1 and Cytochrome c Release in the Retinas of AQP1-Null Mice.

BACKGROUND: Recent studies strongly suggest that low intracranial pressure (ICP) may be involved in the pathogenesis of glaucomatous optic neuropathy. As retinal ganglion cells (RGCs) are highly susceptible to mitochondrial dysfunction, mitochondrial injury may be associated with optic neuropathy related to reduced ICP. In this study, aquaporin-1 (AQP1)-null mice were used to investigate whether reduced ICP is associated with alterations in mitochondrial structure and the release of optic atrophy type 1 (OPA1) and cytochrome c from mitochondria. METHODS: Intraocular pressure (IOP) and ICP wer
