We present a middle childhood boy with type 1b diabetes mellitus (defined as insulin-dependent diabetes mellitus without evidence of autoimmunity) diagnosed at the age of 4 years who was on a basal bolus insulin regimen. Identifying the presence of bilateral optic atrophy in him during an ophthalmological evaluation led to a closer clinical assessment for Wolfram syndrome. Although there was no hearing loss on the audiogram and no evidence presently of arginine vasopressin (AVP) deficiency, we proceeded with genetic testing by next-generation sequencing. This confirmed the diagnosis of Wolfram

BACKGROUND: Wolfram syndrome is a rare genetic disorder characterized by antibody-negative early-onset atypical diabetes mellitus, optic nerve atrophy, sensorineural hearing loss, central diabetes insipidus (arginine vasopressin deficiency), and progressive neurodegeneration, with significant variability in disease severity. We assessed the accuracy of a genotype-based severity scoring system for predicting the onset of cardinal symptoms in Wolfram syndrome.
METHODS: The scoring system is based on the type of WFS1 variants (in-frame or out-of-frame) and their location relative to transmembran

INTRODUCTION: Dominant optic atrophy (DOA) is an inherited mitochondrial disorder characterized by retinal thinning and progressive visual loss. When accompanied by additional neurological or systemic features, such as progressive external ophthalmoplegia, myopathy, or deafness, it is classified as DOA-plus (DOA+). Although central nervous system involvement has been associated with cortical and cerebellar atrophy, specific regional patterns remain unclear. This study aimed to investigate cortical lobe alterations in DOA+ patients and examine the association between retinal thinning and struct

Primary mitochondrial disorders are clinically and genetically heterogeneous and remain underdiagnosed in resource-limited settings. We performed a retrospective observational study (March 2016- January 2024) at a tertiary neurology center in Eastern India to characterize the clinical, biochemical, neuroimaging, electrophysiological, and molecular features of suspected mitochondrial disease and to explore interpretable machine-learning approaches for syndromic stratification. Forty-eight patients from 42 unrelated families were classified as mitochondrial encephalomyopathy, lactic acidosis, an

BACKGROUND: PDXK-related neuropathy is a rare form of hereditary motor and sensory neuropathy with optic atrophy, caused by biallelic variants in the PDXK gene. To date, only four missense variants have been reported in 13 patients from six families. Unlike most inherited neuropathies, which currently have only symptomatic treatments, this type of neuropathy can be managed with pyridoxal 5'-phosphate supplementation.
METHODS: The clinical presentation of the patient was evaluated twice, at ages 14 and 15 years. The whole-exome sequencing was performed for the proband followed by functional st

Biallelic pathogenic variants of the Reticulon 4 interacting protein 1 (RTN4IP1) gene are responsible for optic atrophy, either isolated or associated with ataxia, mental retardation, and seizures. They are identified as a cause of hereditary optic neuropathy in 7% of patients diagnosed before the age of 20. We have built a dataset for this gene by collating all the clinical cases available in the literature, and unpublished patients diagnosed at our centre, using standard nomenclature to describe both the molecular and phenotypic features. We performed a comprehensive data analysis, based on

This study aims to investigate the action mechanism and target molecules of the ultrafiltration membrane extract of Angelicae Sinensis Radix and Hedysari Radix in improving renal tissue injury in rats with diabetic kidney disease(DKD) from the perspective of mitochondrial function. Ten SPF-grade male Wistar rats were selected as the control group, while 40 rats were fed a high-fat and high-sugar diet for four weeks followed by a single intraperitoneal injection of streptozotocin(STZ) 35 mg·kg~(-1) to establish the DKD model. The successfully modeled rats were then randomly divided into four gr

The simultaneous occurrence of Arnold-Chiari Malformation Type II (ACM II), syringomyelia, and an acquired tracheoesophageal fistula (TEF) in a young adult is exceptionally rare and represents a major diagnostic challenge. Arnold-Chiari Malformation II is characterized by the herniation of cerebellar structures through the foramen magnum. It usually presents in infancy in association with myelomeningocele; co-existing TEF and syringomyelia in an adult, as in our patient, is seldom documented. Notably, the present case represents a Chiari II phenotype without spinal dysraphism, lying outside th

Diabetic erectile dysfunction (DMED) is a prevalent complication of diabetes mellitus, with mitochondrial dysfunction playing a central pathogenic role. Current treatments inadequately address these subcellular pathological mechanisms. This study aimed to develop a mitochondria-targeted nanotherapeutic platform for efficient optic atrophy 1 (OPA1) protein delivery to restore mitochondrial homeostasis in cavernous tissue. A triphenylphosphonium (TPP)-functionalized Fe3O4-ferric tannic acid (FeTA) nanocarrier was synthesized and loaded with recombinant human OPA1 to form Fe3O4-FeTA-TPP-OPA1 (OFF

PURPOSE: Older-onset Leber hereditary optic neuropathy (LHON) may present without obvious optic disc swelling, which can delay diagnosis. We explored whether acute-phase retinal nerve fiber layer (RNFL) thickening on optical coherence tomography is less prominent with increasing age in genetically confirmed LHON.
METHODS: We retrospectively reviewed patients with m.11778G > A LHON who underwent OCT within 90 days of symptom onset. Circumpapillary RNFL thickness was measured using ZEISS CIRRUS HD-OCT. Age-adjusted ΔRNFL was calculated as measured RNFL thickness minus age-expected thickness der

Pathogenic variants of the mammalian optic atrophy 1 (OPA1) protein, a key regulator of mitochondrial fusion, may lead to severe mitochondrial dysfunction and morphological alterations, and have also been proposed to be involved in remodeling the mitochondrial membranes lipid profile. To address this last issue, a lipidomic workflow based on HILIC-ESI-HRMS was applied to profile major classes of mitochondrial membrane phospholipids (PL), namely phosphatidylcholines (PCs), -ethanolamines (PEs), and -inositols (PIs), along with cardiolipins (CLs), in mouse embryonic fibroblasts knocked out for O

INTRODUCTION: Wolfram syndrome is a rare autosomal recessive disorder caused by pathogenic variants in the WFS1 gene and characterized by early-onset, insulin-dependent diabetes mellitus, optic atrophy, sensorineural hearing loss, arginine vasopressin deficiency, and progressive neurodegeneration driven by chronic endoplasmic reticulum (ER) stress; no proven disease-modifying therapy currently exists. Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) augment glucose-dependent insulin secretion and, in preclinical models, preserve β-cell mass, attenuate ER stress, and confer neuroprotection

Spaceflight-Associated Neuro-Ocular Syndrome (SANS) has remained poorly understood throughout the history of human spaceflight despite being classified as the largest physiologic barrier that astronauts face. Existing mechanistic explanations of SANS are limited in accounting for its tissue specificity, interindividual heterogeneity, and persistence beyond return to Earth, which has left countermeasure development without a coherent biological target. Through integration of tissue-resolved murine spaceflight transcriptomic datasets with human retinal ganglion cell models of Leber Hereditary Op

BACKGROUND: Bosch-Boonstra-Schaaf optic atrophy syndrome (BBSOAS) is a rare autosomal dominant neurodevelopmental disorder caused by pathogenic variants in the NR2F1 gene. The syndrome is characterized by a complex phenotype including optic nerve atrophy, global developmental delay, intellectual disability, and seizures. We report a patient with this syndrome whose prominent clinical presentation included developmental and epileptic encephalopathy (DEE). Due to overlapping clinical features with other neurodevelopmental disorders, clinical diagnosis remains challenging, necessitating molecular

This review is devoted to Leber's hereditary optic neuropathy (LHON), a rare maternally inherited ophthalmic disorder presenting as optic nerve atrophy and associated with an unfavorable prognosis. LHON is the most common primary mitochondrial DNA disorder, and most patients (more than 90% of cases) carry one of three point mutations. The 11778G>A mutation in the MT-ND4 gene is the most common worldwide and causes the most severe variant of the disease. LHON typically manifests as bilateral, painless, acute or subacute significant vision loss, most often in young men. Environmental factors pla
